Benefits annual enrollment for the 2024 plan year is October 30, 2023 through November 10, 2023. Review this website as well as the 2024 Benefits Guide, linked below, to find information on your 2024 benefit offerings. You will go to Workday to complete your 2024 benefits elections:
You are eligible to participate in the Ministry Brands benefit program if you are a full-time employee scheduled to work at least 30 hours per week. Your benefits eligibility date is the first of the month following your date of full-time employment.
If you have a qualifying life event during the year, you can make changes to your benefits within 31 days of the event. The changes you request must be consistent with the status change event.
NOTE: All changes should be requested in Workday (excluding 401k contribution changes). All 401k contribution changes should be made on Charles Schwab participant portal.
Ministry Brands partners with Cigna, one of the nation’s largest insurance companies, to provide access to comprehensive medical coverage. Eligible employees can choose between three health plan options based on the needs of you and your family. All three plans use the same national provider network.
The PPO ($2000 Deductible) plan is a traditional plan, with copays for office visits and prescription drugs. It offers the lowest copay and deductible, but also has the highest associated premium. The PPO ($4000 Deductible) is also a traditional plan that includes copays for office visits and prescriptions, but has slightly higher office visit copays. This plan has a higher deductible than the PPO ($2000 Deductible) plan, and has a lower monthly premium. On both PPO plan options, Cigna's virtual care program is available with $0 copay!
The Choice Fund HDHP with HSA plan is a Qualified High Deductible Health Plan (HDHP). The HDHP is paired with a Health Savings Account (HSA), allowing members to save pre-tax funds to pay for qualified medical, dental and vision expenses. On the HDHP, you pay more of the cost of medical services until you’ve met your deductible, though you will always receive the significant Cigna network discount for all in-network care. Because you are taking on more of the responsibility for your care, premiums are less than the traditional plans. Eligible employees who elect the HDHP will have an HSA opened for them with HSABank. Ministry Brands will deposit up to $1,000 per year for employee-only coverage and $1,500 per year for those employees covering dependents, in equal contributions each pay period, to assist with qualified medical expenses.
Your Cigna medical plans also include prescription drug coverage. Cigna uses a formulary to indicate covered medications and the associated coverage tiers. To search the formulary, please go to http://cigna.com/druglist and select the Standard 3 Tier drug list from the drop-down menu. You can then search for medications or find your medications in the alphabetical list.
Employees who enroll in the Choice Fund HDHP with HSA plan may also elect a Health Savings Account (HSA), administered by HSABank. Ministry Brands will deposit up to $1,000 per year for employee-only coverage and $1,500 per year for those employees covering dependents, in equal contributions each pay period, to assist with qualified medical expenses. Ministry Brands currently partners with Cigna to provide access to comprehensive dental insurance. Beginning in 2024, Ministry Brands is excited to partner with Delta Dental for your vision insurance.
Employees have a choice between base and buy-up options. Both options include benefits for annual cleanings, as well as minor and major restorative procedures. The buy-up plan also includes adult and child orthodontia benefits, and provides annual maximum benefits of $2,000. Both plans:
Delta Dental offers two provider networks to help cover your smile while keeping costs as low as possible. The Delta Dental PPO Network provides maximum cost savings, while the Delta Dental Premier Network provides a safety net for additional access when you need it.
While the dental network includes thousands of participating dentists, the plan has been structured to minimize the possibility of members being balance billed by non-network dentists. For services provided by a non-network dentist, the plan will reimburse the provider based on the 90th percentile of all provider charges in that area. This high level of reimbursement means that even if your dentist does not participate in-network, the dentist is unlikely to bill you any more than you would pay at an in-network dentist.
Ministry Brands partners with EyeMed to provide access to comprehensive vision insurance.
Employees may choose between a Base and Buy-Up plan. Both plans include benefits for annual eye exams, lenses, frames and contact lenses in lieu of lenses and frames. On the Base Plan, most services are subject to a copay, while copays are not required on the Buy-Up Plan. In addition, the Buy-Up Plan allows one to get new frames every year, while the Base Plan allows for new frames once every 24 months.
Vision benefits for out-of-network providers are substantially lower than in-network benefits. For this reason, covered members are strongly encouraged to use a provider who participates in the network. Ministry Brands employees have access to Medical, Limited Purpose, Dependent Care Flexible Spending Accounts.
With a Medical FSA, you can set aside up to $3,200 pre-tax to pay for qualified medical, dental and vision expenses. You can even roll over up to $640 in unused FSA funds to the next plan year! Access your FSA funds at the point of sale using your debit card supplied by Wex.
A Limited Purpose FSA can be paired with a High Deductible Health Plan and HSA, like the Choice Fund HDHP with HSA medical plan option. It works just like a Medical FSA except funds can only be used for qualified dental and vision expenses.
The Dependent Care Flexible Spending Account (DCFSA) is a pre-tax account used to pay for dependent care services such as before or after school programs and child or elder daycare. With the DCFSA, you can contribute up to $5,000 each year – pre-tax – to pay for the care of children up to age 13 or adult elder care.
Ministry Brands partners with The Standard for life insurance and accidental death & dismemberment (AD&D) insurance.
Company-paid basic life and AD&D insurance is a combined benefit available at no cost for employees.
You may also purchase voluntary life and AD&D insurance for yourself and your dependents. Employees may purchase coverage in increments of $10,000 up to a maximum of $500,000, not to exceed 5 times your annual salary. Spouses may be covered up 50% of the employee coverage amount. Child life coverage of $10,000 is also available. Coverage must be elected on the employee to elect coverage on your spouse or children. AD&D provides an additional death benefit equal to the amount of coverage elected in the case of an accidental death.
When you are initially eligible for this coverage, usually as a new employee, you may elect up to $250,000 in coverage on yourself, up to $50,000 on your spouse and $10,000 on dependent children on a guaranteed issue basis. This means no medical underwriting is required and that coverage cannot be denied. Note: these guaranteed issue amounts are ONLY guaranteed upon being a new hire.
Employees may add $20,000 in coverage during open enrollment, and up to $10,000 in coverage on their spouse, without medical underwriting, as long as the employee and/or spouse have not previously been declined or withdrawn their application for Voluntary Life coverage.
Disability insurance provides income protection in the event of a disability. Ministry Brands partners with The Standard for disability insurance.
Employer-Paid Short-Term Disability Insurance
Ministry Brands provides all full-time employees with short-term disability insurance. On the 8th consecutive day of a qualified disability, your disability coverage will provide benefits equal to 60% of your pre-disability salary, not to exceed $1,500 per week, with benefits lasting for up to 12 weeks. This benefit is provided at no cost to you!
Note that disability benefits coordinate and offset with all applicable state-mandated disability pay programs.
Voluntary Long-Term Disability Insurance
Employees may also purchase long-term disability insurance through The Standard. If elected, long-term disability benefits begin after short-term disability benefits have been exhausted.
Employees may choose between two LTD options. Both options have the same 90-day waiting period, but Option 1 provides benefits equal to 60% of pre-disability wages, while Option 2 benefits are 50% of pre-disability wages. Both options have a maximum benefit of $10,000/month. Benefits can last up to age 65 if one remains disabled as defined in the policy. Note that long-term disability benefits will offset against other sources of income.
Ministry Brands offers Accident and Critical Illness illness coverage through Cigna. New in 2024, Ministry Brands will also offer Hospital Indemnity coverage through Cigna. These coverages are low cost and can help offset unexpected expenses related to your health.

Voluntary Hospital Indemnity Insurance pays a specific fixed dollar benefit amount if you have a covered hospitalization resulting from a covered injury or illness, regardless of the amount charged by providers.

Ministry Brands has chosen Charles Schwab as the recordkeeper to the Ministry Brands 401(k) Plan with Alliant Retirement Services, John Cunningham, as the financial advisor.
Many of us know that saving for the future is difficult when everyday life expenses must be paid. The 401(k) plan makes saving easy by helping you budget your savings with your individual contribution being deducted from your paycheck. You elect a percentage (not to exceed IRS annual limits) to be deducted from your paycheck on a pre-tax or after-tax (Roth) basis. Your contributions and any Ministry Brands matching contributions are invested into mutual funds per your election and direction.
The earnings on your account grow tax-deferred until you withdraw money at the time of retirement or another qualifying event. The amount withdrawn is taxable income in the year of the withdrawal (please see additional information provided on how after-tax (Roth) contributions are taxed in the year of withdrawal).
Calendar Year Contribution Limit: $22,500 in 2023; $23,000 in 2024
Calendar Year Catch-up Contribution Limit: $7,500 in 2023 and 2024 (available for participants age 50 or older)
Employer Matching Contribution: 100% of your contribution on the first 1.5% you contribute
You are always 100% vested (ownership) in your contributions and the earnings on your contributions. You gradually earn vesting (ownership) in the employer matching contributions and earnings on the matching contribution. After two years of service with Ministry Brands, you will own 100% of the employer matching contribution account balance.
