Neighborly offers two medical plans and prescription drug coverage through Blue Cross Blue Shield and Express Scripts (ESI). You have a choice between a High Deductible Health (HDHP) Plan and a Preferred Provider Organization (PPO) plan. With both plans, you will always pay less if you use a BCBSTX in-network provider or facility. Neighborly utilizes the BlueChoicePPO network. Plan rates are available in the Total Rewards Guide.
To support the overall health and wellness of our Associates by discouraging the use of tobacco products, if you are covered under a Neighborly medical plan and are currently using tobacco when you enroll, you will pay $40 per month more for medical coverage. If you engage with resources available on Smokefree.gov and submit proof (i.e. screen capture) to people.services@nbly.com by the 15th of any month, the reduction in monthly medical premium will be applied on the first payroll of the following month.

Preventive care is a critical component of maintaining health. Annual checkups and exams are advantageous in promoting health and wellness.
Preventive care services refer to checkups, health screenings, vaccines, and patient counseling that can help prevent illness, disease, and other health-related issues.
Recommendations can vary depending on the individual’s health status and risk factors. Talk to your doctor about a screening schedule that’s right for you!
If enrolled in a company medical plan, there is no charge for your annual Preventive Care visit. Contact BCBSTX directly at 1-800-521-2227 with questions regarding specific covered services.

The Geo-Blue Global Traveler plan provides accidental injury and illness coverage for you and your traveling spouse and unmarried, dependent children accompanying you, while on a business trip or business sojourn (leisure trip directly connected before, after or during a business trip) when outside your home country for up to 180 consecutive days. Coverage is available to all company employees who are traveling on company business and the plan is intended for emergency use.
BlueCard Worldwide is available to any member covered under the Neighborly Medical plans with BlueCross BlueShield of Texas who is traveling outside the United States. Dental health is important to your overall health. Neighborly offers a comprehensive dental plan through MetLife, which offers you an extensive network of dental providers. Remember, using an in-network PPO dentist can save you money. If you use an out-of-network dentist, you can be balanced billed by your provider.
Find a dentist, review your benefits, download or print your ID card and much more at www.metlife.com.
Our vision coverage is administered by MetLife. You can choose coverage for yourself as well as eligible dependents. You may go to any eye doctor you prefer, but you will save money when you use a participating MetLife provider.
To locate a participating provider, contact MetLife.
If you participate in the BCBS High Deductible Health Plan (HDHP), you can use a Health Savings Account (HSA), administered by Optum Bank, to help pay for qualified medical, dental, and vision expenses on a pre-tax basis.
Eligibility - To be eligible for an HSA, in addition to being enrolled in a HDHP, you also must not:
Advantages - An HSA is one of the best ways to save for future health care expenses. Money goes in tax-free, grows tax-free, and comes out tax-free. If you participate in a HDHP and enroll in an HSA, Neighborly will deposit money in your HSA ($1,000 yearly/$38.46 biweekly for Associate-only coverage, $2,000 yearly/$76.92 biweekly for Family coverage). Employer funding will be prorated and added to eligible accounts on biweekly schedule. You can also contribute pre-tax dollars to your HSA. You decide when to spend this money on eligible health care expenses, and it’s always yours — even if you leave the company.
How to Use the HSA - You can use your debit card provided by Optum Bank to pay for eligible health care expenses. You can also use online bill pay to pay a doctor or hospital bill or to reimburse yourself for an eligible health care expense that you pay for out of your pocket.
You can access your HSA by calling 800-243-5543 or by logging onto www.optumbank.com.
Dependent Care FSAs can be used to pay for expenses associated with caring for your dependents while you’re at work, like daycare centers, babysitters, summer day camp, and eldercare.
You can make pre-tax contributions of up to $5,000 per household. All funds must be submitted for reimbursement for the year in which they are contributed; funds do not rollover.

How it Works:
The money you set aside is excluded from your gross income. That means it is not subject to Federal Income Tax, Social Security, Medicare, and in most cases, state and local taxes. This lowers your taxable income and increases your spendable income. Depending on your tax situation, you could save 20-40% on expenses you would be paying anyway.
Life Insurance is often one of the cornerstones of financial planning. Should the unexpected happen, life insurance can help safeguard your family needs. Neighborly-paid basic life insurance and accidental death & dismemberment (AD&D) insurance is a combined benefit available at no cost for Associates. The coverage amount is equal to 1x benefits annual rate (commission) or total base pay offered at $25,000 to max of $300,000. Coverage reductions begin at age 70.
Sometimes you need additional protection to accomplish your financial goals. Neighborly is pleased to offer additional Supplemental Associate Life Insurance and Optional AD&D Insurance. These are voluntary programs:
Disability insurance provides income protection in the event of a disability. Neighborly offers company paid Short-Term Disability (STD) to all Associates, and you have the option to enroll in Voluntary Long-Term Disability (LTD) through MetLife as well.
Short-Term Disability Highlights:
Voluntary Long-Term Disability Highlights:
You can’t always avoid accidents ─ but you can protect yourself from accident-related costs that can strain your budget. Accident insurance pays a benefit directly to you if you have a covered non-work place injury and need treatment.
You can get coverage for your spouse and dependents, too. As medical costs continue to rise, accident insurance may provide a necessary layer of financial protection. The plan also has an annual cash benefit when you complete a covered wellness screening.
You can choose a lump sum benefit of up to $20,000 in increments of $5,000. Spouses can choose a lump sum benefit of up to $10,000 in increments of $2,500. Spouse and child coverage cannot exceed 50% of your coverage.

New Parent Benefits Overview
As a new parent, it's important to understand how your available leave options can work together to support you during this special time. Here's how your time off benefits are structured, starting from general Paid Time Off through to more specific parental leave options: Family and Medical Leave Act (FMLA)
Family and Medical Leave Act (FMLA)
FMLA provides up to 12 weeks of unpaid, job-protected leave and may be used for maternity recovery, caring for a new child, or bonding with an adopted or foster child. If you meet the eligibility criteria, FMLA will run concurrently with other applicable leave benefits, such as Short-Term Disability (STD) and Parental Bonding PTO.
Non-FMLA
Neighborly complies with all federal and state family and leave laws. However, when these laws do not apply or an associate does not meet the eligibility requirements, the Company will consider an associate’s request for medical leave. If eligible, associates may be granted unpaid time off if unable to work due to illness, injury or a disabling medical condition not covered under the scope of the Family and Medical Leave Act (FMLA), or other applicable state ordinance.
For more details, please review the policy.
Short-Term Disability (STD) Benefits for Maternity
For birth parents, STD provides paid leave during the initial recovery period following childbirth. STD runs concurrently with FMLA and includes:
6 weeks of 100% base salary for a vaginal delivery
8 weeks of 100% base salary for a Cesarean delivery
(A 1-week unpaid waiting period applies before STD benefits begin.)
After STD benefits are exhausted, birth parents can transition to additional leave options, including:
Parental Bonding PTO – 2 weeks of paid time off to bond with your child
Remaining accrued PTO – If you have unused PTO, you may apply it here
Unpaid leave – If all paid leave options have been used, FMLA may still provide job-protected unpaid time
Paid Time Off (PTO)
PTO can be used for a variety of reasons including personal matters, illness, travel, or caring for a family member. New parents may choose to use accrued PTO during their leave period in conjunction with other benefits.
For more information, see the PTO Policy
Parental Bonding PTO Overview
Following the initial recovery period or adoption/placement of a child, eligible associates may use Parental Bonding PTO. This benefit provides eligible associates with up to 80 hours (two weeks) of paid time off to care for or bond with a new child following birth, adoption, or placement. This time must be used within six months of the qualifying event and is separate from Short-Term Disability (STD).
Key Details
Paid at 100% of regular base pay
Must be used consecutively and in full; intermittent or partial use is not allowed
Available per event, not per child
Eligibility
To qualify, associates must:
Have completed at least six months of service within the past 12 months
Work in a state that does not offer state-paid family leave for bonding.
Associates in states with paid family leave should apply for those benefits instead.
How It Works
Submit a Leave of Absence Request Form and notify your manager; aim for at least 30 days' notice when possible
For birth parents, this PTO begins after STD benefits end
Unused hours do not roll over and are not paid out upon leaving the company
For more information, see the Parental Bonding PTO FAQs
