ABC Company is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. We offer two medical plans administerd though Loyal Healthcare to fit your budget and needs.
A Health Maintenance Organization (HMO) usually offers lower monthly premiums than other types of insurance plans. They also tend to have lower copays and coinsurance, which helps make them more affordable.
With this plan, you're required to choose a Primary Care Provider (PCP) and stay within a network of providers, urgent-care centers and hospitals to receive coverage under the plan. You must receive referrals from your PCP before you visit a specialist or are admitted to a hospital, except in a qualifying emergency. There is also no out-of-network coverage under an HMO.
Enrolling in an HMO can be a great option to help minimize your healthcare costs so long as you stay within the network.
Preferred Provider Organization plans (PPOs) provide more flexibiity when picking a doctor or hospital.
The PPO plan offers deductibles and requires copays when receiving treatment. If you're enrolled in the PPO plan, you can choose to contribute to a tax-advantaged healthcare flexible spending account to help pay for healthcare expenses not covered by your plan.
With the PPO medical plan, you have the choice of two benefit tiers: Plan providers (in network) and non-plan providers (out of network). The PPO option offers a larger network of providers and specialists to choose from. Unlike an HMO, you do not need to designate a Primary Care Provider (PCP) and you do not need to obtain a referral to see a specialist.

If you're enrolled in one of ABC Company's medical plans, you automatically have prescription drug coverage through Pharmacy Benefit Management. Your prescriptions are subject to your medical deductible and may be paid with available HSA dollars.

If you're enrolled in a consumer-driven High-Deductible Health Plan (HDHP with HSA), you're eligible to contribute on a pre-tax basis to an HSA. ABC Company contributes $1,500 annually in one lump sum on the first of the year. You can use these funds to cover medical, dental and vision expenses.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax free in retirement.
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to $2,850 in pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year. ABC Company partners with Peak Benefits for FSAs.
Dollars invested in an FSA are tax free and the entire amount is available on the first day of the plan year. However, any money not used by end of the plan year is forfeited.
An FSA covers eligible out-of-pocket healthcare expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent.

Employees will be 25% vested for each year of service. If you have four or more years of service, your employer match will be 100% vested.
