Medical Plans
ABC Company is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. We offer two medical plans administerd though Loyal Healthcare to fit your budget and needs.
PPO 1500 Plan
The PPO 1500 plan includes higher monthly premiums but lower annual deductibles. The PPO 1500 plan offers deductibles and copays when receiving treatment. If you are enrolled in the PPO 1500 plan you can choose to contribute to a tax-advantaged health care flexible spending account to help pay for health care expenses not covered by your plan.
With the PPO 1500 medical plan, you have the choice of two benefit tiers: Plan providers (in-network) and non-plan providers (out-of-network).
HDHP Plans
The High Deductible Health Plans (HDHP) offer lower monthly premiums and higher deductibles, making these plans an ideal choice for those who want to save on regular healthcare costs and are prepared for higher out-of-pocket expenses when receiving treatment. ABC Company offers two different HDHP plans to choose from.
What Is A High Deductible Health Plan?
High deductible health plans (HDHP) reward you for taking an active role as a health care consumer and making smart lifestyle choices and decisions about your health care spending. ABC Company's HDHP 2500 and HDHP 1800 plans are paired with a tax-advantaged Health Savings Account, or HSA, that you can use to pay eligible out-of-pocket expenses like your deductible, coinsurance, and copay.
How an HDHP Works
Deductible
You pay all medical and prescription costs until you meet the plan's annual deductible, which you can cover using your HSA.
Preventive Care
To emphasize the importance of wellness, in-network preventive care is covered at 100%.
Coinsurance
Once you meet the deductible, you and ABC Company share any further health care costs up to the out-of-pocket maximum.
Out-of-Pocket Maximum
There's a limit to how much you'll pay out of pocket each year. Once you meet this limit, 100% of your eligible expenses are covered for the remainder of the year—you pay $0.
Medical Plan Summary
Note: Out-of-network providers do not have to adhere to the reasonable-and-customary fees when determining what they will charge. Therefore, your out-of-network costs will be more for each of the plans. See your Summary Plan Description (SPD) for specific out-of-network coverage.
*ABC Company’s HDHP 2500 and HDHP 1800 plan deductibles are not embedded, which means if you cover a member of your family, the entire family deductible must be met prior to the plan paying. The out-of-pocket (OOP) maximum is embedded, which means if an individual meets the OOP maximum, the individual will receive 100% coverage for the remainder of the year. The entire family OOP maximum is not required to be met for an individual with family coverage to meet their individual OOP maximum and receive 100% coverage.
**2025 HSA maximum contributions are $4,300 for single and $8,550 for family.

HSA and FSA
ABC Company provided both an HSA and FSA to help employees budget and save for qualified medical expenses incurred over the course of the plan year.
Health Savings Account
A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you're enrolled in one of the HDHP plans, you're eligible to contribute on a pre-tax basis to an HSA. ABC Company will match your HSA contribution based on the schedule below, depending on which plan you enroll in. You can use these funds to cover medical, dental and vision expenses.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can also choose to save it for eligible healthcare expenses tax-free in retirement.
What Is A Health Savings Account?
When you enroll in the HDHP 2500 or HDHP 1800 plan, you’re eligible to contribute to a tax-advantaged* Health Savings Account (HSA), which you can use now to pay current health care expenses for yourself, your spouse, and your dependent children, or save it for future expenses including during retirement. You get to decide.
How an HSA Works
1. Anyone Participating in an HDHP plan can contribute
The IRS sets limits on how much can be contributed (you and ABC Company combined) pretax* to an HSA each year. For 2025, the limits are $4,300 for self-only, and $8,550 for family.** ABC Company will match every dollar you contribute to your HSA up to the limits shown below:
HDHP 2500 Match
-
$600 for self-only
-
$1,800 all other coverage
HDHP 1800 Match
-
$300 for self-only
-
$900 all other coverage levels
Note: For new hires, these amounts are prorated based on the number of months remaining in the calendar year following your enrollment date.
Anyone 55 or older by 12/31/2025 can make an additional catch-up contribution of $1,000.
2. You Withdraw Money Tax-Free*
Use the money in your HSA to make tax-free payments toward eligible out-of-pocket medical expenses. These include your plan deductible, coinsurance, prescription costs, and anything not covered by your medical plan.
3. Let Your Funds Roll Over
Once contributed, funds in your HSA are yours to keep for as long as you want until you use them. Take the account with you if you change jobs and after you retire.
4. Invest Your HSA
When your account reaches a minimum balance, you have the option to invest it in a range of mutual funds through Fidelity Investments.
* You don’t pay federal income taxes on any HSA contributions. You may have to pay state and local taxes, however, depending on your residence. Speak with your tax advisor for details.
** Family includes: Employee + Spouse and Employee + Child(ren).
Flexible Spending Account
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to $3,300 of pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year.
Dollars contributed to an FSA are tax-free and the entire amount is available on the first day of the plan year. However, any money not used by end of the plan year is forfeited.
An FSA covers eligible out-of-pocket health care expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent.
Dependent Care FSA
A Dependent Care Flexible Spending Account (DCFSA) is a pre-tax benefit account that allows you to set aside money to pay for eligible dependent care expenses. These expenses can include childcare, adult daycare, preschool, summer day camp, and before or after school programs.
For 2025, the limit is $5,000 for single filers and couples filing jointly, and $2,500 for married couples filing separately. You are not allowed to change your contribution amount for the year unless you experience a qualifying event, such as a change in your marital status, employment status, or the number of dependents you have.
