Driver Eligibility Guidelines
To qualify for benefits, a driver must earn an average of $375 per week during the designated Testing Period. Each driver has 30 days to meet this requirement.
Example Timeline:
Driver Hired: February 15, 2024
Testing Period: March 1 – March 31, 2024
The testing period begins on the first of the month following the hire date and spans 90 days, with eligibility assessed monthly.
Administration Period: April 1 – April 30, 2024
This is the window when benefit information is provided and enrollment is completed.
Insurance Effective Date: May 1, 2024
This is the date your insurance coverage officially begins.
If You Don’t Meet the Weekly Average:
If you do not meet the $375/week average, you will continue to be evaluated monthly. During this time, you will remain eligible for Acme’s limited health plan through TrueChoices.
Open Enrollment – November 2024
For 2025 benefit eligibility, we will review your salary data from October 15, 2023, to October 15, 2024. To qualify, your average weekly earnings must be at least $375 over that period.

Protect Your Income with Short-Term Disability Benefits
If you become injured or ill and are unable to work, Short-Term Disability (STD) insurance can help safeguard your current lifestyle. This coverage provides a weekly benefit to help replace lost income - so you can continue to pay your bills and protect your savings during recovery.
Benefit Overview:
STD benefits are paid on a weekly basis.
The amount and duration of benefits are determined by your eligibility class.
Important Note on Pre-Existing Conditions:
Benefits are subject to pre-existing condition exclusions. A Pre-existing Condition refers to any injury or illness for which you received medical treatment, advice, consultation, care, or services - including diagnostic tests - or for which medication was prescribed or taken, during the 3 months prior to the date your coverage begins under the policy.
Mutual of Omaha will not pay benefits for any disability resulting from a pre-existing condition if the disability occurs within the first 6 months of continuous coverage.
Protect Your Income with Long-Term Disability (LTD) Benefits
If an illness or injury prevents you from working for an extended period, Long-Term Disability (LTD) insurance can help protect your financial stability. LTD benefits provide ongoing income replacement, ensuring you continue to receive a portion of your paycheck even when you're unable to work.
Typically, LTD coverage begins after Short-Term Disability benefits end.
Benefit Overview:
LTD pays a monthly benefit.
The amount and duration of benefits are based on your eligibility class.
Important Note for Drivers:
To qualify for Voluntary Long-Term Disability coverage, you must be receiving regular income. Specifically, you must have earned an average of at least $375 per week during the 12 weeks immediately prior to your disability.
Please Note: Pre-Existing Condition Exclusions Apply
A Pre-existing Condition is any injury or illness for which you received medical treatment, advice, consultation, care, or services - including diagnostic testing - or for which medication was prescribed or taken in the 12 months before your coverage began under the policy. Benefits will not be paid for any disability resulting from a pre-existing condition if the disability occurs within the first 12 months of continuous coverage under the policy.

We’re proud to launch a meaningful new initiative for Acme Truck Line employees and their dependents - the Samaritan Fund Program. This program is designed to provide financial relief for those facing serious medical challenges or managing the high cost of critical medications.
Best of all, there is no cost to participate - this support is completely free to you.

How the Program Works
Participation in the Samaritan Fund Program is completely voluntary and based on your unique medical and financial needs.
If you qualify, the program will:
Help you identify a health insurance plan that works for both you and your providers
Connect you with Samaritan Sponsors who can cover your medical expenses, including insurance premiums
Assist with the enrollment process and eliminate out-of-pocket costs associated with medical care
The goal is to provide peace of mind during challenging times, while giving you full control over your healthcare decisions.

Getting your ID card for medical, dental, and vision benefits should be simple - but sometimes things don’t go as planned. Whether your card got lost in the mail or you just never received it, we’re here to help! Here’s how to access or request your medical ID card through UMR:

Visit UMR’s website.
Click the blue Login button at the top right to access your Member Portal. Don’t have an account? Just follow the steps to create one.
Once you're logged in, select "ID Card" from the menu on the left.
You’ll see a few options:

Need it right away? You can print a temporary ID card while you wait for the physical one to arrive by mail.
Dental – Delta Dental
Need access to your dental ID card? Here’s how to get it:
Head over to Delta Dental’s website and log in to your Member Connection account. If you haven’t registered yet, it’s quick and easy to create one.
Once logged in, you can print your ID card or request a mailed copy right from your homepage.
Helpful tip: Most dental offices can verify your coverage without an ID card, so you usually don’t need to bring one to your appointment.
Vision – UnitedHealthcare
Need a copy of your vision ID card? Here's how to find it:
Go to myuhcvision.com.
Log in to your account - or register if you haven’t already.
Click on “Print ID Card.”
Use the drop-down menu to select the family member whose card you want to print.
Click “Get ID Card.”
A document titled “How to Use Your Vision Care Benefits” will open. Scroll to the bottom of the page.
A toolbar will appear - click the printer icon to print your card.
Good to know: You typically don’t need an ID card to see your vision provider, as they can look up your coverage with your basic information.
