CSAA IG offers two medical plan options administered by UnitedHealthcare (UHC) and one Kaiser DHMO option (California employees only). These plans are designed to offer you choice with excellent care at a variety of costs. All three medical plan options include prescription coverage and 100% coverage for preventive care.
Review the Medical Plan Comparison Chart for plan details and rates to select the best plan for you and your family. Also review how to search the UHC network and select a Primary Care Physician (PCP).
This plan only covers benefits in the UnitedHealthcare network and there is no out-of-network coverage. The plan has lower deductibles. Office and therapy visits, basic labs, and prescription drugs are covered at a copay (without needing to meet an annual deductible). To receive the highest benefit coverage, you must have a Primary Care Provider (PCP) and a referral to see a “Tier 1” provider. You will be assigned a PCP if no selection is made, and you may change your PCP monthly. Coverage is provided if you use other network providers (not in Tier 1) (with or without a PCP referral); however, it will cost you more.
This plan is a high deductible plan with a tiered benefit structure, which means you must visit a “Tier 1” provider to receive benefits at the highest level. All other in-network care provided will be paid at the Tier 2 benefit level. This plan also allows for care received outside of the network; however, this will come at a lower benefit level. A PCP and referrals are not required however it is recommended, and you will be assigned a PCP if no selection is made. You may change your PCP monthly.
You may be eligible to enroll in a Health Savings Account (HSA) to receive company base contributions and well-being incentives for completing activities. You can also make your own contributions to the HSA.

Kaiser is a Health Maintenance Organization (HMO) that focuses on preventive care within one network – the Kaiser network. The Kaiser DHMO Plan includes a deductible for some services. Other services such as primary care, specialist, mental health office visits and prescription drugs are based on a small copay with no deductible.
You will be enrolled in a Health Reimbursement Arrangement (HRA) and the company will deposit money into your HRA for completing well-being activities.

CSAA IG offers the choice between two dental Preferred Provider Options (PPO) options to meet your needs:
Both dental PPO options cover preventive care, which includes two cleanings per year covered at 100%. Orthodontia is also covered for children and adults, with different lifetime benefit maximums.
You can choose any dentist for your dental care, but you will save the most when you use a dentist in the Delta Dental PPO Network. Delta Dental PPO Network dentists charge reduced fees for covered services, so you’ll pay less out-of-pocket expenses for dental care. If you use a non-Delta Dental PPO Network dentist, you’ll pay your typical share for the dental care, in addition to any amount the provider charges above what is reasonable and customary for the given service.

The vision plan, administered by Vision Service Plan (VSP), helps monitor the health of your eyes and minimizes the cost of eyeglasses or contact lenses. Exams, frames, lenses and contact lenses are allowed every 12 months. To maximize your vision benefits, you are encouraged to use a VSP provider. Member discounts are available on contact lens supplies, Lasik surgery, and other services. The VSP Signature network includes numerous private and retail providers including Sam’s Club, Costco and Walmart.
To locate a provider near you, visit VSP.com. To search for a Signature network provider, click on “Advanced Search” button under “Find a Doctor” to Select “Signature” under “Doctor Network.
Vision care practices that participate in the Premier Program as designated on vsp.com offer the latest eye care technology and services through the use of high-tech equipment, access to bonus offers, and more.

Health Care Reimbursement Account (HCRA) is for employees enrolled in the Kaiser DHMO Plan, UnitedHealthcare In-Network Plan, waived the CSAA IG medical plan, or employees who are not eligible for the HSA. You are eligible to set aside your own money on a pre-tax basis in the HCRA for eligible medical, dental, vision, and prescription drug expenses. This plan is considered a use-it-or-lose-it account.
The Reimbursement Accounts are administered by Optum Financial and the Health Savings Account is administered by Optum Bank, a subsidiary of Optum Financial. For all accounts log on to www.Optumbank.com to manage your accounts.
CSAA IG offers two non-healthcare tax savings accounts to help you save on dependent care and commute expenses. By setting aside pre-tax money from your paycheck in a special account for these expenses, you save on federal, state, and local income taxes.
Dependent Care Reimbursement Account (DCRA) - The DCRA allows you to set aside money up to the IRS allowed amount on a pre-tax basis to cover dependent care expenses, such as day care or afternoon programs.
Transit Commuter Account Program (TCAP) - The TCAP allows you to set aside pre-tax dollars up to the allowed amount to pay for eligible parking and transportation expenses. Enrollment in the TCAP is allowed between the first and 20th of the month for the following benefit month. New hires are eligible the first of the month after their hire date.
The Reimbursement Accounts are administered by Optum Financial. The Optum Bank (subsidiary of Optum Financial) Mastercard can be used for both tax savings accounts.

At no cost to you, CSAA IG provides Basic Employee Life Insurance coverage at two times annual salary up to a maximum of $1,000,000. Coverage is automatic - you are not required to enroll.
For additional protection, you may elect to purchase Supplemental Life Insurance coverage for yourself, your spouse or domestic partner, or your children.
You may also purchase AD&D coverage for yourself and your family up to a maximum of $750,000.
Ensure your life insurance benefits are paid how you want by designating your beneficiary or beneficiaries when you enroll or make changes as needed. View the instructions below for updating your beneficiary.

If you are no longer eligible for coverage as an active employee, you can convert or port your group life insurance to individual life coverage. You’ll need to submit your completed conversion or portability form to Securian within 30 days of termination. You're responsible for payment of insurance premiums. The forms are available in the Benefits Guide for Employees Leaving the Company located on Vista.
Short-Term Disability (STD) coverage pays you a benefit if you are temporarily unable to work due to injury, illness or pregnancy. CSAA IG provides STD coverage to all eligible employees - you do not need to elect coverage. After seven days of disability, you may be eligible to receive STD benefits replacing a percentage of your weekly salary for up to 26 weeks, reduced by other sources of disability income.
If your disability continues beyond 180 consecutive days and you meet the plan's definition of disability, Long-Term Disability (LTD) provides a monthly income replacement of 60% of your monthly salary up to $20,000. CSAA IG provides all eligible employees with LTD insurance from New York Life (formerly Cigna) - you do not need to elect coverage.

The Critical Illness and Accident Protection Plans offer financial protection for you and your family to help give you peace of mind when the unexpected happens. These voluntary benefits are employee paid and offered through payroll deduction.
The voluntary UHC critical illness coverage provides a lump-sum cash benefit to help cover direct and indirect costs associated with a qualifying serious illness (pre-existing conditions may apply). Critical illness insurance coverage can help pay for expenses such as rent, mortgage or other daily living expenses, as well as health-related expenses not covered by major medical insurance. You can purchase coverage for yourself, and for your dependents – provided you purchase coverage for yourself first.
The UHC Accident Insurance Protection Plan can pay for expenses resulting from a covered accident or accidental death and dismemberment not covered by your medical plan. This benefit can complement your medical and disability insurance to help you pay for unexpected or common costs such as medical deductible, coinsurance, emergency room visits, physician office visits and hospital charges. This coverage is available to purchase for yourself and dependents.
