Aetna is Payscale's new medical and prescription carrier. For 2023, you have a choice of three medical plans with a range of coverage levels and costs. This gives you the flexibility to choose what's best for your needs and budget. Active enrollment is required for medical.
CDHP with HSA $1,500 - a high-deductible health plan that puts you in charge of your spending through lower paycheck contributions, higher deductibles, and a tax-advantaged Health Savings Account (HSA).
Base PPO $1,000 - a preferred provider organization plan that reduces your out-of-pocket responsibility when you need care by offering a lower deductible and copays.
Buy-Up PPO $500 - a preferred provider organization plan that has the lowest deductible, giving you the most protection from out-of-pocket expenses when you need care, but costs the most from your paycheck.
Aetna Concierge
Payscale employees enrolled in one of the Aetna medical/Rx plans have access to a dedicated service team Monday - Friday 8 am to 6 pm (local time) by calling 1-800-331-0596 (also found on your Aetna ID card) or going online to www.Aetna.com and log in to your member website.

Each medical option includes coverage for prescription drugs.
Prescription drug benefits are provided through Aetna/CVS Caremark, with a pharmacy network that includes more than 74,000 pharmacies nationwide.
These include not only participating CVS pharmacies but also Walgreens, Walmart and many other pharmacy chains, as well as more than 20,000 independent pharmacies. For a list of covered drugs, you can either call Aetna/CVS Caremark at 1-800-331-0596 or go to the website.

Delta Dental is Payscale’s new dental carrier. For 2023, you have a choice of two dental options provided through Delta Dental. The plan provides the same benefits coverage for both in-network and out-of-network providers. However, it's usually to your advantage to use an in-network dentist because they will generally charge you lower fees. Using in-network providers also protects you from being balance-billed for services (charged more than the reasonable and customary amounts for services rendered).
You can choose the option that's right for you. For example, if you don't need orthodontic care (braces) or major restorative care, the Base Plan may be all you need. You can contact Delta Dental at 800-554-1907 (Monday - Friday from 7am to 5pm PT) or www.deltadentalwa.com.
This plan helps pay for a wide range of vision-related services and products, including periodic eye exams, eyeglasses and contact lenses for you and your covered dependents.
Your vision care plan is provided through VSP, which has a large network of vision care providers. You can check if your provider is in the network at www.vsp.com. Services and supplies are covered once every 12 months unless otherwise noted.
You'll pay the cost of vision coverage with before-tax dollars. How much you pay is based on the dependents you cover. You can enroll any combination of you, your spouse/domestic partner, and your children.
If you enroll in the Aetna CDHP-HSA plan, you're required to open a Health Savings Account (HSA) at a minimum annual contribution of $0 in order to receive Payscale's contribution to your account. You can increase, decrease or stop your HSA contributions at any time.
For 2023, Payscale will make the following contributions to your account:
An HSA is a tax-free savings account you can use to pay for eligible health expenses anytime, even in retirement. You can make before-tax contributions from your paycheck into your HSA, allowing you to save money by using tax-free dollars to pay for eligible medical, prescription, dental, and vision expenses. The total amount that can be contributed to your HSA each year is limited by the IRS. The following limits for 2023 include any company contributions you receive from Payscale:

Tax-advantaged FSAs are a great way to save money. The money you contribute to these accounts comes out of your paycheck without being taxed, and you withdraw it tax free when you pay for eligible healthcare and dependent care expenses. Payscale offers you the following FSAs:
Health Care FSA
When you enroll in a Health Care FSA, WEX will send you a debit card, which you can use to pay for eligible expenses such as medical plan deductibles, copays and coinsurance. You can contribute up to $3,050.
Dependent Care FSA
Use your Dependent Care FSA to pay for eligible dependent expenses such as care for a child so you and/or your spouse can work, look for work, or attend school full time. You can contribute up to $5,000, or $2,500 if you are married and filing separately.
Limited Purpose FSA
Use your Limited Purpose FSA to pay for dental and vision expenses. You can contribute up to $3,050. Note: if you enroll in the CDHP and have an HSA, you're not eligible to open a Health Care FSA, but you can open a Limited Purpose FSA.

Commuter Benefits
Another great way to save money through tax-free spending is Payscale’s commuter benefits program. You can use before-tax dollars to pay for monthly parking or transit costs related to your work commute. It’s easy and flexible. Before-tax deductions are allowed up to the IRS 2023 limit of $300 per month for parking or transit. Enroll or make changes by the 25th of each month via employee self-service through ADP at www.workforcenow.adp.com.
The Payscale 401(k) Retirement Savings Plan, offered through Empower, provides advantages you may not get with other types of savings plans and helps you meet one of life's important goals - saving for a financially secure retirement. Saving for retirement is important for your financial future, whether you're retiring soon or years from now. The Payscale 401(k) Savings Plan is designed to assist you in meeting your retirement goals.
You can contribute to your 401(k) with before-tax money or Roth after-tax money. The type of contributions you make will depend on your financial goals and circumstances.
Both before-tax and Roth after-tax contributions count toward the IRS maximum of $22,500 in 2023. You also may not contribute more than 50% of your income.
If you're 50 or older, you may make additional catch-up contributions - up to $7,500 in 2023.
Auto-enrollment is at 4% of your base salary with a company match of 50% on the first 4% of your base salary (or 2% base salary match). Eligible pay includes your base salary, not including any bonus or commissions.
You're always 100% vested in your own contributions and you're immediately vested in Payscale's contributions.

EMPLOYEE BASIC LIFE AND AD&D
Payscale automatically provides you with a $50,000 basic life insurance and a $50,000 accidental death and dismemberment (AD&D) policy. AD&D policies pay benefits for certain serious losses due to an accident.
VOLUNTARY LIFE AND AD&D INSURANCE
You can also elect voluntary life and AD&D insurance for you, your spouse or domestic partner and your dependent children. If you enroll within 31 days of your first day of employment or qualifying life event, you can avoid completing an evidence of insurability (EOI) form by selecting the guarantee-issue amounts ($150,000 for employees and $25,000 for qualifying spouse coverage).
HAVE YOU NAMED A BENEFICIARY?
Be sure you’ve selected a beneficiary for all your life and accident insurance policies. Your beneficiary will receive the benefit paid by a policy in the event of the policyholder’s death. It’s important to designate a beneficiary and keep that information up to date. You can add or change a beneficiary through ADP at www.workforcenow.adp.com.
Payscale offers you two disability plans at no cost to you. The loss of income due to illness or disability can cause serious financial hardship for your family. Payscale's disability insurance programs work together to replace a portion of your income when you're unable to work. The disability benefits you receive allow you to continue paying your bills and meeting your financial obligations during this difficult time.
Short-Term Disability begins 15 days after disability and pays 60% of your weekly salary up to a maximum of $2,000 for 11 weeks. STD is a company-paid benefit.
Long-Term Disability begins 91 days after disability and pays 60% of your monthly salary up to a maximum of $10,000 per month until you recover or reach full social security retirement age. LTD is a company-paid benefit.
