Benefit changes made during open enrollment will become effective July 1, 2022. As a new hire, your benefits will become effective on the 1st of the month following your hire date of full-time employment.
Open enrollment is from May 23 through June 6, 2022. This is an active enrollment. If you do not complete open enrollment, you will lose your benefits.
This is your opportunity to make changes to your current health benefit elections for the 2022 plan year. During open enrollment you may:
- Enroll or re-enroll
- Make changes to your benefits
- Add or remove dependents
- Enroll in health care flexible spending account(s)

Medical Plans
Centerline Communications is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. We are offering two medical plans, administered through Cigna, to fit your budget and needs.

PLANselect – our medical benefits decision support tool - can help! PLANselect is a web-based, video-guided tool developed by physicians and industry experts, providing users like you with a personalized analysis of the Green Thumb health plans.
PLANselect is totally confidential, anonymous, mobile responsive, and simple to use. It takes less than five minutes to see your results. You can try it multiple times, using different scenarios, to determine the right options for you. Click on the link and start your PLANselect modeling now!
HRA PPO Plan
The HRA (Health Reimbursement Account) is a PPO plan and, with this option, some of your costs may be more predictable, because you’ll pay copayments, or a flat dollar amount, each time you see your primary care provider or a specialist.
You can choose any in-network or out-of-network provider when you receive care, but will pay less for services when you use in-network providers. With the HRA you can utilize a Health Care FSA, but you are not eligible to enroll in a Health Savings Account (or HSA). This plan is preferable if you or your family members frequently visit the doctor or if you are expecting major medical expenses. Centerline will contribute to the first portion of your deductible with this plan.
HSA PPO Plan
On the other hand, the HSA PPO pairs with a Health Savings Account and this plan is designed to give you more control over your healthcare dollars, which could result in savings for you. With the HSA PPO, your per-paycheck cost is lower compared to HRA PPO, allowing you to keep more of your paycheck each month. HSA also uses the same network of providers as the HRA. This plan is favorable if you or your family members do not frequent the doctor and if you are not anticipating major medical expenses throughout the year. Centerline will contribute to your HSA account on a per-check basis.
MD Live
Both medical plans include virtual visits with MDLIVE visits, which provides 24-7-365 access to board-certified primary care doctors and pediatricians by secure video chat or phone. For an illness or injury that is not an emergency, MDLIVE’S telemedicine program offers a convenient, cost-effective alternative to hospital emergency rooms and urgent care clinics.
MDLIVE is not intended to replace your relationship with your doctor, but rather provides access to healthcare when reaching the doctor is difficult or inconvenient.

Download the MDLIVE mobile app, call MDLIVE at 888-676-4204 or go to MDLIVE.com. Enter your first and last name, date of birth and Cigna Member ID number.
Register and complete your account profile, including a brief medical history, for you and your enrolled family members. Video chat or talk with a doctor from home, work or when traveling.
Prescription Plan
If you are enrolled in one of Centerline Communications' medical plans, you automatically have prescription drug coverage through Pharmacy Benefit Management. Your prescriptions are subject to your medical deductible and may be paid with available HSA or FSA dollars.
Home delivery is also provided by PBM. Certain controlled substances and prescriptions that have expired will require a new prescription from a doctor.

Dental Insurance
Maintaining good oral hygiene is paramount to your overall health and wellness. That is why Centerline Communications has partnered with Cigna, one of the largest carriers, to provide our employees and their families with excellent dental coverage.
Regular dental care is an important part of caring for your overall health, and keeping your teeth fit may protect you from major expenses in the future. Use dental coverage through Cigna to help pay for the cost of routine checkups and just about any other type of dental work you might need, such as crowns or root canals.
If you have an HSA or an FSA, you can use these funds to cover eligible dental costs like your deductible or copay.
Vision Insurance
Regular eye examinations can not only determine your need for corrective eyewear but also may detect general health problems in their earliest stages. It can help identify early signs of diseases that impact your whole body such as high blood pressure, diabetes, and high cholesterol.
Whether you have perfect vision or require some type of corrective lenses, preventive eye care is an important part of your overall health. VSP can help you offset the expensive costs of exams, frames, contact lenses, corrective surgery, and more.
If you have an HSA or an FSA, you can use these funds to cover eligible vision-related costs like your deductible, glasses or contact lenses.

Health Savings Account
A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you're enrolled in a HSA PPO medical plan, you're eligible to contribute on a pre-tax basis to an HSA. Centerline Communications contributes $500 toward individual coverage and $1,000 toward family coverage throughout the year. You can use these funds to cover medical, dental and vision expenses.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax-free in retirement.
HSA is a triple tax benefit. Money goes into the HSA account tax free, grows tax free if invested, and can be withdrawn tax free for qualified medical expenses.
Flexible Saving Account
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to $2,850 of pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year. Centerline Communications partners with HSA Bank for FSAs.
Dollars invested in an FSA are tax-free and the entire amount is available on the first day of the plan year (January 1, 2023). However, any money not used by end of the plan year is forfeited.
An FSA covers eligible out-of-pocket health care expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent.

Life Insurance
Life Insurance coverage is a critical part of planning for your family’s current and future financial needs. Proceeds from life insurance can help with salary replacement, mortgage protection, cost of childcare, debt repayment and children’s education expenses.
Centerline Communications provides Basic Life insurance coverage up to 1x your salary. This coverage includes an Accidental Death and Dismemberment (AD&D) provision in the same amount in the event of accidental death and certain other conditions. Basic Life and AD&D insurance is administered by The Hartford and is paid for by Centerline Communications. You are automatically enrolled in these benefits.
Voluntary Life Insurance
As a new hire, you can purchase Voluntary Life insurance for you, your legal spouse or domestic partner, and dependent children without providing medical information up to certain guaranteed issue (GI) amounts. If you leave the Company, this coverage can be taken with you.
Employee and spouse amounts applied for over the GI as a new hire will require you to provide Evidence of Insurability (EOI) for review and approval by The Hartford.
If you elect not to enroll within 30 days of your date of hire, you will still be able to purchase coverage in the future. However, ALL amounts elected will be subject to the EOI requirements provision. At that time, if your EOI is not satisfactory to The Hartford, you will not have Voluntary Life coverage.
Please refer to the 2022 Benefit Guide for details regarding the Guaranteed Issue (GI) amounts, and Evidence of Insurability (EOI) Requirements.
To enroll in Voluntary Spouse/Domestic Partner and/or Child Life, you must have elected at least the same amount of Voluntary Life insurance for yourself.
Short-Term Disability & Long-Term Disability Insurance
Could you and your family survive without a paycheck, if you were out of work due to an injury or illness? Disability insurance is essentially “Paycheck” insurance, ensuring you will receive a portion of your income if you are unable to work due to a non-work-related injury or illness. Short-Term Disability (STD) provides a weekly benefit, while Long-Term Disability (LTD) pays a monthly benefits after STD insurance has been exhausted.
Centerline Communications is proud to offer both STD and LTD insurance through The Hartford, at no cost to you. You are automatically enrolled for STD and LTD coverage. You are taxed on the value of this coverage, which provides you the advantage of receiving tax-free STD and LTD benefits, should you file a claim.
Supplemental Medical Coverage
Centerline Communications offers three supplemental health plans for you to consider. These are intended to supplement your medical plan elections, by paying cash directly to you for qualifying health conditions. These benefits may help you fund costs associated with your medical plan deductible or coinsurance requirements.
Accident
Accidental Insurance pays a cash benefit when you or your covered family members suffer injuries sustained in a covered accident.
Critical Illness
Critical Illness Insurance pays a lump-sum cash benefit following the diagnosis of a covered critical illness or event. Learn more about Critical Illness in our video.
Hospital Indemnity
Hospital Indemnity Insurance pays a lump-sum cash benefit following a hospital stay or event. Learn more about Hospital Indemnity in our video.
LifeLock Identity Theft Protection
Identity theft is when thieves steal your personal information in order to take over or open new accounts, file fake tax returns, rent or buy properties or commit other crimes in your name.
Centerline Communications offers ID Theft Protection through LifeLock. This coverage can help you avoid identity theft and, in the worst-case scenario, get your life back after a breach of your secure personal information.

Employee Assistance Program
Centerline Communications provides a confidential Employee Assistance Program administered by The Hartford. This program is available at no cost to you and all members of your household. This includes dependent children up to age 26 whether or not they live at home. Services are confidential 24 hours a day, 7 days a week. Access educational resources and services, including:
- Daily life assistance
- Emotional wellbeing support
- Up to six counseling sessions per issue each per year
- Financial services
- Legal services - free 30-minute consultation with a participating attorney
- Online resources - articles, assessments, video resources, fitness discounts, and more
