This year, we want to help you be the healthiest you – at work and home. That’s why we offer the Best Damn Benefits for you and your family.
You have until the end of the month you are hired to enroll in benefits. Benefits are effective the 1st of the following month of your hire date.

Live Health Online
This benefit provides services for those with and without our insurance plans. The copay is $20 for those on the Mid and High medical plans and $59 for those on the Low plan and those who don’t use our insurance plan.
Hearing Aid Coverage
This benefit covers exams and hearing aids for children under the age of 18 every twelve months; for those over the age of 18, this benefit is covered every 36 months. Must meet a deductible and then coinsurance applies.
Sword Healthy Back and Joints Program (starting July 1, 2024)
Start your journey towards living pain free at home with the LiveHealth Online Healthy Back & Joints Program powered by Sword. Digital physical therapy for back, joint and muscle pain which are proven to work better than in-person physical therapy.
The Healthy Back & Joints Program is available through LiveHealth Online at no cost to employees, spouses, and dependents 18 and older on the Anthem Medical Plan.
Bloom Women's Pelvic Health Program (starting July 1, 2024)
Bloom is an innovative digital pelvic therapy solution developed by Sword Health. Members are partnered with Pelvic Health Specialists, all of whom have Doctor of Physical Therapy degrees. It is designed to address pelvic dysfunctions such as bladder leakage, bowel issues, and pelvic pain.
IVF Benefit Coverage
This benefit covers infertility diagnostic services, and qualifying employees have an unlimited maximum per member.
FSA and Dependent Care FSA
The max FSA amount is $3,200, with an allowable rollover of $610. The max amount for Dependent Care FSA is $5,000. The Liberty Energy ER code is RMRLIB. FSA funds can only be used in the current plan year (Example: You can't use your funds from this year to pay for a claim in 2021.)
Your prescription drug coverage is important to your health. With pharmacy benefits from Anthem, powered by CarelonRx, we make it simple for you to track and manage your medications — all in one convenient place. You can refill and renew your prescriptions, find a pharmacy and even look up the price of a drug before you get it. To get started, log in to anthem.com or use the Sydney mobile app.

Home Delivery Program
Home delivery is provided by CVS Caremark. Certain controlled substances and prescriptions that have expired will require a new prescription from a doctor.

That is why Liberty Energy has partnered with Delta Dental of Colorado to provide our employees with affordable dental coverage. We are excited to offer a new High Dental Plan this year, complete with enhanced orthodontic coverage for you, your spouse or your children up to age 26!

The dental plans offered through Delta Dental of Colorado are PPO plus Premier plans. You and your family members may visit any licensed dentist, but will enjoy the greatest out-of-pocket savings if you see a Delta Dental PPO dentist.

Delta Dental is continuing to offer Virtual Visits for enrolled members! You have access to a dentist 24/7, 365 days a year available through TeleDentistry. This is not intended to replace your existing dental provider but an option for members to utilize if there is a dental emergency to avoid emergency room visits and charges. TeleDentistry will provide an initial consult and can write prescriptions when appropriate. You will then be referred to a Delta Dental in-network dentist for diagnosis and further treatment. You can visit the TeleDentistry website to schedule your virtual visit or call the 24/7 hotline at 1-866-243-2014.
How to Schedule Your Virtual Dental Visit:
Getting an eye exam is not just about finding out if you need glasses. It’s about your health! An eye exam can detect eye-health problems like glaucoma or cataracts, and can also identify early signs of diseases that impact your whole body, such as high blood pressure and diabetes. Liberty Energy offers access to an affordable vision plan through Blue View Vision to protect the health of you and your family.

As a Liberty Energy employee, you automatically receive Basic Life and Accidental Death & Dismemberment (AD&D) coverage – at two times your annual earnings for natural death and four times your earnings for accidental death, to a maximum of $500,000. This coverage is offered through Prudential at no cost to you.
Life can be unpredictable, but you can prepare for whatever comes your way with benefits to protect the ones you love. Life Insurance, issued by Prudential, will help ensure your family is financially secure should anything happen to you
You can purchase additional Life and AD&D Coverage beyond what Liberty Energy provides through Prudential. They guarantee coverage during your initial enrollment period — which means you can’t be turned down for coverage based on medical history.
Liberty Energy provides both Long- and Short-Term Disability insurance through Prudential at no cost to you.
The plan benefit is 80% of your basic weekly earnings, based on a 40-hour week.
Eligible after 90 days of employment.
Short Term Disability duration = 12 weeks (7-day elimination period).
The plan benefit is 60% of basic monthly earnings up to a maximum of $10,000 per month. Basic earnings are the average of your gross monthly income for the year immediately prior to the onset of disability.
Long Term Disability max duration = to age 65
Eligible after using all 12 weeks of STD & still unable to return to work.

401(k) Retirement Plan
At Liberty Energy, we know planning for the future is important for the wellbeing of you and your family. An employee is eligible and automatically enrolled upon hire (see the link to the Benefit Guide for exceptions to this rule). Our plan offers a broad range of investment options from conservative to aggressive. You may make transfers among the various investment options daily, though some funds do have short-term trading restrictions or fees.
You may contribute up to the IRS limit - $22,500 in 2023. If you are over age 50, you may contribute an additional $7,500 per year. You may change your ongoing deferral/contribution percentage per the plan document. The Liberty Energy Plan allows you to make both before tax and after-tax Roth deferral contributions (combined up to the IRS limit - $22,500 in 2023).
Liberty Energy matches up to 6% for the pre-tax account. Any contribution changes to your 401k can be made directly through Principal.

Birth mothers may take up to ten weeks of paid medical leave in order to medically recuperate from birth.

Outside of your enrollment period, you may not make any changes to your plans without a Qualifying Life Event, which grants you a special enrollment period. Qualifying Life Events include:
Death of the employee, spouse, or eligible dependent
When you experience a QLE, you have 30 days to notify HR that you would like to make changes to your benefits.
Liberty Energy provides financial support to eligible employees who are adopting a child. This policy recognizes qualified adoption expenses (with receipt of payment) in the form of court costs, attorney’s fees, adoption agency, and placement fees, immunizations required for adoption, reasonable and necessary travel and lodging expenses while away from home (not to include meals), directly associated with the adoption.
Liberty Energy is here to support you and your family, with in vitro fertilization (IVF) coverage. We cover tests and services to find the cause of infertility as well as inpatient services, outpatients services and doctor office services for the diagnosis of infertility. We also cover services to treat the underlying medical conditions that cause involuntary infertility as well as artificial insemination, IVF and prescription drugs related to infertility. Qualifying employees have an unlimited maximum per member.
Make sure you are seeing an in-network provider and have prior authorization from Anthem Blue Cross Blue Shield before obtaining prescriptions, labs and procedures. Prior authorizations can be done by a clinic or a hospital. Remember, your deductible and coinsurance will still apply until you meet your out-of-pocket maximum. Treat this process like any other process traditionally covered under our insurance.
Employees who have gone through childbirth can take up to 12 weeks off, paid, through a combination of Liberty’s Childbearing Leave, Parental Leave, and Short Term Disability (STD).

How do I change my 401(k) contribution?
Participants may change their 401(k) contributions by logging into their Principal Account, select “Contributions” and then select “Manage my Contributions”.
How can I update my beneficiaries?
Retirement plan participants may be able to update their beneficiaries online. Select the retirement plan you want to update from the "Accounts" tab above, then once you are in the site for that account, find the "Beneficiaries" link. If you can't change your beneficiaries online, you may call us at 800-547-7754.
How do I get another FSA card?
If you are enrolled in the Flexible Spending Health Account, you can reach out to Rocky Mountain Reserve at 888-722-1223 or you can do so online at their website by registering with "RMRLIB" for the Employer Code and your social for your Employee Number.
Where can I download the Liberty Energy Mobile app?
