As a new teammate to TB12 Inc you're eligible to participate in open enrollment. This is your opportunity to elect the benefits most suitable for you and family (if applicable). Elections for all benefits but commuter and 401(k) will become effective on your hire date and be in place through December 31, 2022, unless you qualify for a life event.

We offer you two robust medical plan options through CIGNA. These plans allow you the flexibility to choose which plan coverage is right for you, and/or your family. The HMO (Health Maintenance Organization) provides in-network coverage and requires the direction of your Primary Care Physician to seek other medical care. The PPO (Preferred Provider Organization) allows you to direct who you see for medical services and provides coverage for out-of-network services. There are cost differences with each and you're encouraged to fully explore the plan details in your benefits guide. Medical ID card(s) should be received in the mail within 14 days from open enrollment. Should you lose your card or never received one, please reach out to order one from CIGNA here.
PLANselect is totally confidential, anonymous, mobile responsive, and simple to use. It takes less than five minutes to see results. You can try it multiple times, using different scenarios, to determine the right options for you.
Prescription benefits are provided under each health plan offered through CIGNA. They provide tiered prescription drug coverage at varying copay amounts.
Generally, you'll save money if you're able to purchase your prescriptions in a 90-day supply via mail order and, when possible, selecting generic drugs.

A Health Reimbursement Account has been set up to integrate automatically with your CIGNA medical plan. TB12 will pay directly to CIGNA costs associated to meeting your deductible automatically. The last 50% of the medical deductible will be reimbursed. Cigna will process claims automatically and reimburse providers directly on your behalf up to the plan maximums, so you won't incur charges once you've met 50% of your deductible. Members pay the first half of the calendar year deductible and TB12 will pay the second half, limiting your out-of-pocket expenses.
Maintaining good oral hygiene is paramount to your overall health and wellness. That's why we've partnered with CIGNA to provide you, at no cost, great dental coverage benefits that just get better over time.
Getting an eye exam isn't just about finding out if you need glasses. It's about your health! An eye exam can detect eye-health problems like glaucoma or cataracts. However, it can also help identify early signs of diseases that impact your whole body such as high blood pressure, diabetes, and high cholesterol. That's why we offer this affordable voluntary vision plan through EyeMed. Beyond the basic coverage of this plan, those who enroll are also granted several other vision discounts through EyeMed.
Flexible Spending Accounts are administered by HRC Total Solutions. You have two FSA options as a smart and convenient way to stretch your benefit dollars and receive real tax savings: a Health Care FSA and a Dependent Care FSA. Both allow you to contribute pre-tax dollars through payroll deductions. You're then reimbursed tax free through the account for eligible expenses either through claims submitted or your FSA debit card for eligible expenses.
Note that if you want to participate in an FSA, IRS rules require that you must actively enroll each year.
A Medical Care Reimbursement Account (Health Care FSA) is for all medically necessary physician-prescribed medical expenses that are not covered by insurance and that are eligible under Section 213 of the IRS Code (i.e., deductibles, co- pays, prescription co-pays, dental services and vision services). Over-the-counter medical expenses may only be reimbursed if prescribed by a physician. Be sure to review the list below of FSA Qualified Medical Expenses prior to electing an amount to contribute for 2022.

A Dependent Care Reimbursement Account is for the cost of caring for eligible dependents while you work (i.e., child care expenses, daycare, after-school care, preschool, summer camp, caregivers for a disabled spouse or dependent who lives in your home).

Transit and Parking
Save Time and Money on Your Daily Commute!
Enroll in the pre-tax accounts, which is administered by HRC Total Solutions. You decide how much money to deduct each month to cover your monthly transit and parking costs, not to exceed current IRS limit of $280. The funds will be available to spend once deposited.
Elections may not exceed $280 per month for either transit or parking and will be deducted in only two pay periods per month. In months where a third pay period exists, no deduction will be made.
Elections made to either transit or parking are NOT interchangeable. Amounts contributed to each may only be used for the elected account.
Transit – In order for the expense to be eligible, it needs to be incurred on a mass-transit system such as commuter rail, bus transportation, Uber Pool, Lyft Line or another public/private commuter system.
Parking – In order for the expense to be eligible, it needs to be for a garage you park at near your work or where you park to access a public-transportation system.

We've enhanced our time-off policies for 2022 and created a robust and flexible benefit offering for all full-time employees.
We've increased number of holidays to 10 and added flexibility with the addition of increased floater days to support scheduling flexibility and allow you the opportunity to take the time off for what's important to you!
The corporate team can carry over three days (24 hours) into 2022 to be used in January before it will expire to accommodate vacation scheduling through the busy season.
In recognition of giving and giving back to our communities we serve, we've added a Volunteer Day. This is a day you can take out of the office to volunteer and give back.
Short-Term Disability (STD) Insurance — When you're unable to work due to a sickness or injury, this coverage can help replace a portion of your income temporarily if you become too sick or injured to work.
Long-Term Disability (LTD) Insurance — When you're unable to work due to a sickness or injury, this coverage can help replace a portion of your income if you become too sick or injured to work for a longer period of time.
TB12 offers disability insurance for Massachusetts residence through the state's Paid Family Medical Leave program and all through UNUM.
Please reach out to HR should you need to request a leave so we can review your options.
Effective in January 2022, eligibility to enroll in the TB12 401(k) will be after one month of employment.
There will then be a one-month enrollment period where you can make changes to the auto-enrollment of 3%. If you do nothing, you'll be auto-enrolled, contributing 3% of your salary once eligible to enter the plan.
Contribution election changes always need to be made directly with VOYA. They cannot be made in iSolved and HR is not able to make these changes for you per regulations. Once the one-month enrollment period ends, you'll be joined to the plan in the first pay period following the first of the month following the end of the enrollment period.
You're encouraged to set up your account through VOYA and leverage their app for ease of retirement tracking. Please take some time to review VOYA's on-demand learning sessions to educate yourself on retirement. They have many different areas to explore, like the differences between a Roth versus Traditional retirements saving plans, understanding loans and withdrawals, nearing retirement and a focus on financial wellness.
