Benefits Showcase
Plan Year: July 1, 2023 - June 30, 2024
New-Hire Enrollment
If you're a new hire, you have 30 days from your date of hire to elect benefits on ADP Lifion (except for the 401k Plan & ESPP). You'll receive a letter at your home address on file explaining the new-hire enrollment process, as well as receive internal Herc communications through your email. During that 30-day window, you can make changes anytime. After the 30-day window ends, the last confirmed benefit elections will be your benefits for the remainder of the plan year. Choose your benefits wisely, this will be your only opportunity to make any benefit elections/changes until open enrollment or if you experience a qualifying event during the plan year.
If you do not make any benefit elections during your new-hire period, you'll automatically be enrolled in the below company-provided benefits:
-
Basic life (one times annual base pay)
-
Short-term disability insurance
-
Core long-term disability insurance (40% of base pay)
Any company-provided or elected benefits become effective the first of the month coinciding with or following 30 days of continuous employment. Be sure to print or save your Benefits Confirmation Statement.
Open Enrollment
Open enrollment is your annual opportunity to make changes to your benefits that remain in effect for the full plan year, July 1 – June 30. This event typically occurs in May each year and benefits become effective on July 1. During the open enrollment window, you can make changes anytime. After the open enrollment window ends, the last confirmed benefit elections will be your benefits for the plan year. Be sure to print or save your Benefits Confirmation Statement.
Don’t worry, we send out lots of communications before and during open enrollment, so you don't miss your one time per year to make benefit changes!

Qualifying Life Event
Certain changes in your life situation are known as qualifying life events – things like a loss of health coverage, a change in your household, or a change in residence.
These qualifying events give you the opportunity to enroll or make changes outside of open enrollment. You only have 31 days from the date of your event to go on to ADP Lifion and report an event.
Qualifying life events typically include, but are not limited to:
-
Becoming newly married or divorce
-
Having a baby or adopting a child
-
Experiencing a death of the insurer in the family
-
Losing health insurance coverage due to job loss
-
Losing eligibility for Medicare, Medicaid, or Children’s Health Insurance Program (CHIP)
-
Turning 26 and losing coverage from your parent’s health insurance plan
-
Moving to a different zip code or state that changes your health plan area
You will be required to provide documentation as proof of the qualifying event to ADP. ADP will reach out to you requesting needed documentation and provide a deadline to provide such information. If you do not provide the requested proof, the changes to your benefits will not be allowed.
Send the benefits team your questions!
Covering a Dependent on Benefits
If you’re enrolling a dependent, you must provide proof that he or she is eligible. ADP will send a verification communication via your Herc Rentals email or the preferred email that you've listed on record. You must return the requested information by the deadline, otherwise, your dependent’s coverage will be canceled.
Covering a spouse/domestic partner?
If you cover a spouse/domestic partner who's eligible for medical coverage through his or her employer (other than Herc Rentals) and they waive that coverage, you must pay a $50 per week surcharge. If your spouse/domestic partner doesn’t have access to his or her own coverage, you won’t have to pay the surcharge. Your spouse/domestic partner’s eligibility for coverage will be verified following your enrollment.
Who is eligible?
- Your legally married spouse/domestic partner
- Your child (including any legal stepchild, adopted child, or foster child) younger than 26

Medical Plan Options
Good health starts with a great medical plan. And, as an eligible employee, you can take advantage of the option that best fits your needs. Although there are some differences in our plans, there are some things they all have in common:
- All medical plans are administered by UnitedHealthcare
- Your medical provider network is determined by your residential state
- All plans provide comprehensive coverage and cover the same services for you and your enrolled dependents
- Free preventive care is included at in-network facilities
- Discounted fees for care provided by in-network providers
- An out-of-pocket maximum, which is the maximum you would have to spend for eligible medical expenses before your care is covered at 100 percent
The chart below shows all the medical plans that Herc offers. Please note that the Value Plan and HRA plan are closed to new entrants, and the HRA plan is going to sunset on June 30, 2025.
This information does not apply to Hawaii employees. To learn more about Hawaii specific medical/pharmacy plans, please reach out to your benefits team.

HSA vs. HRA

How to Optimize Your HSA
If you're enrolled in HSA Plan 1 or 2, you'll receive a debit card from Optum Financial. This will provide access to the company-provided HSA contributions and your contributions if you're making contributions via payroll.
You can access your Optum account directly from www.myuhc.com!

Medicare Advisory Service
If you're turning age 65 soon, take advantage of our free Medicare advisory service.
Pharmacy Benefits
All our medical plan options include prescription drug benefits. You can review all the details below. MagellanRx manages our pharmacy benefits for all our medical plans. Be sure to check your formulary for a full list of the medications covered by the MagellanRx Precision Formulary. Generic medications are usually less expensive than brand names and you can always ask your doctor about using lower-cost medications. Maintenance medications must be filled at CVS, Kroger (and affiliates), Walmart and Walgreens or through our mail-order program to avoid a penalty.
ArchimedesRx manages our specialty pharmacy benefits for all our medical plans. All specialty Rx is exclusively mail order and no courtesy fills are allowed at any retail pharmacies. All specialty prescriptions will be dispensed by AcariaHealth. For questions regarding our specialty Rx program, contact ArchimedesRx at 1-888-504-5563. Please refer to the Open Enrollment Benefits Guide 2023 for details regarding prescription benefits for the frozen HRA Plan and Value Plan.

Tools for Wellness
Get Healthy. Get Rewarded.
Herc offers you ways to earn credits to offset your medical plan premiums or add credits to your HSA account if enrolled in an HSA medical plan when you complete certain activities.
Tools for Wellness
You’ll pay premiums for medical benefits over time with a set amount coming out of each paycheck. The Tools for Wellness program works the same way; any credits you earn are also distributed evenly throughout the plan year to help offset the medical plan premiums or you can receive credits as an HSA contribution. Your enrolled spouse/domestic partner can also earn credits by completing the activities.
Check out Rally®, our wellness web portal, at www.myuhc.com to learn more about the Tools for Wellness program and complete your Rally Health Survey to earn your $50 e-gift card and then you’ll have the opportunity to earn full credits of up to $750 (includes e-gift card) each for you and your covered spouse/domestic partner.
The full credit earning period runs from March 1 to the last day of February each year. Credits you earn during this period will apply to the next medical plan year (July 1 to June 30).
Your participation in the program is completely confidential. Herc Rentals will receive only summary information about the overall health status of its employee population.
Dental
Here’s something to smile about, we offer comprehensive dental benefits to help cover regular cleanings, emergency dental care and more! We have two dental plan options; a Dental PPO and a Dental HMO (DHMO) administered by Cigna Dental.
The PPO plan includes in and out of network providers, but your benefit dollars will go further, and your out-of-pocket costs will be lower, if you use a PPO network dentist.
The DHMO plan has lower premiums and no annual limits, however, the network is much smaller than the PPO network and the plan is not available in all areas. You must reside in a zip code where the plan is available. In the DHMO, you’re assigned a network dentist who will provide all dental care or refer you to specialist, if necessary.
You can change to a different DHMO dentist from the assigned network dentist as long as the provider is in the network. For details on the DHMO benefits, check out the Cigna Patient Charge Schedule.
The chart below is a snapshot of the dental plans and benefits associated with each plan. If you enroll in dental benefits, create an account on www.mycigna.com. This site or app provides your plan information at a glance, ability to locate network providers, access to digital insurance cards, ability to view claims and so much more!
This information does not apply to Hawaii employees. To learn more about Hawaii specific medical/pharmacy plans, please reach out to your benefits team.
Vision
You only get one set of eyes, and we know how important it is to take care of them. In your quest for good health, an eye exam can spot early signs of diabetes, high blood pressure, high cholesterol, and heart disease. Not to mention cataracts and glaucoma. That’s why we offer vision insurance through EyeMed to help cover the cost of eye exams, frames, lenses and more.
You can have an eye exam each year for a $20 copay, and you have a choice between contact lenses or prescription glasses. The amount covered for contact lenses or glasses depends on the kind of lenses, contact lenses, frames, and extra features you choose. Check out the below chart for vision benefits.
If enrolled, be sure to create an account at
www.eyemed.com to view network providers, claims,
and so much more!
Flexible Spending Accounts
Whether you have out-of-pocket health care expenses or dependent care costs, you may want to take advantage of a flexible spending account. An FSA can save you money and cut your taxes. Before enrolling, you should get a thorough understanding of how an FSA plan works.
Herc offers three types of FSAs: a healthcare FSA, a limited purpose healthcare FSA, and a dependent care FSA.
A healthcare FSA is used solely for qualified healthcare costs, such as medical, dental, vision and other typical medical expenses. Qualified expenses may also include copayments and deductibles, but you cannot pay health insurance premiums with your FSA.
A limited purpose healthcare FSA is only for those who are enrolled in a high-deductible health plan. You are limited initially to dental and vision expenses until you reach your medical plan deductible. After you reach the deductible, you may use as a regular healthcare FSA.
A dependent care FSA is similar to a healthcare FSA but can cover only qualified dependent care expenses. You may use your dependent care FSA only for expenses for any child under the age of 13 or older dependents who are physically incapable of caring for themselves.
Qualified expenses include preschool, nursery, before and after daycare, summer daycare and other dependent care-related expenses. Since Herc offers both a dependent care FSA and healthcare FSA, you may choose to enroll in both.
For the healthcare FSA and the limited purpose FSA account, you'll receive a debit card to use for eligible expenses. You can log on to www.myuhc.com and view your balances, file claims manually 24/7. If you need to order a new card, call UHC at 844-255-3068.

Basic and Optional Life
Life insurance protects you and your family in the event of a death from a serious illness or accident.
Herc Rentals provides life insurance coverage called “basic group life” at one times your annual base pay at no cost to you. This coverage begins the first of the month coinciding with or following a 30-day waiting period as a new hire.
As your pay increases with your tenure at Herc, so does your basic life amount. This coverage is guaranteed, which means you don’t need to take a life insurance medical exam or answer health questions to qualify.
Annual base pay means annual pay excluding overtime, incentive pay, bonuses and any other forms of special compensation. For commission salespeople, base pay equals 80% of the prior year’s annualized Herc Rentals W-2 commissions (updated each January) or regular base pay, whichever is greater. Maximum guaranteed issue is $750,000 for basic life.
In addition to basic life, you can elect up to 5 times your annual base pay in life insurance, called optional life. The combined maximum between the basic life and optional life cannot exceed $3,450,000. Optional life requires you to provide proof of good health to the insurance provider, The Hartford by answering health questions or even having a medical exam. This process is sometimes called “Evidence of Insurability” or EOI. You can elect optional life coverage as a new hire or during annual Open Enrollment.
Why Get Life Insurance?
After electing this coverage, you will receive an email from The Hartford outlining the EOI process. After completing the EOI process, you will receive notification with an approval or denial. If approved, your coverage will begin that day.
Accidental Death & Dismemberment (AD&D)
Electing this coverage may be smart because accidents are a leading cause of death in the US. Depending on how you make a living, a loss of a limb, speech or hearing could make it difficult or impossible for you to continue working as you did before an accident. Herc Rentals offers this coverage through Prudential.
You can elect coverage for yourself only or for you and your family up to ten times your annual base pay or $750,000, whichever is less.
If you are covering a spouse only, you would receive 60% of your coverage amount. If you are covering a child(ren) only, you would receive 20% of your coverage amount. If you are covering a spouse and child(ren) you would receive 50% for a spouse and 10% for a child(ren), of your coverage amount.
Short-Term Disability Insurance (STD)
Simply put, a short-term disability (STD) is an instance that puts you out of work temporarily, such as an injury, illness, or medical procedure. When one of these incidents happen and you cannot earn an income, you may be able to qualify for benefits during this time. Short-term disability insurance is a plan specifically designed to help with the financial stressors that can arise when an injury negatively impacts your ability to work.
To apply for short-term disability, you must contact Matrix to start the process. You will then be required to have your attending physician complete a medical certification to validate the need for a leave. You will also be evaluated at that time under the Family Medical Leave Act, FMLA. You can reach out to Matrix via telephone, online or the Matrix mobile app.
Exempt employees have short-term disability benefits through Herc’s policy, Short-Term Disability Pay Policy. The benefit amount paid depends on your length of service. This policy provides a benefit up to 26 weeks.
For non-exempt employees, Herc provides a core short-term disability plan insured through Reliance Standard. The benefit amount is 50% of your weekly base pay up to $300 per week when on an approved STD leave for up to twenty-six weeks.
You can also enroll in supplemental STD coverage which increases the benefit paid to 50% of your weekly base pay, up to a maximum of $500 per week. If you waive or end coverage, you will have to prove your good health to the insurance company if you enroll or re-enroll in the future. This supplemental is not available for enrollment if you live in a state that provides a higher benefit.
Long-Term Disability (LTD)
Herc provides core long-term disability insurance equal to 40% of your monthly base pay (up to $13,000 per month) through Reliance Standard. This coverage is for those who qualify medically and cannot return to work after 26 weeks of STD. You can elect to Buy-Up additional coverage up to 60% of your monthly base pay (up to $16,000 per month) as new hire and or during Open Enrollment. If during Open Enrollment, you will be required to submit proof of good health to be enrolled.
Just like life insurance, annual base pay means annual pay excluding overtime, incentive pay, bonuses and any other forms of special compensation. For commission salespeople, base pay equals 80% of the prior year’s annualized Herc Rentals W-2 commissions (updated each January) or regular base pay, whichever is greater.
Employee Assistance Program (EAP)
We’re here to support you at work every day. But we also know that sometimes, real life presents its own challenges. We’re committed to helping you meet these challenges head-on and offer a free Employee Assistance Program or EAP. Your EAP provides confidential access to professionals 24/7 who will work with you in person or by phone to address personal and work-related issues. Your spouse and eligible dependents have access to the program as well. You can use this service at no cost to get help and advice for many issues including family issues, stress, and substance abuse.
For more information on this service, review the flyers below.
Surviving Family Member Benefits
Helping to care for families during severe times of need remains a priority at Herc Rentals. We recognize the difficulty and pain in losing a loved one and have gathered the following benefits and resources for immediate family members of deceased employees in our Custom Benefit Program.
This program is automatically extended to your immediate family members.
Health Coverage – Continuation of medical, dental and vision benefits for six months for surviving family members. Herc Rentals will pay the full cost of COBRA coverage during this period.
Monetary Assistance – Continuation of your base salary for four weeks to your surviving spouse/domestic partner or dependent children.
Counseling and Assistance – Continued access to our Employee Assistance Program (EAP) for six months for surviving family members.
Accident Insurance
Accident Insurance pays a cash benefit directly to you (unless otherwise assigned) when you or a covered family member are injured in a covered accident. No one likes to think about the possibility of an accident, but the very likelihood is inescapable. Think about it: Have you or anyone in your family ever had an automobile accident? Slip and fall? How about a bicycle or skiing accident? When was the last time you saw the inside of an Emergency Room? If you’re like most Americans, it wasn’t too long ago!
Accident insurance provides a hedge against this possibility, paying a fixed, lump-sum benefit for injuries resulting from a covered accident — up to and including death.
Coverage is through Reliance Standard. There are no health questions or physical exams required to purchase coverage plus there is an annual wellness benefit if you get a covered health screening. For details of covered injuries, view the certificate.
Critical Illness Insurance
A critical illness like cancer or heart attack can lead you to having many out-of-pocket incurred costs and unable to work. There’s lost income — even if you have disability coverage, which only pays a portion of your regular earnings — along with co-pays, deductibles, family and living expenses, and even transportation costs to and from treatment. Many of these additional costs can contribute to the pressure you are already under at the worst possible time. Critical Illness Insurance can help to cover some of those gaps in your financial plan that you may not have even known existed. With Critical Illness Insurance, the lump sum benefit you receive at the diagnosis of a critical illness can help offset those expenses not covered or not entirely covered by other sources of income or insurance. This coverage is through Reliance Standard and there are no health questions or physical exams required to purchase coverage plus there is an annual wellness benefit if you get a covered health screening.
The coverage available to your spouse or domestic partner and children depends on which option you select for yourself. If you choose $15,000 in coverage, the coverage limit for your spouse or domestic partner is $7,500. Each eligible dependent child can receive $3,750 in coverage at no additional cost.
If you choose $30,000 in coverage, the coverage limit for your spouse or domestic partner is $15,000. Each eligible dependent child can receive $7,500 in coverage at no additional cost.
Group Legal Plan
The Group Legal Plan, offered through Hyatt Legal Plans (a MetLife company), provides you and your eligible family members with fully covered legal services from a national network of attorneys.
- Plan attorneys: Select one of more than 14,000 experienced attorneys from the Hyatt Legal Plans nationwide network. You won’t pay for covered services and there are no limits on usage.
- Non-plan attorneys: You’ll be reimbursed for covered services according to a set fee schedule.
