Blue Cross Blue Shield Medical Plans
Vulcan Materials Company is a diverse group of employees and their families with different needs for medical care. That is why Vulcan offers three plans in the Blue Cross Blue Shield network with varying deductibles, coinsurance, and premium costs. Plan options include:

CVS Prescription Drug Benefit
Vulcan Materials Company partners with CVS to provide prescription benefits to those enrolled in the BCBS medical plans. This benefit is provided at no additional cost. The CVS Prescription Drug Benefit is designed to bring you quality pharmacy care that will help you save money.

Kaiser Permanente (California Only)
In addition to the BCBS Plans, California residents also have the option to enroll in the Kaiser HMO plan. If you enroll in the Kaiser medical plan, your prescription benefit is provided directly through Kaiser.

Teladoc
Taking to a doctor has never been easier! Teladoc gives you 27/7/365 access to a doctor through the convenience of phone or video consults. It is an affordable option for quality, non-emergency healthcare.
To be eligible for Teladoc, you must be enrolled in the BCBS medical plans. The copay for Gold and Silver plan members is $10, and for Bronze members is $55.
New to Teladoc! Dermatology visits will now be covered. The Dermatology copay for Gold and Silver plan members is $15 and $85 for Bronze plan members.
*If you are enrolled in the Bronze plan, the copay will reduce to the Gold and Silver plan copay once you have met your deductible.

Delta Dental Plans
Vulcan offers two dental plan options through Delta Dental. If you elect the Delta Dental PPO (preferred provider organization), you can use any dentist or specialist you choose, although you will receive maximum savings if you choose a provider in the PPO network. DeltaCare USA HMO (health maintenance organization) is only available to employees residing in California, Florida, Georgia, and Texas. DeltaCare USA is a closed network of skilled general dentists from the DeltaCAre USA network. Select a general dentist for your primary care and, if necessary, he or she will refer you to a specialist.

Vulcan offers a vision plan that is administered by Vision Service Plan (VSP). You will maximize your benefits when you use a network provider.

Healthcare Flexible Spending Account
A Flexible Spending Account (FSA) allows you to budget and save for qualified medical expenses incurred over the course of your plan year. The FSA can be paired with the BCBS Gold and Silver Plan. Dollars invested in an FSA are tax free, and the entire election amount is available on the first day of the plan year. The FSA account is administered through Optum Financial.
Remember each year during open enrollment you decide and make an election for what your annual FSA contribution will be. The 2022 limit for FSA contributions is $2,750.

Dependent Care FSA
If you elect to contribute to a Dependent Care FSA, you can pay yourself back tax-free for eligible dependent care expenses such as day care, after-school care, or elder care expenses.
Dependent Care costs are refunded through reimbursement from Optum Financial. There is no debit card associated with this account. The current annual contribution is $5,000 or $2,500 if you are married and file your taxes separately from your spouse.

Health Savings Account
If you enroll in the Bronze Plan, you can elect to contribute to a Health Savings Account. Your contributions towards the HSA account are tax-free, grow tax-free, and comes out as tax-free when you use it for qualified medical expenses. Unlike a FSA, you can carry over funds year, and the account is your to keep even if you change jobs, health plans, or retire.
For the 2022 plan year, the Individual HSA contribution maximum is $3,650 and the Family contribution maximum is $7,300.

Basic Life and AD&D – Company Paid
Vulcan provides hourly employees with Basic Company-Paid Life and AD&D insurance coverage of $45,000 at no cost to you. In the event of accidental death, an additional amount of AD&D Insurance, equal to the Basic Life Insurance amount, is paid to the beneficiary. At age 65 and over, the Basic Company-Paid Life Insurance is 50% of $45,000, or $23,000.

Supplemental Life & AD&D Insurance – Employee Paid
Vulcan gives you the opportunity to purchase Supplemental Life Insurance for yourself, your spouse, and dependent children. You may elect to enroll in this coverage in the amount of $25,000, $35,000, or $45,000 for yourself. This benefit would pay in addition to the Basic Life Insurance provided by Vulcan.
- All Supplemental Life Insurance plans include AD&D coverage. This means that in the event of accidental death, the AD&D amount would equal the amount of elected coverage.
- Supplemental Life Insurance is conveniently payroll deducted. The cost of coverage varies based on coverage amount, age, and tobacco status.

Dependent Life & AD&D Insurance – Employee Paid
An employee may elect Employee-Paid Dependent Life Insurance. The amount of insurance is subject to a maximum benefit of $15,000 for a spouse and $10,000 per child ($500 for children under 6 months of age).
In the event of accidental death, an additional amount of AD&D insurance equal to the Dependent Life Insurance amount is paid to the beneficiary. The cost of the coverage for the spouse is based on the age of the spouse, tobacco usage, and the amount of insurance purchased. The cost for children is a flat monthly rate regardless of the number of children or their age and no evidence of good health is required. Late enrollment after the initial 31 days of employment is subject to the provision of satisfactory evidence of good health to the insurance company for the spouse.

Disability Income Protection
Short-Term Disability Plan (STD) – Company Paid
Short-Term Disability Insurance replaces a portion of your income if any injury or illness forces you out of work for an extended period of time. Vulcan provides STD benefits at no cost to you, and enrollment is automatic. If approved by The Hartford, you will receive STD benefits for a maximum of 26 weeks, based on a rolling 12-month period. The eligibility waiting period is 90 days from hire. The plan pays 60% of your pre-disability earnings.
Long-Term Disability Plan Long-Term Disability Plan (LTD) – Employee/Employer shared contribution
LTD insurance protects your finances when your disability continues beyond the period covered by the STD plan. If approved by The Hartford, LTD benefits will continue until normal retirement age under Social Security, or earlier depending on age at the time of disability. Benefit payments begin on the 181st day of disability, and the plan pays 60% of your pre-disability earnings.
The Core LTD/Employer Paid - This Plan provides 50% of your pre-disability earnings and is provided at no cost to you.
The Buy-Up Long-Term Disability (LTD) Insurance/Employee-Paid - The Buy-up LTD Plan provides an additional 10% payment so your LTD total is equal to 60% of your pre-disability earning. The LTD Buy-upPlan is paid by you.

Accident Insurance
Major injuries are painful, but the financial impact of the medical treatment doesn’t have to be. Vulcan now offers Accident Insurance through The Hartford. Accident Insurance pays benefits directly to you if you suffer a covered injury such as a fracture, burn, ligament damage, or concussion. Benefits are paid even if you have other coverage. The benefit amount is calculated based on the type of injury, its severity, and what medical services are required in treatment and recovery. The plan covers a wide variety of injuries and accident-related expenses, including injury treatment, hospitalization, physical therapy, transportation, and emergency room treatment.

Critical Illness Insurance
Medical insurance does not prevent all of the financial strain of a major illness or injury. Supplemental medical benefits like Critical Illness can help cover this out-of-pocket financial exposure for a reasonable cost. Critical Illness Insurance through The Hartford pays a lump sum benefit directly to you if you are diagnosed with a covered illness that meets the plan criteria. Covered illnesses include heart attack, stroke, cancer, major organ failure/transplant, coronary artery disease/bypass graft, and COVID-19. The benefit is paid in addition to any other insurance coverage you may have. For coronary artery disease/bypass graft and COVID-19, the coverage pays 25% of the face amount of the policy once per lifetime.

Hospital Indemnity Insurance
If you are admitted into a hospital, it doesn’t take long for the out-of-pocket costs to add up. Hospital Indemnity Insurance through The Hartford pays benefits directly to you if you are admitted into a hospital for care or childbirth. Benefits are paid even if you have other coverage. You receive a benefit as soon as you are admitted and then an additional benefit based on the number of days you are confined to the hospital. The benefit increases if you are admitted and confined to an intensive care unit. The following are covered: first-day hospital confinement, daily hospital confinement, daily Intensive Care Unit confinement, and hospital observation/short stay.

Employee Assistance Program (EAP)
Vulcan's Employee Assistance Program (EAP) with OPTUM offers confidential support for a full range of services including work-life programs, educational resources, financial wellness, legal consultations, and counseling visits. EAP services are available to all employees and their household members at no additional cost. Counseling Services: Employees and their household members may have up to 8 no-cost counseling sessions per issue, per year. Counseling sessions are available in the face-to-face setting and as virtual visits. WorkLife Services: When you contact OPTUM for WorkLife Services, a WorkLife advisor will conduct a search based on your needs using an extensive database. Adult and eldercare support, child care support, financial and legal issues, and pet services are a few topics WorkLife can assist with. You will get a list of options detailing the services offered, cost, professional credentials, and contact information. There is no charge for referrals and all records are confidential.
Talkspace Online Therapy
Vulcan employees now have the ability to receive online messaging therapy at no cost through the EAP. TalkSpace is an online therapy service that connects users to dedicated, licensed, network EAP providers. Users can send their therapist in-app messaging, voice, or video messages – anytime, anywhere - and it's always confidential and secure.

Diabetes Management
Livongo is available through the Blue Cross Blue Shield medical benefit and offers no-cost tools, services, and support to manage diabetes. You will get Livongo's advanced glucose meter, all the strips and lancets you need, and personalized coaching.

Weight-Loss Program
Real Appeal is a weight-loss program that gives you access to one-on-one coaching as well as great online tools, such as an exclusive website, program guide, food journal, and more. You can speak with a registered dietitian to learn how small changes in your daily food choices and eating habits can make a big difference. This program is available at no cost to Vulcan benefit-eligible employees, spouses, and adult dependents.

Tobacco-Cessation Program
Quit for Life is a tobacco-cessation program available at no cost to Vulcan benefit-eligible employees, spouses, and adult dependents. Participate in phone-based coaching and receive a personalized plan to help you quit. You will also receive up to eight weeks of nicotine patches or gum and quit aids or tools at no cost to you.
If you or your spouse are tobacco users and are enrolling in the medical coverage with Vulcan, you can eliminate the additional tobacco surcharge of $60 (or $120 for both) per month for your medical premium by enrolling and completing four calls with a coach by December 15, 2021.
