Birth of child:
You have 60 days following the delivery date to make the status change in the Benefits Passport system, but the earlier you are able to get that completed after delivery the better. To add your new dependent, go to iowacityschoolsbenefits.com. Enter your full district email address as your username and the password you created during the open enrollment period in May. If you need assistance with your password or benefit enrollment you may contact the Benefits Passport Help Center at 1-877-745-6302.
On your homepage, select Childbirth as your Status Change and select Go. Enter all of the information for your new child and select Continue to make your benefit elections. The event date for this qualifying event will be the date of birth. During the process, please upload the birth certificate and social security card. You may upload the birth certificate and social security information after you have submitted the change as well. Requests will not be approved until all necessary information is provided.
Birth of a child when both are district employees:

The event date for this qualifying event will be the date of birth. During the process, please include a copy of the birth certificate. You may submit the birth certificate and social security information to the payroll office at a later time if you do not have those when making the change to your benefits. Both you and your spouse will need to complete the status change process. One of you will make the change to elect family coverage enrolling all family members. The other will decline all coverage since they will be enrolled on the spouse's family plan. The district contribution for both employees will be applied to the family plan and the remaining premium amount will be equally divided between both employees.
Employee Lost Coverage or Dependent Lost Coverage:
You have 30 days from when you or your dependent’s coverage ends to make changes in the Benefits Passport System. To make a change to your benefit coverage, access the Benefits Passport system at iowacityschoolsbenefits.com. Enter your full district email address as your Username and the password you created during the open enrollment process in May. You may contact the Benefit Call Center at 1-877-745-6302 if you need assistance accessing your account.
Once you have accessed the system, you can select Employee Loses Coverage or Dependent Loses Coverage as your Status Change and select Go. Enter the last day of you or your dependent’s current coverage as the effective date (day before your ICCSD employee only or family plan would take effect) and attach the necessary supporting documentation. Make the necessary benefit coverage changes and confirm the elections. Once the effective date of coverage is verified from the supporting documentation (document from your dependent’s benefit provider or employer specifying the type of coverage held, who was covered, and when that coverage ends), the change request may be approved.
Employee or Dependent Gained Employment/Other Coverage:
You have 30 days from when your dependent’s coverage takes effect to make changes in the Benefits Passport System. To make a change to your benefit coverage, access the Benefits Passport system at iowacityschoolsbenefits.com. Enter your full district email address as your Username and the password you created during the open enrollment process in May. You may contact the Benefit Call Center at 1-877-745-6302 if you need assistance accessing your account.

Enter the last day of your current coverage as the effective date (day before your dependent’s coverage would take effect) and attach the necessary supporting documentation (you can also send the documentation to the payroll office through campus mail). Make the necessary benefit coverage changes and confirm the elections. Once the effective date of coverage is verified from the supporting documentation (document from your dependent’s benefit provider or employer specifying the type of coverage held, who is covered, and when that coverage begins), the change request may be approved.
