Please read this site carefully. It has a summary of your plan options and helpful tips for getting the most value from your benefit plans. We understand that you may have questions about annual enrollment, and we’ll do our best to help you understand your options and guide you through the process.

 

This site is not your only resource, anytime you have questions about benefits or the enrollment process, you can contact your human resources representative. What to know before you enroll

 

Enrollment is mandatory for all employees through UKG whether you are enrolling for the first time, keeping your benefits the same, making changes or waiving/canceling coverage. 

 

If you are a new hire, a Benefits Specialist from Lockton will contact you via email and phone.  Your enrollment must be completed no later than 30 days after your hire date or qualifying event date.

 

Please note:

  • Once you have completed your enrollment and it is approved, it may take up to 10 days before you receive an ID card in the mail. Simply give your provider your date of birth and/or Social Security number so they can look up your coverage with the respective carrier. 
  • It may take up to two (2) weeks for you to be recognized in the insurance carrier’s system. Please keep this in mind when making doctors’ appointments. If you need to schedule an appointment within two weeks (2) of your effective date, contact HR to expedite your enrollment.

Pre-tax/post-tax benefits:

 

Pre-tax benefits — Any deductions from payroll for medical, dental, vision, FSA and 401(k) will be taken on a pre-tax basis. 

 

Post-tax benefits — Deductions from payroll for voluntary life, critical illness or accident benefits will be taken on a post-tax basis. 

Qualifying life events

 

It is your responsibility to notify human resources within 30 days of the qualifying life event. Failure to do so may result in an inability to change your benefit election(s).

 

Here are some examples of qualifying life events:

  • Birth, legal adoption or placement for adoption.
  • Marriage, divorce or legal separation.
  • Dependent child reaches age 26.
  • Spouse or dependent loses or gains coverage elsewhere. 
  • Death of your spouse or dependent child.
  • Court-ordered change.
  • Spouse or dependent becomes eligible or ineligible for Medicare/Medicaid or your states children’s health insurance program.
  • Change in residence that changes coverage eligibility.
  • Spouse’s open enrollment that occurs at a different time than yours.

Let's talk about eligibility for benefits

 

Regular, full-time associates are eligible for the benefits described in this guide. 

 

When benefits become effective: All benefits in this site are effective through 2024. If you are a new hire, your benefits are effective the first day of the month following 60 days of full-time employment.

 

In addition to enrolling yourself, you may enroll the following eligible dependents* in Cargo Largo’s medical, dental, vision, voluntary life and supplemental/worksite coverage: A spouse to whom you are legally married. A dependent child under age 26, according to the following guidelines:

  • A child who is yours and/or your spouse’s by birth, legal adoption or placement for adoption.
  • A child whom you must provide coverage as a result of a court order.
  • Dependent child medical, dental and vision coverages will terminate at the end of the calendar year following their 26th birthday. Coverage may be extended past the age of 26 for disabled dependents.

 

*You will be required to submit any applicable dependent documentation upon enrolling your dependents in the medical, dental or vision coverage.

 

Newly hired/eligible employees

New hires and newly eligible associates MUST complete enrollment through UltiPro even if choosing to waive coverage.

 

Employees changing status from part-time to full-time

Benefits for associates moving from part-time to full-time go into effect the first of the month following the date the associate becomes full-time or when the associate has satisfied the new hire waiting period (first of the month following 60 days), whichever is greater.

 

Pre-tax benefits: Section 125 
The company’s benefit plans utilize Section 125. This enables you to elect to pay premiums for health, dental, vision and flexible spending account coverage on a pre-tax basis. When you use pre-tax dollars, you will reduce your taxable income and have fewer taxes taken out of your paycheck. 
Pre-tax note: When you pay for your dependent’s benefits on a pre-tax basis, you are certifying that the dependent meets the IRS’ definition of a dependent [IRC 152, 21 (b)(1) and 105 (b)]. Children/spouses do not satisfy the IRS’ definition will result in tax liability to you, such as changing that dependent’s election to a post-tax election or receiving imputed income on your W-2 for the dependent’s coverage that should not have been
