Kaiser Foundation Health Plan (KFHP) coverage is available to all benefits-eligible physicians and their eligible spouses/domestic partners and dependent children (up to age 26).
Kaiser Permanente Signature HMO
The HMO plan coverage is comprehensive and includes basic and major medical care at Kaiser Permanente medical facilities, such as hospitalization, surgery, diagnostic imaging, laboratory, and emergency care.
Supplemental Medical Insurance
If you enroll in the HMO plan, you will be automatically enrolled in the Supplemental Medical Insurance plan, which covers certain medical services/procedures that are excluded by the HMO plan, or that exceed plan benefit limits.
MAPMG covers 100% of the cost of coverage.
You may enroll your parents and/or in-laws in a Kaiser Foundation Health Plan, provided they meet all of the requirements below. If your parents/in-laws enroll, you will pay the required premiums through post-tax monthly payroll deductions.

Maintaining good oral hygiene is paramount to your overall health and wellness. That is why MAPMG has partnered with Delta Dental, one of the largest carriers, to provide our physicians and their eligible dependents with excellent dental coverage. MAPMG covers 100% of the cost of coverage.
Regular dental care is an important part of caring for your overall health, and keeping your teeth fit may protect you from major expenses in the future. Use dental coverage through Delta Dental to help pay for the cost of routine checkups and just about any other type of dental work you might need, such as crowns or root canals.

Our no-cost, confidential Employee Assistance Program (EAP) provides up to 5 in-person counseling sessions per occurrence per year for you and your dependents. This program is available 24/7/365.
Just need some guidance? You can also access financial and legal support, assistance with day-to-day challenges, family relationships and so much more. This benefit is confidential and at no cost to you.

For more extensive counseling, you and/or your dependents enrolled in the health plan may choose to obtain care by using your Kaiser HMO benefits, or by using Optum behavioral health benefits by paying the applicable co-payments for in-network care. For out of network care, benefits are paid at 80% of Optum's plan allowance.

A Flexible Spending Account (FSA) is a plan that allows you to budget and save pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) for the incurred plan year. Dollars invested in an FSA are tax-free.
The Health Care FSA covers eligible out-of-pocket healthcare expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent. MAPMG allows you to rollover up to $660 of unused 2025 funds to the new year (2026). Unused funds over the rollover allowance will be forfeited. For 2025, the contribution maximum for the Health Care FSA will be $3,200 (per individual).
Use your Dependent Care FSA to pay for eligible dependent expenses such as care for a child so you and/or your spouse can work, look for work, or attend school full time. You can contribute up to $5,000, or $2,500 if you are married and filing separately. Any unused funds for the plan year will be forfeited.
Another great way to save money through tax-free spending is MAPMG's commuter benefits program. You can use pre-tax dollars to pay for your mass transit expenses as well as for parking at a mass transit lot (e.g. Metro, Marc, VRE). The monthly transit or parking costs must be related to your work commute. It's easy and flexible. The 2025 monthly limits for the Transit Plan and the Parking Plan are $325 respectively.

Plan now for the life you'll want in retirement. MAPMGs 401k plan offers several contribution options and provides an employer contribution.
MAPMG will automatically begin to contribute 5% of your bi-weekly gross wages to your 401(k) account after you’ve completed 1,000 hours of service. There is a 5-year graded vesting schedule for the MAPMG employer contributions.
Excess Contribution Plan
MAPMG’s contributions to the 401(k) Plan are limited by the IRS. However, the Excess Contribution Plan provides you with a lump sum taxable benefit made at the end of the year equal to 5% of your base and incentive pay that exceeded the IRS limit for that year.
2025 Contributions Limits:
MAPMG provides a very generous pension plan to physicians. There are two types of service that affect the Pension Plan:
Vesting Service: time counted to determine if you are eligible for the Pension Plan benefit. To be eligible to receive Pension Plan benefits, you must have at least 5 years of Vesting Service. You earn a year of Vesting Service for each calendar year that you work 1,000 hours. Vesting Service is prorated when a full year is not worked.
Credited Service: time counted to determine the amount of retirement income. You earn a year of Credited Service for each year that you work 2,000 hours. Credited Service is prorated for partially worked calendar years as well as for part time benefits-eligible physicians.
In addition to Credited Service, your Highest Average Compensation (HAC) is used in the benefit calculation. HAC is the average monthly base compensation for the highest 36 consecutive months of the last 120 months as a benefits-eligible physician, up to the annual IRS limit.
The formula to determine the Pension Plan benefit is: 2% of HAC x years Credited Service up to 20 years + 1% of HAC x years of Credited Service over 20 years.
Full and Early Retirement: This program allows Shareholders between the ages of 60-64 with 20+ years of vesting service to retire early and receive a subsidized benefit until the normal retirement age of 65.
Supplemental Retirement Plan: The Supplemental Retirement Plan is a non-qualified plan that provides a lump sum taxable benefit to vested physicians upon separation from service for accrued pension plan benefits in excess of IRS HAC compensation limits.

MAPMG provides $250,000 in Basic Life and AD&D insurance at no cost. This provides peace of mind ensuring your family is protected.
You have the option to purchase supplemental Life and AD&D insurance for yourself and your dependents. Rates are based on age and volume.
If you're not able to work, how will the bills get paid? We make sure you're protected with company-paid Short Term Disability coverage. Physicians receive Short Term Disability benefits paid as salary continuation at 50% of base pay for Shareholder Track physicians for up to 166 days. Short Term Disability benefits increase to 67% of base pay for Shareholders. Benefits commence on the 15th calendar day of disability.
Voluntary Long-Term Disability
Long Term Disability coverage can provide lasting income protection if you remain unable to work. You have the option to purchase coverage. There are two plan options allowing physicians a choice in the level of income protection. Both plan options provide coverage of 60% monthly income up to a $20,000 monthly maximum.
MAPMG provides Business Travel Accident insurance through MetLife for all benefits eligible physicians. This coverage provides accidental death and dismemberment benefits when traveling for business and is paid by MAPMG. In case of accidental death, the benefit amount is $250,000.

Physicians and their immediate family members are eligible to purchase Long Term Care insurance to help reimburse expenses associated with care needed when confined to a nursing home, assisted living facility or even at home. Long Term Care insurance is provided by Unum.
Have access to a wide choice of local law firms to provide you with basic will preparation and consultation at 100%. Other services are eligible at a discount, plus parents are covered for discounted services when you enroll in this prepaid legal insurance plan.
The plan covers you, your spouse, and qualifying dependent children for $18 per month.
MAPMG provides 15 days per year of emergency child or adult/elder back up care and for self-care needs when regular arrangements fall through. You may also exchange back up care days for pet care support during a lapse or breakdown in normal care arrangements.
Registration is required prior to utilizing this benefit. You may register at any time.
Please note, Back-Up Care is intended to provide care for your children and/or adult/elder family members solely while you are scheduled to work and is not intended for care outside of working hours. If you need care outside of your working hours, MAPMG provides access to Sittercity through Bright Horizons so you can find and vet reliable caregivers. Thank you for using the program as intended so MAPMG can continue to provide all employees and their families with subsidized Back-Up Care.

MAPMG supports physicians by providing reimbursement programs for state licenses (including DEA and CDS), maintaining board certification, medical education programs/events to earn CME credits, as well as no cost liability coverage. All programs are administered in accordance with established policies.
Continuing Medical Education (CME): Annual (calendar year) reimbursement benefit of $3,000, which is prorated during your first year of employment. Eligible expenses include but are not limited to dues to professional societies, books, software, and conferences.
Board Certification: MAPMG provides 100% reimbursement for the cost of obtaining and maintaining board certification, including registration and exam fees.
Professional Liability Coverage: Complete professional liability coverage is provided by MAPMG to all physicians at no cost for all professional activities performed on behalf of MAPMG.
State Licenses, DEA, CDS and Hospital Privileges: 100% reimbursement for the cost of obtaining Maryland, D.C. and Virginia state licenses, as well as DEA, CDS and hospital privileges at our participating hospitals.
Local Medical Societies: You may choose to join the Maryland, D.C. or Virginia local medical society upon hire, and MAPMG will automatically pay your annual membership.
