2026 Medical Benefits

Benefits provided through Meritain.

WellSky offers you three medical plans through Meritain to choose from, which are highlighted in the button below.

Three plan options are available:

• PPO is primarily a copay plan. This means some medical services, such as an office visit, will require a copay only as they are not subject to deductible. This plan will cost you more out of your paycheck but you may pay less when you need care.
• HDHP A is a qualified health plan, these benefits are subject to deductible plus coinsurance. Since it is a qualified plan, it is HSA eligible. WellSky will contribute to your HSA, which is dependent on level of coverage you choose.
• HDHP B is a qualified health plan, these in-network benefits are subject to deductible. Since it is a qualified plan, it is HSA eligible. WellSky will contribute to your HSA, which is dependent on level of coverage you choose. HDHP B is an embedded plan, meaning if you enroll a family and one person meets the individual deductible amount, the plan begins sharing in cost for that specific family member.

Those who enroll in either of the HDHP medical plans will be eligible for a health savings account (HSA). More information on HSAs can be found on the HSA & FSA tab.

Those who enroll in the PPO plan will have access to the Zero card, which gives access to various healthcare services at no cost. More information can be found below.

What is Carrum Health?

Carrum Health is a special surgery and medical benefit provided by WellSky to you.

Carrum Health partners with the top hopsitals and surgeons to help you get the best possible care, often at little or no cost to you! Covered procedures include but limited to:

Whether you need cancer care or a range of surgical procedures, Carrum Health works with the top cancer specialists and surgeons in the country—those who have better outcomes and outstanding bedside manner. And when you receive care through Carrum Health, most, if not all, costs are covered.

Note: Individuals enrolled in the high deductible plans (HDHP A or HDHP B) must first meet the federal minimum deductible, but coinsurance will be waived. Per IRS rules, a portion of any covered travel expenses will be reported as taxable income for any Carrum participant.

IMPORTANT:

It is required for medical plan members to go through the Carrum Health benefit, provided by WellSky, for total hip/knee replacements, bariatric weight loss surgery, and certain spine procedures. Your surgery will not be covered by the Meritain medical plan if you get one of these surgeries outside Carrum.

2026 Prescription Drug

Pharmacy benefits

WellSky provides prescription drug coverage with your medical plan enrollment. Your prescription benefit is designed to bring you quality pharmacy care that will help you save money. The information below is a brief summary of your prescription benefits as well as some frequently asked questions about the CVS Caremark prescription benefit program. CVS Caremark and WellSky are confident you will find value with your prescription benefit program.

CVS Caremark Retail Pharmacy Network

For short-term medications (up to a 30-day supply)

The CVS Caremark Retail Network includes more than 68,000 participating pharmacies nationwide, including independent pharmacies, chain pharmacies, and 9,600 CVS Pharmacy locations. To locate a CVS Caremark participating retail network pharmacy in your area, simply click on “Find a Pharmacy” at caremark.com or call a Customer Care representative at 888-626-1159.

CVS Caremark Maintenance Medications

For maintenance medications (up to a 90-day supply) you have the choice of:

• Filling your maintenance medications at a CVS Pharmacy retail location, or
• Simply mail your original prescription and the mail service order form to CVS Caremark. Your medications will be sent directly to your home, office, or a location of your choice.

Specialty Medications - Understanding CVS Caremark’s True Accumulator Program

The True Accumulator program ensures that only the amount you personally pay for certain specialty medications counts toward your deductible and out-of-pocket maximum. If you use a manufacturer copay card, the portion covered by the card won’t be applied to your benefit totals. This helps manage overall plan costs while keeping your medication access intact.

For example, if a specialty drug has a $50 copay and you use a $45 copay card, only the remaining $5 you pay is counted toward your deductible and out-of-pocket maximum. The True Accumulator program is available for specialty medications dispensed by CVS Specialty that offer non–needs-based copay assistance.

Traditional drugs

• Generic drugs offer the lowest copay: You pay the lowest copay for generics, making these drugs the most cost-effective option for treatment.

• Brand drugs are a higher copay: This category includes preferred, brand name drugs that don't yet have a generic equivalent. These drugs are more expensive than generics, and a higher copay.

• Non-preferred drugs are the highest copay: In this category are nonpreferred brand name drugs for which there is either a generic alternative or a more cost-effective preferred brand. These drugs have the highest copay. Make sure to check for mail order discounts that may be available.

Specialty drugs

• Preferred drugs offer the lowest specialty drug copay: Specialty drugs are generally more effective and less expensive than nonpreferred specialty drugs.

• Non-preferred drugs are the highest specialty drug copay: These drugs have the highest copay for specialty drugs, usually because there may be a more cost-effective generic or preferred brand available.

• True Accumulator: Only the amount you personally pay for certain specialty medications counts toward your deductible and out-of-pocket maximum under the True Accumulator program, excluding any manufacturer copay assistance.

HELPFUL COST SAVING TIPS 

• Mail order: Many drugs are available in a 90-day supply, rather than the 30-day retail supply. The Mail order cost is 2.5x the preferred copay.

• Good Rx: You can save on prescription costs by utilizing online tools like GoodRx.com. This platform provides an Rx database to help you find the most affordable pharmacy for your specific prescription. You may also find coupons directly from the drug manufacturers via their websites that may reduce your expenses. Keep in mind that some coupons can only be used outside of your prescription plan and will not count towards your annual out-of-pocket maximums.

2026 Medical Carrier Programs

Teladoc for medical, dermatological, and behavioral health  

All teammates enrolled in the medical plan and their covered family members will have access to Teladoc. This is a company paid benefit under the PPO plan. For the HDHP plans, you will pay for the cost of the visit. If you have not met your deductible, you will pay the following towards your deductible:

• General medicine ($56 - $58)

• Psychiatrist (initial $215; subsequent $100)

• Dermatology ($85)

• Therapist ($90)

Once you have met your deductible on the HDHP plans, WellSky covers the entire cost of your Teladoc visit.

CVS Minute Clinic for all members

Minute Clinic makes it easy for you to get the care you need, when you need it. Now WellSky members can access all eligible services, including general medical Minute Clinic Virtual Care visits at any in-network clinic at little to no cost to teammates. Services include:

• Treatment of illnesses
• Treatment of injury and conditions
• Write prescriptions when necessary

Minute Clinic is a walk-in clinic inside select CVS Pharmacy and Target stores and is the largest provider of retail health care in the United States.

Preventive care through CheckUps by PathFinder

WellSky's medical plans provide coverage for in-network annual preventive exams and biometric screenings at no cost.

To provide you and your eligible dependents greater access to healthcare,

WellSky has partnered with Pathfinder Health - to provide an innovative option to bring preventive health to your doorstep! PathFinder offers you a virtual visit with a provider to review your biometric screening results and discuss next steps.

Wellness Incentive for completing your annual physical

Teammates enrolled in a WellSky medical plan are eligible to receive a $100 wellness incentive for preventive services. Teammates have the option to complete their annual physical through their primary care physician or through CheckUps by PathFinder.

Teammates enrolled in the HSA plan may receive a $100 contribution to their HSAs; teammates enrolled in the PPO plan may receive a $100 wellness reward paid directly to you. Wellness rewards paid directly to you are considered taxable income.

Teammates will be asked to complete a preventive care affidavit to acknowledge services have been completed. Wellness incentive requests must be submitted by December 1 for services rendered within the calendar year. Guidelines and the wellness incentive form may be found on Teammate hub > Benefits & Wellness.

2026 Health Savings & Flexible Spending Accounts

Health savings account (HSA)

An HSA is a personal healthcare bank account you can use to pay out-of-pocket medical expenses with pretax dollars. If you enroll in a high-deductible health plan, you can open an HSA. You own and administer your HSA.

You determine how much you contribute to your account, when to use your money to pay for qualified medical expenses, and when to reimburse yourself. Remember, this is a bank account; you must have money in the account before you can spend it.

HSAs offer you the following advantages:

TAX SAVINGS: You contribute pretax dollars to the HSA. Interest accumulates tax-free, and funds are withdrawn tax-free to pay for medical expenses.

REDUCED OUT-OF-POCKET COSTS: You can use the money in your HSA to pay for eligible medical, dental and vision expenses and prescriptions. The HSA funds you use can help you meet your plan’s annual deductible.

A LONG-TERM INVESTMENT THAT STAYS WITH YOU: Unused account dollars are yours to keep even if you retire or leave the company. Also, you can invest your HSA funds, so your available healthcare dollars can grow over time.

THE OPPORTUNITY FOR LONG-TERM SAVINGS: Save unused HSA funds from year to year — you can use this money to reduce future out-of-pocket health expenses. You can even save HSA dollars to use after you retire.

HSA eligibility

• Employee funded based on elected payroll deductions.

• Employee must be enrolled in company’s HDHP (HSA) plan.

• Not enrolled in Medicare, covered under your spouse's non-qualified medical plan/disqualifying health coverage, or listed as a tax dependent on someone's tax return.

How to access/make contributions to your HSA

Once your account is open, you can access it by visiting the WEX login for WellSky. You’ll set up your payroll contributions during your enrollment window. You can make contribution changes at any time during the year through WEX. Note that it may take between one and two payroll periods for an HSA change to be processed.

The HSA is administered by WEX. WellSky pays the monthly administrative fee for your HSA. If your coverage status or employment status changes, you will be responsible for all HSA account holder fees.

How much you can contribute into an HSA

WellSky employer contributions count toward the annual HSA contribution limits, so you need to plan carefully how much you’ll contribute annually to avoid excess contributions. These limits apply even for participants entering the plan midyear.* The 2026 IRS maximum contribution for these accounts are:

• $4,400 for employee-only coverage.

• $8,750 for all other coverage levels.

Individuals ages 55 and older may make an additional $1,000 annual contribution to their HSA.

*If you make the full-year contribution based upon your status of Dec. 1, you may be subject to an IRS Testing Period and could owe tax and a penalty on part of that contribution if you do not remain an eligible individual through Dec. 31 the following year. You may also need to prorate your contribution if you drop or reduce the level of your coverage midyear.

Contributions 

HSA contributions are tax-free if they are used to pay for qualified medical expenses.

• Qualified medical, dental and vision expenses not covered by insurance.

• Qualified long-term care services and long-term care insurance.

• Continuation of coverage required by federal law (i.e., COBRA).

• Health insurance for the unemployed.

• Medicare expenses (but not Medigap).

• Retiree health expenses for individuals age 65 or older.

• Distributions made for any other purpose are subject to income tax and a 20% penalty. The 20% penalty is waived in the case of death or disability. The 20% penalty is also waived for distributions made by individuals age 65 or older.

Watch your account balance change and grow

By setting a goal of saving $1,000 per year in your HSA, see how you could accumulate $15,000 in savings over 15 years. These funds will be put in tax-free and grow tax-free, and they can be taken out tax-free for qualified expenses.

The funds in your HSA will earn interest annually. Additionally, after your account balance reaches $1,000, you will be able to invest your funds in select investment options. The earnings on your HSA funds will never be taxed.

Since this account is always yours and rolls over from year to year, you can choose to use it now or as another investment tool for retirement.

Medical flexible spending account (FSA)

• Pay qualified medical, dental and vision expenses for yourself and your dependents with pre-tax dollars.

• A debit card is available, or you may file claims for reimbursement.

• You may contribute up to $3,400/year through payroll deduction.

• Elections may be changed only during open enrollment, unless you have a change in status.

• You cannot participate in the medical FSA if you contribute to an HSA.

• Plan year is Jan. 1 through Dec. 31.

• Elections do not carry over from year to year; you must re-enroll each year to continue participation.

This is a use-it-or-lose-it account, meaning any funds remaining in the account following the close of the plan year will be forfeited. Medical care FSA new to you? WEX provides more information about this account.

Limited purpose flexible spending account (LFSA)

If you are enrolled in an HSA eligible plan, you are eligible to enroll in the limited purpose flexible spending account. IRS rules state that you cannot have both an HSA and general health FSA since both apply funds toward your medical expenses.

A limited purpose health FSA allows you to continue to contribute to an HSA. A limited purpose health FSA is much like a medical care FSA. The main difference is that the limited purpose account is set up to reimburse only eligible FSA dental and vision expenses. The maximum contribution in the 2026 plan year for the limited purpose flexible spending account is $3,400.

Dependent care flexible spending account (FSA)

Dependent care FSAs allow you to set aside money pretax to pay eligible out-of-pocket day care expenses for all dependents up to the age of 13 so that you or your spouse can work or attend school full time. You must contribute money through payroll deduction to your dependent care FSA before you can spend it.

When making your enrollment selections, you must decide how much to set aside for this account for the upcoming year. You may contribute up to $5,000, or up to $2,500 if you are married and file separate tax returns. The limit is $5,000* for those that are single/head of household or married and filing jointly. While the IRS has increased the Dependent Care FSA contribution limit to $7,500, our plan will remain at the previous limit of $5,000. This decision reflects a careful balance of financial priorities and overall benefits strategy tied to required IRS non-discrimination testing. We appreciate your understanding and remain committed to offering a competitive and sustainable benefits package.

REMEMBER:

Changes to your medical or dependent care FSA elections can be made only during your enrollment window or if you experience a qualifying life event.

FSA dollars are on a "use it or lose it" basis. Please be cautious when enrolling in the offered FSAs, especially if you are contemplating enrolling in both. If you miscalculate your needs for either category, you could lose a significant amount of pre-tax money. All services should be incurred from Jan. 1, 2026, through Dec. 31, 2026, however, WellSky offers an additional 2.5 month grace period. All services must be incurred by March 15, 2027. Claims must be submitted by March 31, 2027.

To ensure our dental benefits meet the needs of our Teammates and their families, WellSky has carefully evaluated a range of dental carriers. Our selected offering provides access to one of the largest networks of in-network providers, helping make quality dental care more accessible and convenient.

WellSky offers two unique plan designs with Delta Dental of Kansas to allow you the choice of enrolling in the plan that fits your personal needs.

You may see any dentist of your choice, however, by using a dentist that is in the Delta Dental network, you may pay less than if you seek care at an out-of-network provider.

2026 Life & Disability Insurance

Benefits provided through The Standard. 

Basic life and accidental death and dismemberment (AD&D)

WellSky provides, basic life and AD&D at no cost to you. In the event of your death, our life insurance policy helps provide a general safety net for your beneficiaries. If you are a full-time or part-time employee, working 30 hours or more per week your coverage is the amount equal to one times the your annual salary (up to a max of $500,000). AD&D provides another layer of benefits to either you or your beneficiary if you suffer from loss of a limb, speech sight or hearing, or if you die in an accident. For the 2026 plan year, WellSky has decided to partner with The Standard for our life coverage.

Voluntary life and AD&D

You have the opportunity to purchase voluntary life and AD&D insurance for yourself, your spouse and/or your dependent children. Please note, voluntary life and AD&D are elected separately and can be different amounts. Your cost for this coverage is based on the amount you elect and your age. You must purchase voluntary life and AD&D insurance for yourself in order to purchase spouse and/or dependent child(ren) coverage. You may purchase additional life insurance for the teammate, spouse, and children. Coverage is up to $500,000 for teammate,
$250,000 for spouse, and $10,000 for children. If you did not enroll in this coverage when you were first eligible, you will be subject to medical underwriting.

Voluntary life and AD&D summary

IMPORTANT:

Beneficiary Updates

Please take this enrollment opportunity to review your designated beneficiary (or beneficiaries) within the enrollment system. In the case of your death while covered on this plan, your life and/or AD&D benefit will be paid to the beneficiary you have listed. 

STD benefits are designed to replace a portion of your income for a non-work-related short-term injury or illness. STD benefits are paid at 60% of your eligible weekly base pay, up to $2,500 weekly, during the first 180 days of injury or illness. The STD benefit is paid for by WellSky; there is no cost to you. However, any income replacement benefits received are taxable.

This benefit offers financial protection to you when you need it most — if you become disabled and can no longer work. The plan will also help you return to work, if appropriate.

If you become totally disabled, you will receive 60% of your base salary, up to $10,000 monthly, after you have satisfied the 180-day waiting period for benefits. Your benefit amount may be offset by other benefits you are receiving, such as Social Security or workers’ compensation. Your monthly benefits are subject to federal income tax and may be subject to state and local taxes.

2026 Voluntary Benefits

Benefits provided through Voya.

Voluntary benefits insurance through Voya can help protect you from significant or unexpected out-of-pocket expenses. Consider your anticipated medical needs along with the cost of the insurance plans available to you. Keep in mind, these benefits are intended to supplement, not replace, your medical benefit. Curious about these types of benefits?

Accident

The accident insurance through Voya is designed to supplement your medical benefit by paying specific benefit amounts for expenses resulting from injuries or accidents. Hospitalization, physical therapy, intensive care, transportation and lodging are some of the out-of-pocket expenses that this accident insurance could cover. Coverage is available for you, your spouse and/or your child(ren).

Critical illness

WellSky partners with Voya to offer critical illness insurance, which supplements your medical benefit by helping you pay costs associated with a critical illness or event. Some of the conditions covered under this program include cancer, heart attack, stroke, Alzheimer’s, kidney failure and paralysis. Benefits are paid tax-free in a lump sum ranging from $10,000 to $30,000 to be used at your discretion.

Hospital indemnity

Hospital indemnity insurance through Voya is designed to help provide financial protection for covered individuals by paying a benefit for hospitalization. You can use the benefit to pay for out-of-pocket expenses and extra bills that can occur. Indemnity lump-sum benefits are paid directly to you based on the amount of coverage listed, regardless of the actual cost of treatment.

2026 Retirement 401(k)

Benefits provided through Principal.

All teammates over the age of 19 are eligible to participate in the plan after the first day of the month following hire date. You can contribute any percentage of your pay, subject to plan and IRS limits. Teammate and WellSky contributions are always 100% vested, and you can change your contribution rate at any time. Once eligible, information on plan details including enrollment, auto-escalation, beneficiary and investments options will be available to you through Principal.

WellSky provides a generous match provided on a per paycheck basis subject to IRS limits. WellSky will match 100% on teammate contributions up to 3% of your pay, plus a 50% match on teammate contribution amounts over 3% up to a maximum of 5% of your pay.

Medicare Information

Medicare is health insurance for people 65 or older. You’re first eligible to sign up for Medicare three months before you turn 65. You may be eligible to get Medicare earlier if you have a disability, End-Stage Renal Disease (ESRD), or ALS (also called Lou Gehrig’s disease).

Wellness incentive for completing your annual physical

Teammates enrolled in a WellSky medical plan are eligible to receive a $100 wellness incentive for preventive services. Teammates have the option to complete their annual physical through their primary care physician or through CheckUps by PathFinder.

Teammates enrolled in the HSA plan may receive a $100 contribution to their HSAs; teammates enrolled in the PPO plan may receive a $100 wellness reward paid directly to you. Wellness rewards paid directly to you are considered taxable income.

Teammates will be asked to complete a preventive care affidavit to acknowledge services have been completed. Wellness incentive requests must be submitted by December 1 for services rendered within the calendar year. Time Away

WellSky recognizes that time away from work is integral to well-being and to help lower stress and improve mental health. WellSky teammates are provided with a generous variety of time off options.

Vacation

Vacation time is available to eligible teammates and is based on length of service and position.  Part-time teammates may be eligible for a prorated amount depending on work schedule.

Holidays

Teammates receive a set number of paid holidays annually, including "floating holidays” that can be used at their discretion. Part-time and new teammates receive holiday benefits on a prorated basis.

Well-being Time

Eligible teammates accrue well-being time off from the start of their employment to use for personal illness or caring for sick dependents. Accrual is based on legal guidelines and is capped at a set maximum.

Volunteer Time Off

All regular full-time and part-time teammates are eligible to take time off to volunteer with a charitable organization, 501(c)(3) nonprofit, or its U.S. equivalent.

Civic Duty

Paid time off is granted to teammates for participating in civic activities, including voting and serving on a jury.

Bereavement Leave

Paid time off is granted to teammates following the death of a family member, with the number of days varying based on the relationship to the deceased.

The Summary of Benefits and Coverage (SBC) is required to be provided and contains specific information about the plan’s coverages (including deductibles, coinsurance, and copays). SBC’s must be provided in a standard format and contain the same plan level details to allow you to easily compare multiple benefit plan options (even when the plans are offered through different carriers). The SBC is a snapshot of health plan’s costs, benefits, covered health care services, and other features that are important to you.

The Summary Plan Description (SPD) or Certificate of Coverage provides an in-depth description of the plan benefits, including coverages, limitations, eligibility, claims submission details, etc. Also included in the SPD/COC are certain legally mandated notices and information. Helpful Terms

Here are some terms you may see.

Age reduction

The group term basic life and AD&D insurance coverage are subject to a reduction in benefit amount as you age.

Balance Billing 

When a provider bills you for the balance remaining on the bill that your plan doesn’t cover. This amount is the difference between the actual billed amount and the allowed amount. For example, if the provider’s charge is $200 and the allowed amount is $110, the provider may bill you for the remaining $90. This happens most often when you see an out-of-network provider. A network provider may not bill you for covered services.

Claim

A request for a benefit (including reimbursement of a health care expense) made by you or your health care provider to your health insurer or plan for items or services you think are covered.

Coinsurance

Your share of the costs of a covered health care service, calculated as a percentage (for example, 20%) of the allowed amount for the service. You generally pay coinsurance plus any deductibles you owe. (For example, if the health insurance or plan’s allowed amount for an office visit is $100 and you’ve met your deductible, your coinsurance payment of 20% would be $20. The health insurance or plan pays the rest of the allowed amount.)

Copay

A fixed amount (for example, $15) you pay for a covered health care service, usually when you receive the service. The amount can vary by the type of covered health care service.

Cost Sharing

Your share of costs for services that a plan covers that you must pay out of your own pocket (sometimes called out-of-pocket costs). Some examples of cost sharing are copayments, deductibles, and coinsurance. Family cost sharing is the share of cost for deductibles and out-of-pocket costs you and your spouse and/or child(ren) must pay out of your own pocket. Other costs, including your premiums, penalties you may have to pay, or the cost of care a plan doesn’t cover usually aren’t considered cost sharing.

Deductible

An amount you could owe during a coverage period (usually one year) for covered health care services before your plan begins to pay. An overall deductible applies to all or almost all covered items and services. A plan with an overall deductible may
also have separate deductibles that apply to specific services or groups of services. A plan may also have only separate deductibles. (For example, if your deductible is $1,000, your plan won’t pay anything until you’ve met your $1,000 deductible for covered health care services subject to the deductible.)

Embedded Deductible

If you are on a family medical plan with an embedded deductible, your plan contains two components: an individual deductible and a family deductible. Having two components to the deductible allows each member of your family to have your insurance policy cover their medical bills prior to the entire dollar amount of the family deductible being met. The individual deductible is embedded in the family deductible.

Explanation of Benefits (EOB)

An EOB is a statement from the insurance company showing how claims were processed. The EOB tells you what portion of the claim was paid to the healthcare provider and what portion of the payment, if any, you are responsible for.

Emergency Medical Transportation

Ambulance services for an emergency medical condition. Types of emergency medical transportation may include transportation by air, land, or sea. Your plan may not cover all types of emergency medical transportation, or may pay less for certain types.

Emergency Room Care/Emergency Services Services to check for an emergency medical condition and treat you to keep an emergency medical condition from getting worse. These services may be provided in a licensed hospital’s emergency room or other place that provides care for emergency medical conditions.

Health Insurance

A contract that requires a health insurer to pay some or all of your health care costs in exchange for a premium. A health insurance contract may also be called a “policy” or “plan.”

Hospitalization

Care in a hospital that requires admission as an inpatient and usually requires an overnight stay. Some plans may consider an overnight stay for observation as outpatient care instead of inpatient care.

Hospital Outpatient Care

Care in a hospital that usually doesn’t require an overnight stay.

Individual Responsibility Requirement

Sometimes called the “individual mandate”, the duty you may have to be enrolled in health coverage that provides minimum essential coverage. If you don’t have minimum essential coverage, you may have to pay a penalty when you file your federal income tax return unless you qualify for a health coverage exemption.

Imputed income

Federal regulations require payment of income and Social Security taxes on the value of the life insurance premiums in excess of $50,000 when paid for by your employer. The value of dependent life coverage paid for by your employer is also taxable. These values are known as imputed income. Contact your tax professional for information regarding these tax consequences if you have questions or concerns

In-network Coinsurance

Your share (for example, 20%) of the allowed amount for covered healthcare services. Your share is usually lower for in-network covered services.

In-network Copayment

A fixed amount (for example, $15) you pay for covered health care services to providers who contract with your health insurance or plan. In-network copayments usually are less than out-of-network copayments.

Maximum Out-of-Pocket Limit

Yearly amount the federal government sets as the most each individual or family can be required to pay in cost sharing during the plan year for covered, in-network services. Applies to most types of health plans and insurance. This amount may be higher than the out-of-pocket limits stated for your plan.

Network Provider (Preferred Provider)

A provider who has a contract with your health insurer or plan who has agreed to provide services to members of a plan. You will pay less if you see a provider in the network. Also called “preferred provider” or “participating provider.”

Out-of-Network Coinsurance

Your share (for example, 40%) of the allowed amount for covered health care services to providers who don’t contract with your health insurance or plan. Out-of-network coinsurance usually costs you more than in-network coinsurance.

Out-of-Network Copayment

A fixed amount (for example, $30) you pay for covered health care services from providers who do not contract with your health insurance or plan. Out-of-network copayments usually are more than in-network copayments.

Out-of-network Provider (Non-Preferred Provider)

A provider who doesn’t have a contract with your plan to provide services. If your plan covers out-of-network services, you’ll usually pay more to see an out-of-network provider than a preferred provider. Your policy will explain what those costs may be. May also be called “non-preferred” or “non-participating” instead of “out-of-network provider”.

Out-of-Pocket Limit

The most you could pay during a coverage period (usually one year) for your share of the costs of covered services. After you meet this limit the plan will usually pay 100% of the allowed amount. This limit helps you plan for health care costs. This limit never includes your premium, balance-billed charges or health care your plan doesn’t cover. Some plans don’t count all of your copayments, deductibles, coinsurance payments, out-of-network payments, or other expenses toward this limit.

Plan

Health coverage issued to you directly through an employer, union or other group sponsor (employer group plan) that provides coverage for certain health care costs. Also called "health insurance plan", "policy", "health insurance policy" or "health insurance".

Portability and conversion

Portability and conversion are available if your employment with IronEdge ends. Portability allows you to continue your term life coverage, while the conversion option allows you to convert your term life policy into an individual whole life policy

Preauthorization

A decision by your health insurer or plan that a health care service, treatment plan, prescription drug or durable medical equipment (DME) is medically necessary. Sometimes called prior authorization, prior approval or precertification. Your health insurance or plan may require preauthorization for certain services before you receive them, except in an emergency. Preauthorization isn’t a promise your health insurance or plan will cover the cost.

Premium

The amount that must be paid for your health insurance or plan. You and/or your employer usually pay it monthly, quarterly, or yearly.

Prescription Drug Coverage
Coverage under a plan that helps pay for prescription drugs. If the plan’s formulary uses “tiers” (levels), prescription drugs are grouped together by type or cost. The amount you'll pay in cost sharing will be different for each "tier" of covered prescription drugs.

Preventive Care (Preventive Service)

Routine health care, including screenings, check-ups, and patient counseling, to prevent or discover illness, disease, or other health problems.

Primary Care Provider

A physician, including an M.D. (Medical Doctor) or D.O. (Doctor of Osteopathic Medicine), nurse practitioner, clinical nurse specialist, or physician assistant, as allowed under state law and the terms of the plan, who provides, coordinates, or helps you access a range of health care services.

Provider
An individual or facility that provides health care services. Some examples of a provider include a doctor, nurse, chiropractor, physician assistant, hospital, surgical center, skilled nursing facility, and rehabilitation center. The plan may require the provider to be licensed, certified, or accredited as required by state law.

Reasonable and Customary

The amount of money a health plan determines is the normal or acceptable range of charges for a specific health-related service or medical procedure. If your healthcare provider submits higher charges than what the health plan considers normal or acceptable, you may have to pay the difference.

Referral

A written order from your primary care provider for you to see a specialist or get certain health care services. In many health maintenance organizations (HMOs), you need to get a referral before you can get health care services from anyone except your primary care provider. If you don’t get a referral first, the plan may not pay for the services.

Rehabilitation Services

Health care services that help a person keep, get back, or improve skills and functioning for daily living that have been lost or impaired because a person was sick, hurt, or disabled. These services may include physical and occupational therapy, speech-language pathology, and psychiatric rehabilitation services in a variety of inpatient and/or outpatient settings.

Screening

A type of preventive care that includes tests or exams to detect the presence of something, usually performed when you have no symptoms, signs, or prevailing medical history of a disease or condition.

Specialist

A provider focusing on a specific area of medicine or a group of patients to diagnose, manage, prevent, or treat certain types of symptoms and conditions.

Specialty Drug

A type of prescription drug that, in general, requires special handling or ongoing monitoring and assessment by a health care professional, or is relatively difficult to dispense. Generally, specialty drugs are the most expensive drugs on a formulary.

True Accumulator

A CVS program that helps ensure that only the amount you personally pay for your prescriptions counts toward your deductible and out-of-pocket maximum. If you use a manufacturer copay card, the portion covered by the card won’t apply to your benefit totals—only your actual payment will. This helps keep your pharmacy benefits fair and sustainable while still giving you access to support programs.

UCR (Usual, Customary and Reasonable)

The amount paid for a medical service in a geographic area based on what providers in the area usually charge for the same or similar medical service. The UCR amount sometimes is used to determine the allowed amount.

Urgent Care

Care for an illness, injury, or condition serious enough that a reasonable person would seek care right away, but not so severe as to require emergency room care.
