Who Is Eligible?
Employees
EBNHC is pleased to offer a full range of benefits to employees who work 18 or more hours per week. You're eligible for benefits on the first of the month following your date of hire.
Dependent(s)
Your family is important to us too. You can enroll your qualified dependents for medical, dental, vision and life insurance benefits. Eligible dependents include:
- Legal spouse
- Natural, step, legally adopted or eligible foster children through the last day of the month in which they turn 26
- Child(ren) recognized under a Qualified Medical Child Support Order
- Qualified same-gender or opposite-gender domestic partner
Domestic Partners
Under federal law, EBNHC’s contribution toward the cost of healthcare coverage for your domestic partner and his/her dependents is considered taxable income to you. You may wish to consult with a tax adviser for more information.
Did You Know?
You'll need to provide documentation (marriage certificate, birth certificate, etc.) to enroll your dependents in coverage.
Enrollment
New Employees
New employees are eligible for benefits on the first of the month following the date of hire. If you do not enroll when you're first eligible, you will not be able to enroll until the next annual open enrollment period, unless you have a qualifying life event change.
Open Enrollment
EBNHC typically conducts open enrollment during the fall. Open enrollment is your once-a-year opportunity to enroll in or make changes to benefits for the next plan year. Your choices will be effective for one full year unless you have a qualifying life event change.
Qualifying Life Event Change
We understand that, when life happens, your benefit needs might change. Because some of these benefits are paid for with pre-tax dollars, federal regulations restrict when you can make changes to your enrollment. Changes are not allowed unless they are made during open enrollment or are classified as a qualifying event. Qualifying events are defined as:
- Family Status: Marriage, divorce, birth, adoption or death
- Employment Status: Starting a job, leaving a job or changing hours
- Coverage Status: Loss of coverage for yourself or an eligible dependent
- Dependent Status: Loss of dependent status
The changes to your coverage must be made within 30 days from the date of the qualifying event.
How to Enroll
Enrolling in benefits is easy and convenient. Just log on to Workday and go to your inbox.
Consolidated Omnibus Budget Reconciliation Act (COBRA)
COBRA coverage allows you to temporarily continue your group healthcare coverage in situations where it would otherwise cease, due to termination, layoffs, or other changes in employment status; death of the employee; legal separation or divorce of a dependent spouse; eligibility for Medicare; or a dependent child becoming independent. Although this coverage is at the employee’s expense, it can extend up to 18 months for covered employees and their dependents, up to 29 months if a social security-determined disability occurs during the first 60 days of COBRA coverage, or up to 36 months for certain qualifying events. For more details, contact the benefits team.

Dental Benefit
EBNHC offers dental insurance to our employees through BCBSMA Dental. The Dental Blue Freedom plan offers a large network of dentists nationwide. Dental Blue Freedom members can choose from the Dental Blue PPO (preferred dentist) or Dental Blue (participating dentist) networks. You'll receive the greatest value if you visit a preferred dentist, while using a network dentist will minimize your out-of-pocket expenses.

- Receive at least one service during the benefit period
- Remain a member of the plan throughout the benefit period
- Do not exceed the claim payment threshold in the benefit period
BCBSMA also enhanced their routine-care benefits so members can now get fully covered, routine dental checkups twice a year, without having to wait six months in between.

Flexible Spending Accounts
Do you anticipate medical, dental or vision expenses this year that won’t be covered by your insurance? Do you have a child in daycare or take care of an elderly parent that needs care during your work day? If glasses, braces or daycare for loved ones are on your horizon, you can receive some financial relief by opening a Flexible Spending Account (FSA).
When you participate in a flexible spending account, you'll lower your taxable income by setting aside pre-tax dollars to pay for eligible expenses. View the list of eligible expenses by clicking the button below as you plan your contribution amount.
Participants have an additional two-and-a-half-month grace period to incur claims for the previous plan year. Participants may incur expenses through March 15, 2024 for the plan year that will end December 31, 2023. You have until March 31, 2024 to file healthcare or dependent care claims incurred for 2023.
Healthcare FSA
You can contribute up to $3,050 through pre-tax payroll deductions for expenses like medical, dental and vision copays and deductibles. The amount you elect cannot be changed during the year unless you have a qualifying event.
Dependent Care FSA
You can contribute up to $5,000 ($2,500 if married and filing separately) through pre-tax payroll deductions for dependent care, such as caring for children under age 13 and for elders. The amount you elect cannot be changed during the year unless you have a qualifying event.
Auto and Home
Take advantage of group discounts by purchasing auto and home insurance through our partners. There are two options to choose from: Farmers and MAPFRE.
Visit Farmers or call 1-800-438-6381.
You can pay for coverage in four ways:
- Payroll deduction
- Automated payment
- Direct billing
- Credit card
Save 4% on your auto insurance, with potential savings up to 19% when you combine your home and auto insurance.
There is no payroll deduction option for MAPFRE but you can take advantage of great discounts via direct bill.
Group Legal
You never know when you’ll need legal advice. With MetLife Legal, you, your spouse and dependents will have access to attorneys and resources to help with your legal needs.
Common legal issues include:
- Consumer protection
- Defense or civil lawsuits
- Immigration assistance
- Juvenile court defense
- Real estate matters
- Estate planning
- Family law
- And more
Use a network attorney for a covered matter and you'll have:
- No deductibles or copays
- No waiting periods or claim forms
- No limits on usage
Did You Know?
- The cost for the plan will automatically be deducted from your paycheck.
- Once you enroll, you must remain in the plan for the entire plan year.
Retirement
Be a savvy saver and contribute to the EBNHC retirement plan offered through Aegis Retirement Partners and Empower. This plan offers:
- Automatic Enrollment: You are automatically enrolled in the plan, unless you opt out.
- Contributions: Choose to contribute on a pre-tax basis or Roth post-tax basis up to the IRS maximum.
- Maximum Salary Deferral: The 2023 maximum is $22,500.
- Catch-up Contribution: Anyone age 50 or older is eligible for the catch-up contribution. The 2023 catch-up contribution is $7,500 per year.
- Free Money: EBNHC makes a matching contribution of $0.65 for every dollar deferred up to 4% of your salary.

CONTRIBUTING PRE-TAX OR AFTER-TAX (ROTH) — WHICH IS RIGHT FOR YOU?
Pre-tax (Traditional) Versus After-tax (Roth) — What’s the Difference?
Pre-tax means that the dollars contributed are taken out of your paycheck prior to taxes being applied. Therefore, you are lowering your taxable income and do not pay taxes on those dollars contributed, but you will pay taxes when you withdraw the money.
After tax means that the dollars contributed to the retirement plan are taken out of your paycheck after taxes have been calculated and applied. Therefore, you are paying taxes on those dollars at the time they're earned and will not pay when you withdraw.
Should I Contribute to a Traditional 401(k), a Roth 401(k) — or Both?
The answer comes down to taxes. Do you think you’ll be better off paying taxes on the money now or later? In general, the longer you have until you retire and if you expect your tax rate in retirement to be higher than your current rate, the more likely you are to benefit from a Roth 401(k) account versus a Traditional 401(k).
Example: Let’s look at two 45-year-olds, Jerry and Elaine, both of whom contribute $5,000 to their 401(k) plan in the same year — Jerry to a traditional 401(k) and Elaine to a Roth 401(k). They won’t need the money until they’re 75. With tax rates and investment returns being equal, Jerry’s initial contribution will have grown to $27,404 after paying taxes at the time of withdrawal, while Elaine will have $38,061 as she paid taxes at the time of her contribution. Choosing the Roth 401(k) gave Elaine $10,657 more than Jerry.
ARE YOU PREPARED?
Many financial experts think you’ll need to replace 70% – 90% of your income to maintain the same standard of living.
Make sure you are contributing at least 4% to take advantage of the company match. You can adjust your investments and contributions at any time.
Did You Know?
While setting aside Roth or post-tax dollars won’t save you on taxes now, when you withdraw the money in retirement, you will not have to pay taxes then as long as certain requirements are met. This is great news if you expect taxes to be higher for you in retirement than they are now.

Life and AD&D
EBNHC provides you with Life and AD&D. The best part – you don’t pay for them!
- Group Term Life: 1x your annual base salary up to $300,000
- Group Accidental Death & Dismemberment (AD&D): one times your annual base salary up to $300,000
Please note that your benefit reduces to 50% at age 75.
Do you have a mortgage? Do you have dependents that rely on your income? If so, you might want to consider added financial security by purchasing more insurance for yourself and your family. You pay the full cost for this coverage with after-tax dollars:
- Supplemental Employee Life Insurance: Up to three times your annual salary with a maximum of $500,000; amounts over $300,000 may be subject to
- Spouse Life Insurance: Elect increments of $10,000 up to $20,000. Not to exceed 50% of employees’ supplemental life election.
- Child Life Insurance: $5,000 or $10,000 up to age 26.
What’s Evidence of Insurability?
Evidence of Insurability (EOI), is an application process in which the insurance carrier evaluates your current and past health to determine if you're eligible for coverage. While it sounds intimidating, it's a simple process of completing a medical exam and questionnaire. Many pre-existing conditions may be acceptable for coverage.
IMPORTANT! If you've been previously denied coverage under this plan, you may be subject to EOI.
Don’t miss your chance to purchase financial protection and peace of mind for your loved ones during open enrollment. If you don’t enroll, you'll have to wait for the next open enrollment period.
If you did not elect supplemental coverage when you were first eligible, all amounts may be subject to Evidence of Insurability. Make sure your beneficiary information in Workday is up to date!
A life insurance benefit that exceeds $50,000 is considered taxable “imputed income” by the IRS, and the premiums will be included in your W-2 earnings at the end of the year.
Disability
The financial consequences of a lengthy disability can be very serious. Disability insurance protects a portion of your income should you become ill or injured, or during a pregnancy leave, to help you pay your bills until you can return to work.
Individual Short-Term Disability (STD)
Are you expecting to become pregnant? Are you worried that you will be in a position of not being able to work and will need financial protection before the LTD benefit kicks in? If so, you may want to consider purchasing STD insurance through Unum at group rates. STD benefits replace up to 60% of your monthly salary to a maximum of $5,000. This benefit is not taxed.
Long-Term Disability (LTD)
You automatically receive LTD insurance through Unum at no cost to you. If you find yourself suffering with a covered disability for more than 90 days, you'll receive an LTD benefit that replaces 60% of your basic monthly earnings to a maximum of $15,000 per month. If you go out on an LTD claim, the benefit you receive will be taxed.
Travel Benefit
Don’t forget this travel essential!
Pack your worldwide emergency travel assistance phone number and leave travel worries at home. For more information, click the button below.
Did you know?
Benefit payments will be reduced by any other sources of deductible income that you may be receiving. Disbility benefits integrate with state mandated disability plans if applicable.
Voluntary Benefits
You have the option of purchasing additional coverage from Unum. The voluntary benefits on this page are subject to a pre-existing-condition limitation. If you have a pre-existing condition (a condition for which symptoms existed that would cause a person to seek treatment from a physician or for which a person was treated or received medical advice from a physician or took prescribed medication) within a 12-month period before your coverage effective date, benefits will not be paid for a disability period if it begins during the first 12 months after the policy begins.
Accident Insurance
Accidents happen when you least expect them. Accident insurance pays a lump-sum benefit for both on- and off-the-job accidents. The amount you receive depends upon the accident you sustain.
Critical Illness Insurance
Critical Illness Insurance provides a lump-sum benefit when you are diagnosed with a covered critical illness.
Hospital Indemnity Insurance
An unexpected hospital stay can add a huge burden – both financially and physically. Hospital Indemnity coverage pays a lump-sum benefit to help you cover the expenses from a hospital stay.
Interest-Sensitive Whole Life Insurance
Elect Whole Life Insurance through Unum for yourself, your spouse and your children. Whole Life Insurance works differently than term life insurance:
- Same Coverage, Same Price: Once you purchase coverage, your benefit and premiums do not change as you age.
- Cash Value: Whole Life Insurance earns interest at a guaranteed rate. This is also known as “cash value.” You can borrow from the cash value or buy a smaller, paid-up policy with no premiums due.
