Our medical benefits provide the coverage you need to help you be and stay healthy. From routine check-ups to essential treatments, it is easy to take care of your health. To help meet your unique needs, we offer a combination of a PPO, EPO and HDHP HSA plan. As you go through the enrollment process, you can access the benefits plan comparison, summary of benefits, and search for in-network providers by clicking on the links below.
With a PPO medical plan, you have the choice of two benefit tiers: plan providers (in-network) and non-plan providers (out-of-network). The PPO option offers a more extensive network of providers and specialists, including national networks. You do not need to designate a PCP or obtain a referral to see a specialist.

With an EPO medical plan, you do not have out-of-network coverage and will need to search for providers in-network. Although you do not need referrals with an EPO, it's important to always search for providers in-network to ensure services are covered.
With a High Deductible Health (HDHP) HSA plan, you have access to in-network providers. You can also take advantage of a Health Savings Account (HSA). You contribute money pre-tax into the account and then use the funds to pay for eligible medical, dental, and vision expenses or to cover the costs of meeting the plan's deductible.

In the event of an unexpected loss or severe injury, these benefits offer essential financial support, helping to cover expenses and ensuring peace of mind during challenging times. By opting into Life and AD&D coverage, you're taking an important step in protecting your family's future and safeguarding against the unforeseen.

If you would like additional life insurance, you can purchase voluntary life and AD&D insurance for yourself, a spouse or domestic partner, and your eligible children through Anthem Life. Evidence of Insurability (EOI) is not required up to the guaranteed issue amount. Your contribution for this additional benefit will be deducted post-tax from your paycheck.

Disability insurance provides financial protection if you become too sick or injured to work. If you ever find yourself unable to perform your job due to a medical condition, disability benefits will provide a portion of your income, ensuring you have the financial means to support yourself while you recover. This coverage allows you to focus on your health and recovery without the added stress of lost wages, giving you peace of mind.

Short-Term Disability (STD) coverage begins on the eighth day of an injury or illness and continues up to your recovery or 12 weeks, whichever comes first.
Your weekly benefit will be reduced by any benefit you receive from other sources of income (i.e., state disability payments, workers’ compensation, etc.).

If you are unable to return to work after 90 days, Long-Term Disability (LTD) kicks in. This benefit begins after 90 days of disability and continues to your recovery or normal retirement age, standard ADEA whichever comes first.
The waiting period for this benefit is 3/12.
Flexible Spending Accounts (FSA) are a powerful tool that allows you to set aside pre-tax dollars for eligible healthcare and dependent care expenses. Imagine being able to pay for things like medical co-pays, prescriptions, childcare, and even some over-the-counter items with tax-free money! Not only does this help you save on taxes, but it also makes managing your out-of-pocket costs much easier. By planning ahead and contributing to an FSA, you can enjoy significant savings and greater financial flexibility throughout the year.

Most medical, dental, and vision care expenses that are not covered by your health plan (such as copayments, coinsurance, deductibles, eyeglasses, and doctor-prescribed over-the-counter medications) for you and your dependents.
Rollover: For the FSA plan year ending on 6.30.2025, you may rollover $640 of unused FSA funds into this new FSA plan year. You must submit claims by 9/30/2025. For the FSA plan year beginning on 7.1.2025 and ending on 6.30.2026, you will be able to rollover $660 of unused funds to the next plan year and will need to submit claims by 9/30/2026.

Dependent care expenses (such as daycare, after-school programs, or eldercare programs) so you and your spouse can work or attend school full-time. This account cannot be used for reimbursement of healthcare expenses for dependents.
Grace Period: You can still use unspent funds through September 15, 2026. Submit claims for expenses incurred during the 7.1.25-6.30.26 plan year by 9/30/2026.
Grace Period: For the DCFSA plan year that is ending on 6.30.2025, you can still use unspent funds through September 15, 2025. For the DCFSA plan year beginning on 7.1.2025 and ending on 6.30.2026, you can use unspent funds through September 15, 2026.
With Commuter benefits, you can set aside money on a before-tax and after-tax basis for eligible expenses incurred as part of your commute to work. There are three commuter accounts -parking, vanpool, and mass transit.

A Health Savings Account (HSA) is a triple tax-advantaged savings account that allows you to save for qualified healthcare expenses and retirement.
Think of it as a 401(k) for your health care. The money in your account can be used to pay medical, dental, and vision expenses for both you and your qualified dependents. Contributions are deducted from your paycheck tax-free, and you can use your savings immediately.
Upward Health will contribute funds to your Health Savings Account (HSA) each month to help cover your healthcare expenses—up to $1,000 per year for Employee-only coverage and up to $2,000 per year for Employee + Dependents coverage. In addition to the company’s contribution, you may also choose to make your own contributions through payroll deductions up to the allowable IRS amount for that year.
