Employees working 30 or more hours per week are eligible for coverage (i.e., are benefit-eligible). Benefits are effective on the first of the month following 60 days of hire. If you are a new hire, you must complete the enrollment process by the day before your coverage effective date.
You may cover yourself and your eligible dependents. Eligible dependents include your spouse and children under age 26.
DMS Union Drivers are not eligible for medical, dental, and vision. Hawaii residents are not eligible to enroll in the medical plans.
Choose Carefully!
Due to IRS regulations, changes to your initial benefit elections can only be made during the annual Benefits Open Enrollment Period each year, unless you experience a Qualifying Life Event during the year. Following are examples of the most common Qualifying Life Events:
• Marriage or divorce
• Birth or adoption of a child
• Child reaching the maximum age limit
• Death of a spouse or child
• You lose coverage under your spouse’s plan
• You gain access to state coverage under Medicaid or CHIP
If you experience a Qualifying Life Event during the year, it is your responsibility to make changes to your elections at that time. You must notify us by sending an email from your personal email account to Centerlinebenefits@trueblue.com within 30 days of the qualifying life event (including newborns). Be prepared to show documentation of the event such as a marriage license, birth certificate, or a divorce decree. If changes are not submitted on time, you must wait until the next Open Enrollment period to make your election changes.

Centerline offers two medical insurance options that give our employees the flexibility to choose the plan that best meets your needs. Depending on where you reside, you may be eligible for one or both of the medical plans.
- Blue Shield CA medical plans - available to eligible employees and their dependents
- Kaiser Permanente medical plan - HMO is available to California and Maryland employees and their dependents; HDHP-HMO is available to California employees and their dependents only

Making the Most of Blue Shield of CA's Medical Benefits
Prescription Drug Coverage

If you enroll in the Blue Shield of CA medical plan, prescription drug benefits are included. These benefits can help you save money on certain medications you take on a regular basis by allowing you to purchase prescriptions by mail order, which offers the convenience of having your medications delivered to your home. Your prescription drug coverage uses a four-tier benefit structure based on a formulary, which is the list of prescription drugs approved by the plan. Your cost varies depending on the health plan you choose and the type of prescription.
Amazon Mail Order Pharmacy
You can have eligible non-specialty, prescription medications delivered to your home by Amazon Pharmacy.
As of January 1, 2025, eligible prescriptions will transfer from CVS Caremark Mail Service Pharmacy to Amazon Pharmacy. You will receive an email notification when your prescription has been received by Amazon Pharmacy. This email has instructions on how to begin home delivery service, if you choose to sign up.

Preferred Provider Organization, Choice Plan (PPO)
This dental plan allows the flexibility to select any dentist in-network or out-of-network, but if you stay in network, you’ll pay less. Dental coverage focuses on preventive and diagnostic procedures in an effort to avoid more expensive services associated with dental disease and surgery. The type of service or procedure received determines the amount the plan will cover for each visit. Each type of service fits into a class of services according to complexity and cost.
Dental Health Maintenance Organization (DHMO) CA Only
These dental plans require you to choose one in-network dentist or dental facility to coordinate all of your oral health needs. If you need to see a specialist, your primary care dentist will refer you.
This plan doesn't have any deductibles or maximums. Instead, when you receive a dental service, you pay a fixed dollar amount for the treatment based on a pre-determined fee schedule between the carrier and your dentist.
Please note the DHMO plan is only available in California.
You are eligible to elect a dental plan if you enroll in medical coverage. DMS Union drivers are not eligible for dental coverage.

Vision Insurance
Vision insurance helps offset the cost of routine eye exams and also helps pay for vision correction eye wear like eyeglasses and contacts that may be prescribed by an eye-care provider. You’re eligible for an eye exam and lenses or contact lenses every 12 months. You're eligible for new frames every 24 months. Out-of-network providers will only offer you an allowance towards your vision services.
EyeMed’s Freedom Pass program allows you to purchase frames at Target Optical or Lenscrafters for $0 out of pocket for any available frame and brand—no matter the original retail price! Brands include Ray-Ban, Oakley, Coach, Vogue, DKNY and Armani Exchange. Be sure to mention the program by name. You must also present a code, which you can obtain via EyeMed’s mobile app or member portal.
You are eligible to elect a vision plan if you enroll in medical coverage. DMS Union drivers are not eligible for vision coverage.

Flexible Spending Accounts (FSA) allow you to save money on a pre-tax basis to pay for qualified medical expenses and/or dependent care expenses throughout the year. The money you put into your FSA is done so on a pre-tax basis. This means you are lowering your taxable income and also not paying taxes when the money is used for qualified expenses.
Health Care FSA
You may contribute up to $3,400 to pay for qualified medical, dental and vision expenses for yourself and eligible family members. Funds in this account can cover all eligible expenses on your tax dependents even if they are not enrolled under your health care plan. Eligible reimbursable expenses include medical and dental plan deductibles and copays (if applicable), orthodontia expenses not covered by your dental plan, prescription drugs, prescription eyeglasses and contact lenses, Lasik eye surgery and much more.
Limited Purpose FSA (for BSCA Value CDHP and Kaiser HDHP-HMO participants)
If you enroll in the HSA plan and open a health savings account, you cannot enroll in the Health Care FSA, since you’re already putting tax-free dollars aside to pay for qualified expenses. You may, however, enroll in the Limited Purpose FSA, which allows you to pay for eligible out-of-pocket dental and vision expenses. You can put up to $3,400 into your Limited Purpose FSA for 2026.
Dependent Care FSA
You may contribute up to $7,500 per plan year to pay for qualified eligible dependent care expenses. Funds in this account are saved on a tax-free basis.
FSAs do have a use-it-or-lose-it provision, so be conservative when electing how much to contribute. You have until May 15 each year to incur claims. You can also use your card during this grace period, and the funds will pull from the previous plan year first, prior to pulling from new funds added since March 1. The runout period allows you to file claims for the prior year by May 31 for reimbursement.*

Important information regarding 1095-C forms – for U.S. Employees and Associates
You should receive a 1095-C if you:
- Worked full-time (an average of 30 hours or more per week) in the calendar year, even if you did not enroll in employer-offered qualifying health coverage.
- Were enrolled in employer-offered coverage, even if you did not work full-time hours.
Not everyone will receive a 1095-C
- If you did not work full-time and did not enroll in coverage, you will not receive a 1095-C from us.
- If you did not reside in the U.S. during the calendar year.
- If you were enrolled in coverage through the Health Insurance Marketplace (i.e. such as through Healthcare.gov, not through your employer), you will receive a 1095-A form from the marketplace.
FAQs
When and how will I get my form?
- If you elected to receive a paper form, 1095-C forms are mailed at the end of January from St. Louis, MO.
- If you elected the paperless option, you can access and download your 1095-C form by logging onto mytaxform.com.
If it has been more than 90 days since you have accessed the site, you may need to reset your password. Do this by clicking "Forgot Password".
For new users: you must opt in by January 15th. The company code is 10657.
Can I access my form in Oracle?
- No, unfortunately we are not able to load them into Oracle at this time.
Why is there a dollar amount in line 15?
- The is the employee contribution amount of the lowest cost coverage offered to you. This does not represent any actual costs or charges to you, as you may have elected a differentplan, or did not enroll at all. The IRS uses this in determining whether the coverage offered to you was considered affordable per IRS guidelines. Benefit deductions appear on your pay stubs. If you were not enrolled in health coverage during the year and want to be sure that you were not charged, you can do so by reviewing your pay stubs or contacting Payroll.
Do I need my 1095-C to file my federal income tax return?
- Please refer to the Instructions for Recipients page included with your 1095-C form for descriptions of the different codes.
What do the different codes mean?
- No. You do not need to wait till you have your form to file your federal tax return. Beginning in 2019, the federal individual mandate no longer applies, which means there is no penalty to employees for not having qualifying health coverage.
NOTE: California, New Jersey, Rhode Island, Massachusetts, and Washington, D.C. have their own individual health insurance mandates requiring qualifying health coverage. If you resided in any of these states or district during the year, please check your state or district's requirements.
