Regular dental care is an important part of caring for your health, and keeping your teeth fit may protect you from major expenses in the future. If you enroll in one of QSC's Anthem HSA PPO plans, you are eligible to enroll in a Health Savings Account. An HSA is a savings account used to put pre-tax dollars aside specifically for your out-of-pocket health expenses. Like a regular savings account, your HSA earns interest and is yours to keep if you leave the company (not that we want you to do that!).
Please note you may not have other coverage to be eligible to contribute or receive employer contributions into your HSA. Other coverage includes: Medicare Part A or Part B, Tricare, and any other coverage including spouse enrolled in a full-purpose Healthcare FSA.

A Healthcare Flexible Spending Account (FSA) is a pre-tax savings account that can be used for out-of-pocket healthcare expenses, such as medications and co-payments.
A Limited Purpose Healthcare Flexible Spending Account (FSA) is a pre-tax savings account that can be used for out-of-pocket dental and vision expenses only. Eligible for employees enrolled in one of QSC's HSA PPO plans.
A Dependent Care Reimbursement Account is a type of Flexible Spending Account that allows you to set aside pre-tax dollars that you can use to pay eligible daycare expenses for children or a disabled spouse or dependent. Unused funds in your Dependent Care Reimbursement Accounts are forfeited at the end of the plan year.
The money put into FSA is taken from your pay before federal, state or social security taxes.

You are encouraged to participate in QSC’s 401(k) plan. This plan is a retirement savings plan designed to allow eligible employees to supplement any existing retirement and pension benefits by saving and investing through a voluntary salary contribution up to the IRS maximums. Contributions and any earnings on contributions are tax-deferred until money is withdrawn.
You also have an option to contribute to a Roth account. Roth contributions are made with after-tax dollars, as opposed to the pre-tax dollars you contribute to a traditional 401(k). In other words, with the Roth option, you've already paid income taxes on money you contribute. With the traditional 401(k), your contribution is made on a pre-tax basis and you pay income taxes only when you take a distribution.

This policy offers a cash benefit if you suffer certain injuries, undergo specific treatments, or incur other itemized expenses resulting from an accident. If elected, your premiums are paid through payroll deductions on an after-tax basis.
This policy is portable, which means that you can keep it should you change jobs or retire, with no increase in premiums.

Cigna’s Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-threatening illness. You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as invasive cancer, a heart attack, stroke, kidney failure, paralysis, blindness, and more.
If you are no longer in your initial eligibility period, you may enroll in the Critical Illness insurance anytime during the year as long as you provide proof of good health. To provide proof of good health, you will be asked to complete a health questionnaire (Evidence of Insurability) and are subject to insurance carrier approval. Cigna may approve or decline coverage based on a review of your health history.

During our annual open enrollment period, you have the option to enroll for the first time, add/remove dependents, change plans, or opt out of plans.
There will be no changes to our benefit vendors this year, however, you will still need to log into UGK to confirm your elections. Remember your FSA and HSA contributions need to be updated every year, they will not roll over to the new plan year.
