Benefit changes made during open enrollment will become effective January 1. As a new hire, you will be able to enroll 60 days after date of hire and must complete enrollment by the 90th day of employment. New Hire benefits effective date is on the 91st day of employment.

The BCBS Premium Medical Plan, or PPO, provides flexibility to see any doctor, has copays for office visits, and offers a low annual deductible. Once you meet your deductible, this plan offers coinsurance at percentage of your medical costs. That means you’ll pay only a designated copay for most services. However, the premium is the highest, meaning you’ll pay more out of your paycheck each week for this plan than the Choice Medical Plan.
If you are enrolled in the Premium Medical Plan you can choose to contribute to a tax-advantaged health care flexible spending account (FSA) to help pay for health care expenses not covered by your plan.
The PPO plan (Premium Plan) has an embedded deductible, meaning each individual must meet their individual deductible before coinsurance applies. For those who cover dependents, once the family deductible is met, no further individual deductibles must be met. No individuals may contribute more than the individual amount to the family deductible.
The BCBS Choice Medical Plan, or HDHP, has the highest deductible of the two plans. With an HDHP, you can expect to pay a lower premium than with the BCBS Premium Medical Plan with the same flexibility to see any doctor. In other words, your weekly payment to have health insurance will be among the lowest. But if you end up needing a lot of care throughout the year, you’ll end up paying more out of pocket than you might with the other plan.
With the Choice Medical Plan, enrollees can contribute to an HSA, which is a tax-advantaged account used to pay for qualified medical expenses, with unused funds rolling over year to year. Many preventive services are covered at no cost before meeting the deductible. The plan offers the flexibility to choose from many in-network doctors or hospitals.
The HDHP (Choice Plan) has an aggregated deductible, meaning the family deductible amount will include all combined eligible expenses that you and your dependents incur. The family deductible amount may be satisfied by one member or a combination of two or more members covered under your medical plan. The same typically applies for the out-of-pocket maximum. Until the deductible is met, only preventative care is covered.

Telehealth — sometimes called telemedicine or Virtual Care — lets your health care provider care for you without an in-person office visit. It is primarily offered online with internet access on your computer, tablet, or smartphone.
Medical plan participants have access to telemedicine services through MDLIVE and can see a doctor whenever and wherever. With MDLIVE, you can talk to a doctor while you’re at work, at home, or on the go, which leaves you more time that’s not spent driving to or waiting to visit an in-office doctor. Virtual Visits allow you to talk with a board-certified physician over the phone or via video chat. These doctors can diagnose and treat your symptoms, including writing a prescription if necessary. These services are available 24 hours a day, 7 days a week, 365 days a year.

If you are enrolled in one of Upbound's medical plans, you automatically have prescription drug coverage through CVS Caremark. You can count on CVS for a wide range of cost-effective medication, thousands of network pharmacies, and options like home delivery for you and your family. Your prescriptions are subject to your medical deductible and may be paid with available HSA dollars.
After your enrollment is complete, you will receive information from CVS about how to register online at caremark.com. Once registered, you will be able to view preferred drug lists, locate retail pharmacies, view claim information, use mail order prescription services, and the Specialty Pharmacy program. NOTE: Surcharges may apply to all plans if your spouse is eligible for medical coverage elsewhere but enrolls, a $12 surcharge will be applied per week. Tobacco users will pay a higher premium, a $16 surcharge per week, but could be eligible to remove this surcharge with completion of our tobacco cessation program.
If the coworker signifies tobacco use for themselves or their covered spouse for the Medical Plan or Critical Illness, it will apply to both related benefits. (For example, if the coworker elects a tobacco plan for Critical Illness the company would align the medical plan with the tobacco surcharge.)
What is the Transparency in Coverage Rule and how does it affect Upbound Group coworkers?
The Transparency in Coverage Final Rule required insurers and group health plans to publish machine-readable files starting July 1, 2022. The files contain in-network rates and out-of-network allowed amounts. In response to this rule, Upbound has made these files available to our Upbound Group coworkers. This link leads to the machine-readable files that are made available and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you're enrolled in Upbound's Choice Medical Plan, you're eligible to contribute on a pre-tax basis to an HSA. Upbound provides an annual HSA employer contribution of $250 for coworkers and $500 for coworker and families that will be deposited on a quarterly basis You can use these funds to cover medical, dental and vision expenses.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax-free in retirement.
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to $3,750 of pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year. Upbound partners with Health Equity for FSAs.
Dollars invested in an FSA are tax-free and the entire amount is available on the first day of the plan year. However, any money not used by end of the plan year is forfeited. Upbound does allow the IRS maximum rollover amount to be carried forward in the next plan year. To be eligible to roll over your unused FSA, you must elect to contribute to your FSA for the following plan year.
An FSA covers eligible out-of-pocket health care expenses such as medical copays, coinsurance, prescription drugs, dental and vision costs. It's available to you and any eligible dependents including spouse, children and any other person who is a qualified IRS dependent.
If you separate from Upbound, please ensure all claims have been submitted within 90 days of your last day of employment.
A Dependent Care Flexible Spending Account (DCFSA) is a pre-tax benefit account that allows you to set aside money to pay for eligible dependent care expenses. These expenses can include childcare for dependents under the age of 13, adult daycare, preschool, summer day camp, and before or after school programs*. The purpose of a DCFSA is to help you and your spouse work, look for work, or attend school full-time while still being able to care for your dependents.
For 2026, the limit is $7,500 for single individuals and married couples filing jointly or $3,750 for married couples filing separately.

The Limited Purpose Flexible Spending Account is only compatible with the Choice Medical Plan.
The Limited Purpose FSA works the exact same way as the healthcare FSA except it can only be used for eligible Dental and Vision expenses. It allows you to set aside pre-tax dollars to pay for eligible Dental and Vision expenses not covered by your dental and vision plans.
You decide how much money you want to place in your Limited Purpose FSA based on your estimated dental and vision expenses for the coming plan year. This amount will be deducted on a pre-tax basis each pay period and contributed to your Limited FSA Account. It is important to note a FSA is use-it or lose-it, so plan carefully when trying to decide if and how much you'd like to contribute.

Basic Life Insurance and AD&D is provided to all full-time coworkers, spouses and children at no cost to you. Enrollment in this benefit is automatic.
Coworkers receive 1 time your annual earnings, minimum of $40,000 and up to $1,000,000.
You can purchase additional Life coverage beyond what Upbound provides through Reliance. They guarantee coverage* during your initial enrollment period, which means you can't be turned down for coverage based on medical history.

The Reliance Accident and AD&D Insurance Protection Plan can pay for expenses resulting from a covered accident or accidental death and dismemberment not covered by your medical plan.
This benefit can complement your medical and disability insurance to help you pay for unexpected or common costs such as your medical deductible, coinsurance, emergency room visits, physician office visits and hospital charges. This coverage is available to purchase for yourself and dependents.
Accident insurance is an extra layer of protection and an affordable way to supplement the additional expenses associated with your injuries, treatments and services.
While major medical insurance may pay for a good portion of the costs associated with an illness, there are lots of expenses that are just not covered — from deductibles and copays to living expenses. With a critical illness policy, you can rest assured you will be protected from extreme financial burdens in the event of severe illness or injury.
The voluntary critical illness coverage is designed to help offset the financial effects of a catastrophic illness with a lump sum benefit if you are diagnosed with a qualified serious illness.
Critical illness insurance coverage can help pay for expenses such as rent, mortgage or other daily living expenses, as well as health-related expenses not covered by major medical insurance.

Short Term Disability insurance offers financial protection in the event you cannot work due to a debilitating injury or illness.
Benefits begin after a short elimination period of 0 days for accident and 7 days for illness, following your disabling injury or illness. The benefit amount is 60% of your before-tax weekly earnings, up to a weekly maximum benefit of $1,500, and is payable for 26 weeks.

Long Term Disability insurance offers financial protection by paying a cash benefit if you're disabled for an extended time due to an injury or illness. You can be totally or partially disabled during the elimination period.
You must be injured or ill and unable to return to work for 180 days before this benefit begins paying 60% of your earnings to a maximum of $10,000 per month. Depending on the age of disability, this benefit could pay until you reach your Social Security retirement age.

If you are admitted to a hospital, it doesn’t take long for the out-of-pocket costs to add up. Upbound’s Hospital Indemnity Insurance through Reliance pays benefits directly to you if you are admitted into a hospital for care or childbirth. Benefits are paid even if you have other coverage.
You can use the money any way you choose. Covered hospital expenses include admission, confinement, and Intensive Care Unit (ICU) confinement. You can use your payments for health plan deductibles and other costs such as medications, rehabilitation, or transportation. You may also use them for bills and living expenses or growing your savings account. A hospital indemnity claim can be filed once your hospital stay is complete.
