Medical Plans
Streamland Media is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. We are offering two medical plans, administerd though Cigna and Kaiser, to fit your budget and needs.
Kaiser HMO Plan
An HMO usually offers lower monthly premiums than other types of insurance plans. They also tend to have lower copays and coinsurance, which helps make them more affordable.
With this plan you are required to choose a Primary Care Provider (PCP) and stay within a network of providers, urgent cares, and hospitals to receive coverage under the plan. You must receive referrals from your PCP before you visit a specialist or are admitted to a hospital, except in a qualifying emergency. There is also no out-of-network coverage under an HMO.
Enrolling in an HMO can be a great option to help minimize your healthcare costs so long as you stay within the network.
Open Access Plus (OAP) Network
OAP plans provide more flexibility when picking a doctor or hospital. These plans offer deductibles and require copays when receiving treatment. If you are enrolled in the OAP plan you can choose to contribute to a tax-advantaged health care flexible spending account to help pay for health care expenses not covered by your plan.
With the OAP medical plan, you have the choice of two benefit tiers: Plan providers (in-network) and non-plan providers (out-of-network). The OAP option offers a larger network of providers and specialists to choose from.
Unlike an HMO, you do not need to designate a Primary Care Provider (PCP) and you do not need to obtain a referral to see a specialist.
See plan details below for more information.
OAP HDHP
The High Deductible Health Plan (HDHP) with a Health Savings Account (HSA) offers lower monthly premiums and higher deductibles, providing a cost-effective option for managing your healthcare expenses.
Enrollees can contribute to an HSA, which is a tax-advantaged account used to pay for qualified medical expenses, with unused funds rolling over year to year. Many preventive services are covered at no cost before meeting the deductible. The plan also offers the flexibility to choose any doctor or hospital, with potential savings for in-network services.

Dental & Vision
As part of the comprehensive health insurance coverage, Streamland Media offers employees the option to select dental and vision coverage for themselves and their dependents.
Dental Insurance
Maintaining good oral hygiene is paramount to your overall health and wellness. That is why Streamland Media has partnered with Cigna, one of the largest carriers, to provide our employees and their families with excellent dental coverage.
Regular dental care is an important part of caring for your overall health, and keeping your teeth fit may protect you from major expenses in the future. Use dental coverage through Cigna to help pay for the cost of routine checkups and just about any other type of dental work you might need, such as crowns or root canals.
If you have an HSA or an FSA, you can use these funds to cover eligible dental costs like your deductible or copay.
Streamland Media offers two dental plans, a PPO and an HMO. The PPO allows you to visit in and out-of-network dentists and has an individual deductible of $50. The HMO provides coverage for in-network dentists only and does not require a deductible. Both plans cover preventive care at 100%.
Vision Insurance
Regular eye examinations can not only determine your need for corrective eyewear, but also may detect general health problems in their earliest stages. It can help identify early signs of diseases that impact your whole body such as high blood pressure, diabetes and high cholesteral.
Whether you have perfect vision or require some type of corrective lenses, preventive eye care is an important part of your overall health. VSP can help you offset the expensive costs of exams, frames, contact lenses, corrective surgery, and more.
If you have an HSA or an FSA, you can use these funds to cover eligible vision-related costs like your deductible, glasses or contact lenses.

Spending Accounts
Streamland Media is providing both an HSA and FSA to help employees budget and save for qualified medical expenses incurred over the course of the plan year.
Flexible Saving Account
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to $3,400 of pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year. Streamland Media partners with HealthEquity for FSAs and HSAs.
Dollars invested in an FSA are tax-free and the entire amount is available on the first day of the plan year. However, any money not used by end of the plan year is forfeited.
An FSA covers eligible out-of-pocket health care expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent.
Dependent Care FSA
A Dependent Care Flexible Spending Account (DCFSA) is a pre-tax benefit account that allows you to set aside money to pay for eligible dependent care expenses. These expenses can include childcare, adult daycare, preschool, summer day camp, and before or after school programs. The purpose of a DCFSA is to help you and your spouse work, look for work, or attend school full-time while still being able to care for your dependents.
The limit is $7,500 for single filers and couples filing jointly, and $3,750 for married couples filing separately. Any contributions that exceed the limit are treated as taxable income. You can't change your contribution amount for the year unless you experience a qualifying event, such as a change in your marital status, employment status, or the number of dependents you have.
Limited Purpose FSA
The Limited Purpose FSA works the exact same way as the healthcare FSA except it can only be used for eligible Dental and Vision expenses. It allows you to set aside pre-tax dollars to pay for eligible Dental and Vision expenses not covered by your dental and vision plans. You decide how much money you want to place in your Limited Purpose FSA based on your estimated dental and vision expenses for the coming plan year.
This amount will be deducted on a pre-tax basis each pay period and contributed to your Limited FSA Account. It is important to note a FSA is use-it or lose-it, so plan carefully when trying to decide if and how much you'd like to contribute.

Health Savings Account
A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you're enrolled in a consumer-driven High Deductible Health Plan (HDHP with HSA), you're eligible to contribute on a pre-tax basis to an HSA. You can use these funds to cover medical, dental and vision expenses.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax-free in retirement.

Life & Disability
While no one likes to think about it, planning for your family’s financial security in the event of sickness, serious injury, or death is one of the greatest gifts you can give your loved ones.
Life Insurance
Basic Life Insurance is provided to all full-time employees at no cost to you. Enrollment in this benefit is automatic.
Salaried employees receive 2x your annual salary up to $1,000,000.
You can purchase additional Life coverage beyond what Streamland Media provides through Guardian Insurance. Additional coverage can be purchased for yourself or your dependents. They guarantee coverage during your initial enrollment period, which means you can't be turned down for coverage based on medical history.
Disability
Short-term disability insurance usually lasts three to six months but benefit payments may not go into effect immediately after the disabling event. The period between the disabling event and the start of benefit payments is known as the elimination period. This period can vary by policy and provider, but for most short-term disability policies it's 14 days.
Long-term disability insurance provides benefit payments over a longer length of time – usually a two-, five- or ten-year period – with the length of coverage depending on your policy. Longer benefits periods often mean higher premiums. In most cases, the elimination period for long-term disability is three to six months, so many enroll in both short- and long-term disability coverage to minimize income loss.
AD&D
Accidental Death and Dismemberment (AD&D) insurance is a valuable benefit that provides financial support in the event of a severe injury or accidental death. This coverage pays a benefit to you or your beneficiaries if you suffer a covered accident that results in the loss of life, limb, sight, speech, or hearing. AD&D insurance is designed to supplement life insurance and disability insurance by covering incidents that occur due to accidents, offering an additional layer of financial protection for you and your loved ones.
This coverage can help you and your family with life-altering events including: medical bills, rehabilitation costs, and other expenses that arise from accidental injuries, as well as provide financial security in the unfortunate event of an accidental death.
