Scheduling Your Consultation
Use the online scheduling tool or call to schedule
You can schedule a consultation by using our online scheduling tool. If you prefer to call, please refer to the toll-free phone number on the Contact Us page.
Whether you use the online tool or schedule by phone, you will be asked a few questions that will help us identify the type of consultation appointment you need. Answering the questions will also help the Aptia benefits counselor prepare for your consultation.
You should plan for your scheduling call to last 5-10 minutes.
Review your Enrollment Guide prior to the call so you are familiar with the consultation process.
Be ready on the date and time you selected for your consultation, and...
-
Review your Enrollment Guide in advance of the call.
-
Have your current plan information available.
-
Know what prescription drug medications you are taking (name, dosage, frequency). Refer to the Healthcare Provider & Prescription Drug Information section below for more information on compiling your prescription drug list.
-
Know what providers and facilities you use, including the address.
-
Allow 60 minutes for your initial consultation and, if you are Medicare-eligible, ensure you have your Medicare (red, white, and blue) ID card available for your consultation. You will need to provide both your Part A and Part B effective dates.
-
Make certain any family member or caregiver is available for the call if desired.
-
Wait for your benefits counselor to call you.
-
Promptly reschedule the consultation if you find you cannot keep the appointment you originally scheduled by calling the toll-free number listed under the Contact Us tab at the top of this page.
Healthcare Provider & Prescription Drug Information
Start by keeping track of your individual healthcare coverage needs. Providing correct and complete information about your healthcare providers and prescription drugs will help your benefits counselor analyze your coverage needs.
If you are Medicare-eligible or soon will be, you can use our online tool to create a medication list that will be needed to review prescription plan options.
You will need to log into our Plan Compare portal to complete this process and will need a PIN provided by Aptia365. Follow the directions to View Plans on the Learn About Plan Options (Shop & Compare) page on this site.
Once you log in, follow directions to enter your Prescription and Pharmacy preferences to update your profile. You can use the tool to see what prescription drugs are covered by the various plans.
You can use this form to collect the provider and prescription drug information you will need to provide when you are reviewing plan options. Once you've documented your lists, you can use the online tools to upload this information to your profile.
Help From a Caregiver
Do you have a son, daughter, other family member or caregiver who would like to help you with your health coverage selections?
We welcome their participation throughout the entire process, with your approval. You have the power to choose who, how long and what personal information can be discussed. We provide a Personal Information Authorization Form that gives us permission to speak with one or several people of your choice. Simply download, complete, and return the Personal Information Authorization Form to retiree.exchange@aptia-group.com.
Submitting this document authorizes us to provide assistance (for up to three years) without you physically being part of any conversation with a benefits counselor. We can discuss information about your insurance eligibility, plan enrollment, premiums and payment information, and personal health history or status. However, submitting this document does not allow us to enroll you in any health insurance plan without the appropriate Power of Attorney documentation on file. If you need to arrange for a Power of Attorney, please make your benefits counselor aware so they can provide the necessary information to you.
Personal Information Authorization Form
When you complete and email the form below, your caregiver will be able to assist you with many, but not all, of your health care insurance needs.
Download and complete the authorization form below. You will need to save a copy of it to your computer before you email it to us; your information will not be automatically saved, so if you close the form before saving your information, it will be lost and you will need to start again.
Learn About Plan Options
Learn more about the benefits, process of enrolling and selecting the plans for you and your family.
Medicare Plans
If you want coverage other than Original Medicare (Parts A and B), in most cases you will choose either a Medicare Supplement and a Medicare Advantage plan. Use this chart to learn more about these plan options.
When you enroll in Original Medicare (Parts A and/or B) with or without a Medicare Supplement plan, you will need to enroll in a Part D Prescription Drug Plan to have coverage for outpatient prescription drugs you get at a pharmacy or through mail order. Most Medicare Advantage plans include prescription drug coverage (Part D).
You can use our plan comparison tool to review Medicare Advantage, Medicare Supplement and Part D Prescription Drug plans available in your area. Use the tool to enter your preferences, as well as the providers and prescriptions you use, to find plans that align with your health needs and budget.
IMPORTANT INFORMATION FOR 2027 PRESCRIPTION DRUG PLAN REVIEW:
While our plan comparison tool can help you explore Medicare coverage options, some standalone Medicare Part D Prescription Drug Plans may not be displayed. If your primary goal is to enroll in a standalone Part D prescription drug plan, we recommend also reviewing available plans through Medicare's Plan Finder on Medicare.gov for a complete view of your options.
If you have a PIN* provided by Aptia365

3. Schedule a consultation if you would like to learn more
Have additional questions about pre-Medicare plans and what you need to consider as you weigh your options? You can schedule a consultation with an Aptia365 benefits counselor.

This website contains a wide range of information on many health topics including: Alzheimer’s disease, Arthritis, Balance Problems, Breast Cancer, Colorectal Cancer, Diabetes, Exercise for Older Adults, Hearing Loss, Lung Cancer, Prostate Cancer, Taking Medicines, and many more.
Medicare FAQs
Usually, no. Most nursing home care is custodial care (help with bathing, dressing, using a bathroom, and eating.) This care is not covered by Medicare. Medicare Part A only covers skilled care given in a certified skilled nursing facility. You must meet certain conditions and coverage is limited. For more information about Medicare skilled nursing care, please see “Medicare Coverage of Skilled Nursing Facility Care” on the Medicare website by clicking here.
If you have limited income and resources, Medicaid may help cover nursing home costs. For more information about Medicaid, call your State medical assistance office.
Medicare does not cover routine dental care or most dental procedures such as cleanings, fillings, tooth extractions or dentures. In rare cases, Medicare Part B will pay for certain dental services. In addition, Medicare Part A will pay for certain dental services that you get when you are in the hospital. Some Medicare Advantage plans may include dental benefits.
You can file for Medicare online using the Social Security website when you file for retirement or disability. You also can file for just Medicare if you do not wish to start receiving your retirement benefits.
If you do not wish to apply online you can make an appointment by calling 1-800-772-1213. People who are deaf or hard of hearing may call the Medicare “TTY” number, 1-800-325-0778, between 7 a.m. and 7 p.m. on business days.
If you are already receiving Social Security benefits, you will automatically be enrolled in Medicare Parts A and B. However, because you must pay a premium for Part B coverage, you have the option of turning it down. You will be contacted by mail a few months before you become eligible and given all the information you need. (Note: Residents of Puerto Rico or foreign countries will not receive Part B automatically. They must elect this benefit.)
Some people don’t need Medicare Part B because they are still covered by an employer group plan or their spouse’s health plan. However, if you do not join Part B right away and you are not covered under another health care plan, the Part B premium will increase 10 percent each year after you were first eligible to purchase it. Call 1-800-MEDICARE (1-800-633-4227) for more information, 24 hours a day, 7 days a week. TTY/TDD users should call 1-877-486-2048.
Most people sign up for and buy Medicare Part B. If you want to join a Medicare Supplement or Medicare Advantage plan, you will need to have both Medicare Parts A and B. Call the Social Security Administration at 1-800-772-1213 or visit their website for more information or to enroll.
If you keep working after you turn 65, you may sign up for Medicare Part A as long as you are not contributing to a Health Savings Account (HSA). (If you continue to contribute to an HSA after reaching Medicare eligibility, you should stop contributing six months prior to enrolling in Medicare Part A.)
If you have health coverage through your employer or union, Part A may still help pay some of the costs not covered by your group health plan. Call the Social Security Administration at 1-800-772-1213 to sign up. However, you may want to wait to sign up for Medicare Part B if you or your spouse are working and have group health coverage through you or your spouse’s employer or union. (See note below if you work for a small company.) You would have to pay the monthly Medicare Part B premium, and the Medicare Part B benefits may be of limited value to you as long as the group health plan is the primary payer of your medical bills. In addition, you would start your 6-month Medigap open enrollment period during a time when it will not be of most use to you. For more information on your Medigap open enrollment period, please click here.
Note: If you are age 65 or older and working for a small company (less than 20 employees), you should talk to your employee health benefits administrator before making any decision not to take Medicare Part B. If your employer has less than 20 employees, Medicare is the primary payer and your group health insurance would be the secondary payer. To learn more, please click here.
If you are disabled and working (or you have coverage from a working family member), the Special Enrollment Period rules also apply. Call the Social Security Administration at 1-800-772-1213 or visit their website for more information.
There are many factors to consider before choosing Medicare coverage, such as:
-
Cost — What will you pay out-of-pocket, including premiums?
-
Doctor and hospital choice — Can you see the doctor(s) you want to see? Do you need a referral to see a specialist? Can you go to the hospital you want?
-
Convenience — Where are the doctor’s offices? What are their hours? Is there paperwork? Are they accepting new patients? Do you spend part of each year in another state?
-
Prescription drugs — Are they covered? Are your prescription drugs on the plan’s list of covered drugs (formulary)?
-
Pharmacy choice — Can you use the pharmacy you want? Are the pharmacies convenient?
-
Quality of care — How is the quality of the plans in your area? Information about quality is available at on the Medicare.gov.
-
Benefits — Are extra benefits and services, like additional drug coverage, eye exams or hearing aids covered? (These may be covered by some plans.)
