Medical, dental, and vision coverage provides you and your family the protection you need for everyday health.

Life & Disability Benefits

Basic life and disability insurance are available and provide financial protection in the event of unforeseen circumstances.

Voluntary Benefits

You have the flexibility to select from a range of benefits to keep you and your family healthy and protected.

Qualifying Events

Employees must notify HR within 30 days of any of the following family status changes:

Marriage/Divorce
Birth/Adoption
Death
Loss of eligibility (e.g., dependent child turns 26, etc.)

Failure to notify HR promptly may void COBRA rights!

Nomi Health

Empowering Abilities has added Nomi Health to your benefit offering. Nomi Health provides you access to thousands of medical services, located nationwide. These services include lab work, imaging, physical therapy, surgery, and more.

Contact a Cared Guide at 855-601-1900 or getcare@nomihealth.com.

1. Doctor Recommends A Specific Service

In order to be a candidate for Nomi, there must be valid medical records with a referral order suggesting the specific service needing (imaging, physical therapy, surgery, etc.)

2. Chat with a Care Guide

Call or email Nomi to speak to one of their Care Guides. Your Care Guide will search for the nearest option for that specific service and determine if there are any out-of-pocket costs.

3. Schedule with Nomi Health

Once a location is determined, someone from that facility will reach out to schedule your appointment. If pre-authorization is required, a Care Guide will step in to get your information where it needs to go.

4. Receive a Voucher

Before your appointment, a Care Guide will provide you with a voucher to present to the receptionist on the day of your visit. This voucher will replace the need for you medical ID card.

Voluntary Term Life Insurance

Life insurance helps protect your family’s finances by providing a cash benefit if you pass away. This ensures they’ll be financially supported and cover essentials, from bills to funeral costs. With life policies, you can get affordable life insurance protection for a set period.

Voluntary Term Life is available in $10,000 increments to a maximum of $300,000.

Spouse and Child Benefits are available.

LTD benefits are an additional benefit offered to you. The plan allows you to receive 60% of your pre-disability earnings.

Additional Benefits

Empowering Abilities offers supplemental benefits through Assurity. These benefits are designed to address financial vulnerabilities that may arise due to injuries, illnesses, surgeries, or pregnancies affecting you or a covered loved one. Assurity provides accident, critical illness, and short-term disability benefits that team members can purchase. All these benefits offer cash payments directly to you, which can be used for various purposes such as deductibles, everyday living expenses, childcare, treatment-related travel expenses, or any other needs you may have.

Critical Illness

Voluntary critical illness insurance provides a fixed, lump- sum benefit upon diagnosis of a critical illness, which can include heart attack, stroke, paralysis and more. These benefits are paid directly to the insured and may be used for any reason, from deductibles and prescriptions to transportation and childcare.

•You decide how to use the benefit funds - for medical and non-medical expenses.
•Multiple coverage options for your spouse and children.

Accident Insurance

Employees can also purchase Accident Insurance. When you, your spouse or child has a covered accident, like a fall from a bicycle that requires medical attention, you can receive cash benefits to help cover the unexpected costs.

Accident Insurance can be used however you want, and it pays in addition to any other coverage you may already have. Benefits are payable directly to you.

Short Term Disability Insurance

To ensure your income will continue if you are unable to work due to a disability, Empowering Abilities offers short-term disability (STD). Benefits are payable for a non-occupational injury or illness that keep you from performing the normal duties of your job. If a medical condition is job-related, it is considered Workers’ Compensation rather than STD.
