Three opportunities throughout the plan year to make changes to your benefits:
Benefits will be effective on your hire date/first day of employment, and elections must be submitted within 14 days of your employment start date.
A change in your life situation - like getting married, having a baby, or losing health coverage - that can make you eligible to make changes to your health insurance coverage outside of the regular open enrollment period. Please report qualified life events to the Benefits Team within 30 days.
One time per year when employees can enroll or make changes to their health insurance coverage.

Regular dental exams can tell your doctor a lot about your overall health. Enrollment in dental benefits provides an affordable way to maintain your dental hygiene. We offer our employees a great PPO dental plan through Guardian.
Guardian has one of the nation’s largest selection of network dentists and they’re growing fast, with over 115,000 dentists at more than 370,000 locations across the US. Although you can choose any dental provider, when you use an in-network dentist, you will generally pay less for treatments.

DentalGuard Preferred Network
Eye exams tell your doctor a lot about your overall health. It’s important to schedule regular exams to help detect significant medical conditions before they become serious. We offer our employees a vision insurance plan through Guardian utilzing the Davis Vision network. This network offers access to over 98,000 provider access points nationwide, including private practice providers and many convenient retailers including Costco, Walmart, Sam’s Club, Target, JCPenney, Visionworks, and Pearle Vision.
Guardian’s vision care benefits include coverage for eye exams, standard lenses and frames, contact lenses, and discounts for laser surgery. The vision plan is built around a network of eye care providers, with better benefits at a lower cost to you when you use providers who belong to the Davis Vision Network. When you use an out-of-network provider, you will have to pay more for vision services.

A Healthcare Flexible Spending Account, or HCFSA, is a benefit that allows you to choose how much of your paycheck you’d like to set aside, before taxes are taken out, for healthcare expenses. This saves you money by reducing your taxable income.
For the 2025-2026 plan year, the HCFSA contribution limit is $3,300. The minimum annual election for the HCFSA is $200. Remember HCFSAs are “use it or lose it”. Any remaining funds will be forfeited at the end of the plan year, with the exception of $660 that you are allowed to carry over.
One benefit to an HCFSA is that your annual election amount is available to spend right away – on your benefits effective date. When spending your HCFSA money on eligible healthcare expenses, please make sure to keep all receipts/invoices for eligible HCFSA claims.

A Dependent Care FSA, or DCFSA, lets you pay for eligible dependent care expenses while you reap the benefits of additional tax savings. It is a smart, simple way to save money while taking care of your loved ones so that you can continue to work.
Your Dependent Care FSA can be used to pay for out-of-pocket daycare expenses, after-school programs, and day camp programs for dependents up to the age of 13. It also covers costs for a disabled spouse or dependents of any age.
Your household can elect up to $5,000 annually through the Dependent Care FSA. The minimum annual election for the DCFSA is $200.

A Health Savings Account, better known as an HSA, is an individually-owned, personal health care savings account that you can use to pay for out-of-pocket health care expenses with pre-tax dollars. Your contributions are tax-free, and the money remains in the account for you to spend on eligible expenses no matter where you work or how long it stays in the account.
In order to be eligible for an HSA, you must be enrolled in the High-Deductible Health Plan (HDHP) for medical coverage. An HDHP generally costs less than what traditional heath care coverage costs, so the money that you save on insurance premiums can therefore be put into the Health Savings Account.

Disability benefits pay you a portion of your salary in situations when non-job-related injuries, illnesses, or other medical issues prevent you from working. This benefit provides financial protection to you when you need it most.
MedPro Healthcare Staffing provides both Short-Term and Long-Term Disability coverage at no cost to you.
Short-Term Disability coverage replaces 60 percent of your eligible weekly base pay, up to a maximum of $500 per week, after you have satisfied the 14-day waiting period for benefits.
Long-Term Disability coverage replaces 60 percent of your eligible monthly base salary, up to $5,000 per month, after you have satisfied the 180-day waiting period for benefits.
Your benefit amount may be offset by other benefits you are receiving, such as Social Security or workers’ compensation. Your benefits are subject to federal income tax and may be subject to state and local taxes.

As a full-time employee of MedPro Healthcare Staffing, you are eligible to participate in our 401k retirement plan with The Standard. New hires may begin participating on the first of the month following three months of service. Existing employees can enroll and make changes at any time.
Employees can contribute to the plan on a pre-tax or post-tax (Roth) basis.
MedPro will make a matching contribution equal to 25% on the first 8% that you contribute. For example, if you contribute 8%, MedPro will match 2%.
You can choose how your contributions are invested among the plan’s available investment options. If you do nothing, they will be invested in our plan’s Qualified Default Investment Alternative (QDIA), which is the Mainspring Managed service.
The money you contribute into your 401k is always your money. If you leave MedPro, you can always take your money with you. The match contribution from MedPro, however, is not yours until you become vested into it. MedPro has a three-year graduated vesting schedule based on years of service.
