WHAT YOU NEED TO KNOW

Enrollment in benefits can be one of the most important decisions you will make this year.

Take time to review your benefits information.

Attend benefit enrollment meetings.

Ask questions when you do not understand.

ELIGIBILITY

Full-time employees working an average of 30 hours per week are eligible to participate.

Your benefits are effective on the 90th day of employment.

Regular full-time employees who work a minimum of 40 hours per week are eligible to enroll in employee life and disability coverage on the first day following 180 days of continuous employment from the date of hire.

The enrollment deadline is the last day of the month before your effective date (unless noted).

Enrollment requests must be received by Human Resources within 30 calendar days following the date of the event.

DEPENDENT ELIGIBILITY

Your eligible dependents include:

Your legal spouse;
Married or Unmarried dependents who are:
Your natural, adopted, legal guardianship and/or stepchildren from birth until the end of the month in which they reach the age of 26, regardless of whether they are eligible for another group health plan as an employee.

CHANGES

Changes to your benefits enrollment can only be made:

At the time of hire
During the annual enrollment period
- July 1 effective date
Due to a Qualifying Life Event

Qualifying Life Events (QLE) include:

Marriage
Divorce / Legal Separation
Birth of a child
Adoption of a child
Loss of other employer-sponsored coverage
New election of other employer-sponsored coverage
