QLEs include, but are not limited to: birth or adoption, marriage or divorce, change of dependent status, change in employment status, loss of other group coverage, enrollment in other coverage, and death of a spouse or dependent. Click here to learn more about QLEs and how to update your benefits.

MEDICAL, PRESCRIPTION DRUG, DENTAL & VISION
Medical, dental, and vision coverage are at the core of our team member benefits. They give you access to the care you need, when you need it. Each plan offers annual preventive care to protect your long-term health—and your wallet—by identifying risks that may turn into serious—and expensive—conditions down the road. They also offer diagnostic care and treatment, should you need it.
Medical Coverage
Nothing is more important than ensuring your family has the coverage it needs to stay healthy! That’s why we offer a range of medical plan options so you can find the right fit for your personal situation.
HICV gives you three medical plan options to choose from:
- Core Plan – A great option that provides low copays for most routine care, such as seeing a doctor or getting lab work. More complex care such as inpatient stays or surgery are subject to a higher deductible. This plan has the lowest bi-weekly premium.
- Consumer Plan with HSA – This option has a higher deductible that applies to all care (excluding preventative care, which is free!). Once the deductible is met, a co-insurance will be applied to claims. This plan is eligible for a Health Savings Account with a contribution from HICV, to cover these out-of-pocket expenses.
- Core Plus Plan – This option has the lowest deductible and copays, but the highest bi-weekly premium. Core Plus offers the most comprehensive coverage, including those expensive and complex care needs like surgery.
For additional information on medical coverage, please refer to:
- Medical Coverage At-a-Glance for details of the coverage each plan offers.
- Supplemental Health Benefits for ways to supplement the coverage option you choose to protect yourself from potentially high costs.
- Saving & Spending Accounts for the opportunity to lower your taxable income by putting aside money to pay eligible healthcare expenses.
- Additional Resources for Summary Plan Descriptions and Summaries of Benefits Coverage.

To use OneGuide, simply call (800) 244-6224 or use the myCigna mobile app to start a virtual chat session from 9 a.m. to 8 p.m. ET. (Apple App Store or Google Play)
Prescription Drug Coverage
Your prescription drug coverage is managed by Express Scripts®. The amount you pay for each prescription is based on the drug tier and whether you choose a retail pharmacy or home delivery.
After you’ve filled a 30-day supply of a long-term medication twice at a non-Walgreens/CVS retail pharmacy, Express Scripts will automatically enroll your prescription for 90-day refills. Call Express Scripts to let them know if you would like to:
- Move your prescription to be filled at your local Walgreens or CVS;
- Use the mail-order pharmacy for home delivery and automatic refills; or
- Continue to fill your prescription for 30 days at a time at a local non-Walgreens/CVS pharmacy.
Price Assure – powered by GoodRx® – does the comparison shopping for you. You may find a better price for prescriptions, even when you’re using your insurance. Check it out.

Dental Coverage
Regular dental care is critical to your overall health. Take care of your teeth, and they’ll take care of you!
Your choices:
- Core Dental Plan – The option to choose if you expect to need less extensive care.
- Dental Plus Plan – Offers more comprehensive coverage, including orthodontia.
When you use in-network providers, you’ll pay less for care and your dentist will submit your claims for you. Out-of-network providers will likely cost you more, and you’ll have to submit your own claims.
Virtual Care
HICV offers virtual dental care to help when you’re unable to see your regular provider. Simply log on to your mycigna.com account and follow the prompts to the virtual care portal. After you enter some basic health information, you'll be connected with a dentist in ten minutes or less.
Enroll in mycigna.com or more information, including plan information, oral health assessments, claim information and discounts on a variety of health and wellness services.

Vision Coverage

In addition, we all need to protect our eyes from blue light emitting screens and the sun. The VSP Vision Care Plan can help keep your eyes healthy even if you have 20/20 vision. There’s coverage for everyone!
Coverage comes with a comprehensive vision exam. Then, you choose either prescription eyewear or to use your frames and lenses allowances for ready-to-wear:

Visit vsp.com or call (800) 877-7195 to find in-network providers near you and for additional information.

HEALTH SAVINGS & SPENDING ACCOUNTS
KEEP SCROLLING TO FIND INFORMATION AND RESOURCES ON:
Health Savings Accounts & Flexible Spending Accounts
HEALTH SAVINGS & SPENDING ACCOUNTS
HICV offers two types of tax-advantaged accounts, and the account you are eligible for depends on the medical plan option you enroll in:
-
The Health Savings Account (HSA) is available when you enroll in the Consumer Plan with HSA option; and
-
The Healthcare Flexible Spending Account (Healthcare FSA) is available when you enroll in the Core Plan or Core Plus Plan through HICV or an outside medical plan that is not a high deductible plan.
Review the HSA vs. FSA At-a-Glance to compare how these accounts work.
A “tax-advantaged account” means the money you deposit is taken from your paycheck before taxes are calculated. So, you don’t pay taxes on the amount you deposit in your HSA or Healthcare FSA and you lower the amount you’re taxed on.
Health Savings Account
If you enroll in the Consumer Plan with HSA , you’ll be eligible to open and contribute to a Health Savings Account (HSA). An HSA is a savings account owned by you that allows you to set aside pre-tax dollars to pay for eligible medical, prescription drug, dental and vision expenses for you and your enrolled dependents. You can also save your balance to pay for expenses incurred in the future, even as far in the future as retirement. You never forfeit your balance, even if you leave the company or switch medical plans.
After you’ve opened your account with Optum, you can choose how much to contribute through payroll deductions by completing the “I want to contribute to my HSA” life event in UKG. Contributions can be updated any time throughout the year.
HICV Contributions
As an extra contribution to your healthcare coverage, HICV will contribute to your HSA:
- Up to $500 per year or $19.23 bi-weekly for employee only coverage; and
- Up to $750 per year or $28.85 bi-weekly if you cover any dependents.
To receive employer contributions, you must open an HSA through Optum Bank at optumbank.com or by calling (866) 234-8913. Please reference group #3337320.
IRS Maximums
Under IRS rules, the maximum amount that can be deposited into your HSA (combining your contributions and HICV’s contributions) is:
Calendar Year 2026:
- $4,400, if you have employee only medical coverage; or
- $8,750, if you cover any dependents.
If you are at least 55 years old—or will turn 55 any time in the calendar year—you can make an additional $1,000 contribution to an HSA.
Note: If you are currently contributing to the Health Care Flexible Spending Account (or another general-purpose healthcare FSA), and choose to participate in the HSA in 2026, you must exhaust your full balance in the FSA before January 1, 2026, in order to enroll in the HSA.
Flexible Spending Accounts
The Healthcare Flexible Spending Account (FSA) through Optum is a great way to use pre-tax dollars to pay for predictable healthcare expenses.
Note: If you enroll in the Consumer Plan with HSA or a similar high deductible health plan, you have access to a tax-advantaged Health Savings Account and cannot participate in the FSA.
How the FSA Works
- You must be enrolled in either the Core Plan or Core Plus Plan through HICV or an outside medical plan that is not a high deductible plan.
- You can contribute up to $3,400 in 2026 to use for eligible medical, prescription drug, dental, and vision expenses.
- Estimate your annual contribution carefully. FSAs follow a “use it or lose it” rule, which means any balance left after the grace period for claims and carryover to the following the plan year is forfeited.
- The grace period for claims lets you submit claims until March 31, for expenses incurred through December 31.
- The carryover will allow you to roll over up to $680 of your unused balance from 2026 into 2027, after all eligible claims have been submitted by the March 31 deadline. As long as you maintain your Healthcare FSA, you can continue to roll over remaining funds indefinitely, up to the IRS limit.
- To participate in the Healthcare FSA, you must re-enroll each year during Open Enrollment. Your election will not carry over from one year to the next and you cannot make changes to your contribution election throughout the year.

How These Plans Work
- You don’t need to enroll in HICV medical coverage to take advantage of these plans.*
- You pay for coverage through convenient, after-tax payroll deductions.

- You can use the payment(s) you receive in any way you choose—whether it be paying a medical bill or daily living expenses, like food, transportation or childcare.
- When you enroll on UKG Pro, you will see the costs for this coverage, which are based on the plan(s) you choose, who you cover, and, in some cases, your age.
Earn a Health Screening Benefit
You and your covered dependents can each earn $50 per year from the accident and critical illness plans. This benefit may be paid for receiving eligible screenings such as a mammogram, colonoscopy, annual checkup and more.
*Unless required by local or state law.

LTD Tax Options for Exempt Team Members
There are two options in the LTD plan that define how you are taxed:
- Gross-Up: You automatically pay weekly taxes only on the premiums that we pay on your behalf. So, if you become disabled, benefits will be paid tax-free, meaning you will receive more money.
- Non-Gross-Up: You choose not to pay taxes now on the premiums we pay for you. However, if you become disabled, you will pay taxes on the money you receive, lowering the amount you are paid.
When you enroll, you will be automatically enrolled in the gross-up option. To change to the non-gross-up option, you must complete a waiver form during Open Enrollment or within 30 days of your hire date.

SAVING FOR RETIREMENT
401(k) Plan
To help you reach your retirement goals, HICV will match 100% of the first 4% of your pay that you contribute to the Retirement Savings Plan (401(k) Plan), up to IRS limits.
Planning for retirement is about saving enough money now to ensure you can maintain your lifestyle after you retire. The 401(k) Plan, through T.Rowe Price, can help.
Visit rps.troweprice.com to enroll, view your account balance, change your contribution amount, re-allocate your investments and review and update your beneficiary information.
Plan Overview
- All team members become eligible to participate immediately.
- Your contributions can be deducted from your pay before income taxes are taken out of each check (before-tax contributions) or after income taxes are calculated (Roth contributions).
- Choosing before-tax contributions means you pay less in income taxes now and will be taxed on your 401(k) income in retirement.
- Choosing Roth contributions means you pay the income taxes on your contributions now and will not be taxed on your 401(k) income in retirement.
- You can choose a combination of before-tax and Roth contributions.
- You’re immediately vested (meaning you have an undeniable right to your balance) in the contributions you make.
- As a part of our commitment to helping you plan for retirement, HICV will match 100% of the first 4% of your pay that you contribute to your account, up to IRS limits.
- You become vested in your HICV matching contributions after two years of service. A year of service is any calendar year you work a minimum of 1,000 hours.
- You choose how your balance is invested, choosing from a variety of stocks and bonds available.
- You can change your investment allocations anytime during the year.
- You must designate a beneficiary and keep that information up to date to ensure your account is distributed in accordance with your wishes.
Call the Retirement Service Center at (800) 922-9945 if you need help or have questions.
Accessing Your Account Prior to Retirement
Experts commonly recommend you should have between 70% and 80% of your pre-retirement income during retirement. That takes preparation. HICV and the IRS encourage you to save your 401(K) balance to use during retirement. The IRS imposes penalties on early withdrawals from your account, unless you meet certain eligibility.
HICV understands that life happens and competing priorities may make it necessary to use some of your account balance from time to time. You have two options:
Personal Account Loan
You can take a loan from your vested account balance.
- The minimum amount you may borrow is $1,000.
- The maximum you may borrow is the lesser of 50% of your vested account balance or $50,000 minus your highest outstanding loan balance (a total of all your plan loans) from the preceding 12 months.
- You may not have more than one loan at any time.
- Your loan plus interest must be paid back within five years, except for primary residence loans, which must be repaid in 30 years.
- If you leave the company, you must repay your loan in full. Otherwise, the outstanding balance will be considered a distribution.
Qualified Birth or Adoption Distribution
Raising a child is incredibly rewarding, but it doesn’t come without costs. As soon as your new family member officially arrives (and up to anytime within their first year with you), you can withdraw up to $5,000 (per HICV-employed parent) from your HICV 401(K) Plan—without any early withdrawal penalties—to help pay for associated costs. For more information, please contact T.Rowe Price at (800) 922-9945.

Extra Support When Life Happens
Life doesn’t always go as planned—and your HICV benefits are here to help. From pet insurance to travel benefits, the Heart Travels Relief Fund, and Family Assistance Grants, you have support for both life’s surprises and its meaningful moments beyond work.
Throughout these scenarios, you’ll see callouts showing how these additional benefits may fit someone's story, or you can jump down to the Additional Benefits section at the bottom of the page to explore more.
Life Stage: Family + Caregiving
“Some days I feel like I’m taking care of everyone.”
Most mornings, Juan is the first one up—packing lunches, getting the kids ready for school, and checking in with his mom before the day begins. Between work, family, and helping his mom get to medical appointments or pick up prescriptions, a lot of people rely on him. It’s busy, but taking care of his family is his number one priority.
Having dependable medical coverage helps make those responsibilities a little easier. No-cost preventive care keeps his kids up to date on checkups and vaccinations. When someone wakes up with a sore throat or needs guidance fast, Cigna telehealth appointments help him get answers without rearranging his entire day. And when he needs help navigating care, like confirming an in‑network specialist for his mom, the Personal Benefits Concierge and Nurse Advocates Program help simplify the process.
When healthcare expenses pop up, using pre-tax dollars through a Health Savings Account (HSA) or Flexible Spending Account (FSA) also helps stretch the family budget. They help cover things like monthly medications and glucose monitors for his mom.
Caregiving can be emotionally challenging, too. Through HICV’s Employee Assistance Program (EAP), Juan has access to counseling and caregiver resources when he needs extra support. Sometimes just knowing those resources are there makes stressful moments feel more manageable.
Juan is also thinking about the future. By contributing to his 401(k) and taking advantage of HICV’s company match, he’s building a little more security for his family over time.
For Juan, benefits aren’t just coverage, they’re one more way he can take care of the people who count on him.
You’re covered if…
Juan lives in a high-risk area for hurricanes. In the event of a natural disaster, Juan may be eligible for the Heart Travels Relief Fund to help him and his family recover.
Life Stage: Growing Family
“I want to focus on my baby, not on surprise medical bills.”
Lately, Alyssa’s calendar has been filling up with prenatal appointments, baby prep, and all the little details that come with getting ready for her first child. It’s an exciting time—and a big life change. With so much to plan for, one thing she doesn’t want to stress about is unexpected medical bills.
Having medical coverage that supports routine prenatal care and newborn services helps Alyssa focus on her health and her growing family. She’s enrolled in Cigna’s Healthy Pregnancies, Healthy Babies program, which gives her access to maternity coaches and guidance throughout her pregnancy.
She also uses a Healthcare Flexible Spending Account (FSA) to set aside pre-tax dollars for eligible pregnancy-related expenses, like prenatal vitamins and other items recommended by her doctor. This helps her plan ahead financially and stay healthy.
The emotional side of becoming a parent matters, too. Through HICV’s Employee Assistance Program (EAP), Alyssa has access to behavioral health resources if she needs extra support during this time of change.
Since a hospital stay is part of the plan when the baby arrives, Alyssa is also considering hospital indemnity coverage to help with potential out-of-pocket costs.
While she’s focused on preparing for her baby now, Alyssa is also thinking about the future. Contributing to her 401(k) helps her start building financial security for her growing family.
You’re covered if…
The road to pregnancy wasn’t what Alyssa had pictured, but the support she received from her HICV benefits made it possible. The Family Assistant Grant gave Alyssa the financial security she needed for infertility treatments and the chance to grow her family.
Life Stage: Mid-Career + College Planning
“College tuition is around the corner and retirement isn’t that far behind.”
These days, Marcus feels like he’s always planning ahead. With teenagers at home and college on the horizon, he’s balancing today’s expenses while thinking about what the next few years will bring.
Staying healthy is part of that plan. Medical coverage with no-cost preventive care helps Marcus keep up with routine checkups, and his Health Savings Account (HSA) gives him a way to set aside money for current healthcare costs while also saving for the future.
College planning is a big focus, too. Through HICV’s Employee Assistance Program (EAP), Marcus can access financial coaching to help him think through budgeting questions like in-state vs. out-of-state schools and understanding financial aid. Having someone to talk through those decisions with helps make them feel more manageable.
Marcus also contributes to his 401(k), making sure to take full advantage of HICV’s company match. Knowing his savings are growing over time helps him feel more prepared for what’s ahead.
Benefits are about balance, so Marcus can focus on supporting his family today while also staying ready for the future.
You’re covered if…
Visiting colleges with his teenage kids is an exciting time for Marcus and his family, but all these overnight trips can become pricey. Thankfully, Marcus has been taking full advantage of his travel perks with HICV and saving money on all his stays.
Life Stage: Preparing for Retirement
“I want to make sure I’m ready for the next chapter.”
With retirement starting to feel closer, Denise has been spending more time thinking about what the next chapter of life might look like. She’s excited about the possibilities, but she also wants to make sure she’s prepared.
Saving has become a big focus. Denise contributes to her 401(k) and takes advantage of catch-up contributions to help boost her progress as she gets closer to retirement. Seeing those savings grow helps her feel more confident about the years ahead. She also takes advantage of financial counseling through the Employee Assistance Program (EAP) to make sure her savings plan is staying on track.
Taking care of her health is just as important. Medical coverage that supports preventive care helps Denise stay proactive, and her Health Savings Account (HSA) gives her a tax-advantaged way to plan for healthcare costs in retirement.
She also appreciates having supplemental protection—making sure her financial plan doesn’t get derailed by unexpected medical expenses. Along with well-being programs that support healthy aging, these benefits give Denise peace of mind and allow her to feel comfortable thinking about the future.
You’re covered if…
Denise’s home is full of four-legged friends but taking care of two dogs and two cats can be expensive and time consuming! Thankfully, she has MetLife pet insurance through her HICV benefits to protect her fury loved ones and protect her financially.
Life Stage: Early Career
“This is my first job with real benefits — I’m still figuring them out.”
Starting a first job with real benefits has been a learning experience for Jordan. Between figuring out health plans, managing student loans, and sticking to a budget, there’s a lot to take in, but it’s also an exciting step forward.
Jordan chooses medical coverage that fits their current needs, knowing preventive care is covered at no cost. A Health Savings Account (HSA), along with employer contributions, helps Jordan start building savings early for healthcare expenses down the road.
Through HICV’s Employee Assistance Program (EAP), Jordan also has access to counseling and life-skills resources to help manage stress, create a budget, explore student loan repayment strategies, and navigate the transition into working life.
Even though retirement feels far away, Jordan contributes to the 401(k) and takes advantage of HICV’s company match. Starting early feels like a smart way to build good financial habits.
At this stage, Jordan’s benefits provide a sense of stability that support their health, finances, and build confidence as the future begins to take shape.
You’re covered if…
Outside of work, Jordan loves to travel and explore the world and is excited to take advantage of HICV’s travel discounts and lifestyle perks!
Additional Benefits
Extra Support When Life Happens
Life doesn’t always follow a plan. When the unexpected happens, or when you’re planning for an important life milestone, having extra support can make a meaningful difference.
Through the Heart Travels Assistance Program, eligible team members may be able to access financial assistance when they need it most. The program includes two types of support designed to help during different moments in life.
The Heart Travels Relief Fund provides grants for team members experiencing financial hardship after a natural disaster or other unexpected emergency. This assistance can help cover immediate needs like temporary housing, essential supplies, or other urgent expenses while you focus on getting back on your feet. Team members can voluntarily contribute to the Relief Fund to help colleagues in need.
The program also includes a Family Assistance Grant, which helps support team members who are growing their families. Whether you’re adopting a child, working with a surrogate, or receiving infertility treatment, this grant can help offset some of the costs that come with expanding your family.
Because sometimes support means being there in difficult moments, and sometimes it means helping you take the next step in your life journey.
Coverage for Your Pets, Too
Some members of the family walk on four legs. And just like any other family member, their health and well-being matter.
That’s why HICV offers Pet Insurance through MetLife, helping make veterinary care more manageable when your pet needs it most. Coverage can help reimburse the cost of care for accidents or illnesses, including exams, surgeries, medications, diagnostic tests, and more.
You can also choose optional preventive care coverage, which helps with routine expenses like annual checkups, vaccinations, medications, and other services that keep your pet healthy year-round.
The plan is designed to be flexible and convenient. You can visit any licensed veterinarian in the U.S., and coverage even travels with you. Plus, with 24/7 Telehealth Concierge Services, you can get answers to those late-night questions when something just doesn’t seem right.
Claims are easy to manage through the MetLife Pet mobile app, where you can submit and track claims, manage your pet’s health information and find nearby pet services.
Because when your pet is part of the family, having support for their care matters too.
