The Blue Cross PPO Plan ($150 Deductible) features competitive benefits and allows access to 95%+ of doctors and all hospitals without risk of balance billing. Under this plan, you are responsible for the first $150 in medical costs each year (does not apply for office visit copays). See the attached plan description documents for more details about the plan. Associates pay approximately 20% of the cost of the plan via pre-tax payroll deductions.

The Blue Cross Savers Plan ($1650 Deductible) features benefits that allow access to 95%+ of doctors and all hospitals without risk of balance billing. Under this plan, you are responsible for the first $1500 in medical costs each year. Once you have spent $1650, the plan will then cover 80% of costs until you spend the out-of-pocket maximum of $4,000.
Participation in the $1500 Deductible Plan also enables participants to contribute to a Health Savings Account that can be used to pay for uncovered medical and dental expenses and can be carried over from year to year. New Balance will contribute $500 to an individual Health Savings Account, $750 to an Ind+1 HSA and $1,000 to a family HSA.
In addition, associates are able to contribute to their HSA with pre-tax dollars to a cap of $3,650 for individuals and $7,300 for families. See the attached plan description documents for more details about the plan. Associates pay for about 20% of the cost of the plan via pre-tax payroll deductions.

New Balance Zero-Deductible medical plan option. Administered by GPA, this new plan has the same benefits as the current medical plan but with no deductibles and a lower cost per paycheck. This option was built to deliver exceptional member support with just one phone number to call. It offers built-in price protection to make sure you don’t overpay for care by reviewing provider claims to make sure they don’t exceed your plan’s allowable limits. Occasionally a provider may not accept the plan’s payment. If this happens, the plan will work to get it resolved. Simply notify GPA right away. Note that the plan uses Multi-Plan for a provider network, so associates should check if their doctors participate.

The New Balance Total Fit Medical Plan compensates associates who receive their medical benefits from a source outside of New Balance by reimbursing them for any incremental payroll deductions through a spouse's employer, all copays, deductibles, and coinsurance, plus pays $50 per month. See the attached plan description documents for more details about the plan. The Total Fit Plan is free for all eligible associates who enroll.
Note that participation in the Total Fit plan requires participants to provide documentation around their enrollment in another group medical plan and its cost. Enrollment will not occur until such documentation is completed. Once completed, benefits will be provided retroactive to initial eligibility date.

The New Balance Express Scripts RX Plan covers prescription drug costs for those who enrolled in one of the New Balance Medical Plans. You are automatically enrolled in the plan if you are enrolled in either the Zero, $150, or $1500 deductible plans. See the attached plan description documents for more details about the plan. The cost of the Express Scripts Plan is included in medical plan costs. Maintenance drugs must be filled either through Express Scripts mail order program or in person at CVS. You will receive a 90 day supply, but only pay for 60 days.

The New Balance NB Meds Plan allows participants in the New Balance medical plans (Zero, $500 and $1500), to obtain certain name brand maintenance medications from Canada at no cost. See the attached plan description document for more details about the plan. The cost of the NB Meds Plan is included in medical plan costs. There are no additional costs for the NB Meds Plan.

The New Balance Delta Dental Plan provides participants with coverage for their dental care expenses, including 100% coverage for cleanings and x rays, 80% coverage for fillings and root canals, and 50% coverage for caps and crowns. The New Balance Vision Service Plan provides participants with coverage for their vision care expenses. There are 2 levels from which to choose from, Standard and Premier. Safety Glasses Program: Associates who work in manufacturing, or a support area and are required to wear Safety Glasses are eligible for free prescription safety eye glasses. Associates are eligible for free frames once every two years and lenses annually. To receive your safety glasses, simply make an appointment with a participating eye care specialist within the VSP network.
The New Balance Medical Flexible Spending Account (FSA) Plan provides participants with the ability to lower their taxes by setting aside funds in an Medical FSA via a pre tax deduction from their paychecks. The funds can then be used for medical expenses that are not covered by insurance. Participants should be careful about how much money they deposit into a medical FSA as funds that are not used may be forfeited.

The New Balance Teladoc Plan provides participants with the ability to access a doctor virtually 24 hours a day, 365 days a year. New Balance offers eligible associates the choice of 2 virtual physical therapy programs. Hinge Health allows participants to use their personal devices to receive physical therapy services from the convenience of your home. XR Health uses a virtual reality headset to enable you to receive physical therapy services from the comfort of your home. See the attached plan description document for more details about the plan. New Balance recognizes the need for associates to take time off from work for such reasons as personal or family illness, doctor appointments as well as rest and relaxation.
NB Holidays are available for Regular, active, Full-Time (30+ hours per week) associates and may vary by location and certain positions.
Earned Time is offered to Regular, Full-Time associates (30+ hours per week).
Sick Time is for Part-Time and Temporary Associates only. Associates will earn sick time after 90 days. of service.
Leaves of Absence may be available to associates for a variety of reasons such as personal medical, caring for a family member, baby bonding, Military service, Jury Duty etc. Eligibility requirements can vary by country, state, and city. New Balance's Primary Caregiver Leave provides 16 weeks of full pay for full time (30+ hours per week), Regular Associates with 30 days of service; Secondary Caregiver Leave provides 4 weeks of full pay for full time (30+ hours per week) regular associates with 30 days of service. Regular associates who work 20-29 hours per week with 6 months of service may be eligible for benefits under Short Time Disability.

ET is designed to provide eligible associates with ample paid time off for leisure, personal activities, short term illness, and other planned and unplanned absences. This policy provides paid time off based on an associate’s length of service. Earned Time is logged in Workday in the Absence Worklet.

For Part-Time and Temporary Associates ONLY. Part-Time and Temporary Associates will earn sick time after 90 days.
Sick time will not carry over.
New Balance offers “Work from Anywhere” weeks, where eligible associates may elect to work from anywhere, up to four weeks per year (pro-rated based on hire/transfer date; 1 week/quarter). During Work from Anywhere weeks, associates must work their standard schedule, but can do so remotely from any location, and will not be required to be in person at any New Balance office.
Work from anywhere weeks must be taken in week-long increments (Monday through Friday) and cannot be broken into individual days. Note that some jobs are ineligible for “Work From Anywhere” due to the nature of the work.
New Balance provides leaves of absence to eligible associates, including leaves under the Family and Medical Leave Act. Please see the policies below for specific leaves of absence. Reach out to your local benefits contact when applying for a leave.

Flexible Spending Accounts provide regular, full-time associates who work at least 30 hours per week, with 30 days of service, the ability to save money pre-tax to cover health and/or dependent care expenses. Associate would enroll when newly hired, and can change elections during open enrollment or within 30 days of a qualifying event.
Note: enrollment is required during each year’s Open Enrollment period in order to participate in the coming year (previous elections do not carry over year to year automatically).
The Dependent Care Spending Account allows a maximum savings of $5,000 per year to pay for dependent care expenses.
