Our open enrollment period takes place November 1 - 30 each year. This is your opportunity to make changes to your benefit elections for the following plan year. During open enrollment, you may:
Enroll or re-enroll
Make changes to your benefits
Add or remove dependents
Enroll in health care and/or dependent care flexible spending account(s)
If you do not plan to make any changes to your current benefits, and are not electing FSA for the following year, then you do not need to complete an enrollment form.

Changes to your benefits can be made outside of Open Enrollment only if you have experienced a documented Qualifying Life Event. Changes must be made within 30 days of the date of the event. Your change must be consistent with your life event/status change. Listed below are some events that qualify for a change in coverage.
Marriage
Divorce or legal separation
Birth or placement for adoption of a child
Ineligibility of a dependent
Loss of other coverage
Change in your employment status or that of your spouse
A court order
Entitlement to Medicare or Medicaid
You must complete your benefits enrollment within 30 days from your date of hire or transfer into a benefits-eligible position. If you do not submit your benefit selections within 30 days, you will not have the ability to enroll in benefits until Open Enrollment or in the case of a qualifying life event.
If you do not plan to make any changes to your current benefits, and are not electing FSA for 2024, then you do not need to complete an enrollment form.

Culver Academies is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. Employees are offered the choice of enrolling in one of two Anthem Blue Cross Blue Shield of Indiana plans: the HRA plan or the HSA plan.
Both plans have:
$3,400 individual deductible + additional $1,600 to out-of-pocket max (in-network)
$6,600 family deductible + additional $3,400 to out-of-pocket max (in-network)
Opportunity to earn up $1,400 individual/$2,800 family in Culver contributions through Culver’s Wellbeing program.

An HSA is a tax free account you can use to pay for current and future medical expenses (even medical expenses during retirement). In 2026, you can contribute $4,400 if you enrolled in Employee Only medical coverage and $8,750 if you enrolled in Family coverage (Family Coverage includes one or more covered dependents). Contributions made to a Health Savings Account (HSA) cannot exceed the HSA limit set by the Internal Revenue Service.
An HSA has triple tax benefits:
The money goes in tax-free
The money grows tax-free
Your withdrawals for qualified medical expenses—including any earnings—are tax-free
You’re eligible to open an HSA if:
You enroll in a high-deductible health plan, such as the plan offered to you at Culver.
Your only coverage is a high-deductible health plan, and you have not signed up for Medicare coverage. (If you’re covered under your spouse’s plan and that plan is not a high deductible or your spouse contributes to a Healthcare FSA, then you are not eligible to contribute to an HSA).
Each time you have a qualified expense, you decide whether to:
Pay out of your pocket and let your HSA grow, earning interest for future eligible expenses (e.g., medical expenses during retirement).
or
Use your HSA to pay for eligible medical expenses, such as your annual deductible, coinsurance and copays. Your HSA can also help pay for vision care, dental care and prescription drugs. (For a complete list of eligible expenses, visit www.irs.gov.)

Money you don’t spend rolls over from year to year. So if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it to pay for eligible healthcare expenses tax-free in retirement.

How does the HRA work?
Your provider submits a claim for payment. For covered services, what happens next depends on the following:
If there are sufficient funds in your HRA, the medical expense will be paid automatically from this health account.
If you’ve used all of the funds in your HRA before you’ve met your annual deductible, you’ll pay out of pocket until you reach the deductible.
Once you’ve satisfied your annual deductible and your traditional health coverage has kicked in, you’ll pay the appropriate coinsurance for covered services, up to your plan’s annual out-of-pocket maximum. If you have met your annual out-of-pocket maximum, the plan will pay 100% of the cost of your covered services, up to the allowed amount. (See your Plan Summary for details.)
After your claim is processed, you’ll receive an Explanation of Benefits. This will show the total cost of the service, the “allowable charge” (the provider’s contracted rate), and any amount you may be responsible for. If you have any out-of-pocket costs, your provider will send you a bill for this amount, which will go toward your deductible and out-of-pocket maximum.
Chronic Condition Care Management is focused on improving the health of individuals with chronic conditions. Early identification and effective treatment of chronic conditions slows the progression of chronic diseases allowing our employees to achieve their best health.
It is essential that you establish a comprehensive plan with your physician and that you adhere to protocols, avoiding gaps in medical care (i.e., skipping medications).

That’s why we have a team of registered nurses, supported by clinical experts, trained to help during these stressful times. They’re called nurse care managers, and they’re your health care advocates. Their goal is to understand your needs from all angles and help you get the best care possible.

Managing an ongoing health condition isn’t easy, and having a little extra help and encouragement can make all the difference. That’s why we offer Concierge Care, a no-cost health and wellness program that provides tools, resources, and support to members and their covered dependents.

When you’re not feeling well you can get the support you need easily using LiveHealth Online. Whether you have a cold, you’re feeling anxious or need help managing your medication, doctors and mental health professionals are right there, ready to help you feel your best. Using LiveHealth Online you can have a video visit with a board-certified doctor, psychiatrist or licensed therapist from your smartphone, tablet or computer from home or anywhere.
Your Anthem plan includes benefits for video visits using LiveHealth Online, so you’ll just pay your share of the costs — usually $59 or less for medical doctor visits, and a 45-minute therapy or psychiatry session usually costs the same as an office mental health visit.
Pelvic health disorders affect 1 in 3 women. While common, issues like bladder leakage, bowel discomfort, bloating, and pelvic pain are not normal. Developed by Sword Health, Bloom is a digital solution designed to improve pelvic health from the comfort and privacy of home. It covers all life stages including pregnancy, postpartum and menopause.
Bloom includes 1:1 guidance from a Pelvic Health Specialist, an intravaginal pod by for use during short, pelvic exercise sessions, and a mobile app to follow your program and progress in real-time. All of Bloom’s Specialists have Doctor of Physical Therapy degrees.

As part of the comprehensive health insurance coverage offered to employees and their families, Culver Academies offers employees the option to elect Dental coverage for themselves and their dependents. Seeing your Dentist regularly and keeping good dental hygiene can help your teeth stay healthy and help you catch problems early.
The Anthem Dental Complete plan covers preventive care services from in-network providers at 100%. These in-network preventive services are not subject to a deductible or copays. You may have costs associated with preventive care services if they are from an out of-network provider. Preventive services under this plan include cleanings twice per calendar year, bitewing x-rays twice per calendar year, and fluoride application once per year for dependents under age 19 among others.
Culver Academies is giving you the option to select vision coverage with Anthem Blue Cross and Blue Shield as part of their commitment to comprehensive healthcare. Regular eye examinations can not only determine your need for corrective eyewear but also may detect general health problems in their earliest stages. Protection for the eyes should be a major concern to everyone. You are eligible for benefits the first day of full time employment.

Marathon Health is an independent provider of employer sponsored health management services and operates primary care and wellness clinics across the country. They provide a primary care clinic staffed by board certified physicians, physician assistants (PA), and nurse practitioners (NP) exclusively to Culver full time employees and spouses carried on a Culver medical plan. Marathon Health also provides acute care services for child dependents age 6 years and older that are covered on a Culver medical plan.
Marathon Health carries more than 150 frequently prescribed medications at no or low cost. Patients with a prescription from an outside provider can present the prescription to a Marathon Health provider during a required, no charge consultation.
Marathon Health requires a consultation with a provider when filling a prescription in order to ensure complete oversight of a patient’s medical treatment. An identical or similar prescription may be issued with the Marathon Health provider’s agreement. Marathon Health does not carry any controlled substances such as narcotics.
The majority of medications and services through Marathon Health are at no cost to you! For the HSA plan there is a $20 fee per visit for ACUTE visits only and $4 medication charge for ACUTE medications. All preventive services and chronic care management are still free.

Culver is excited to partner with Vitality for Wellbeing! Culver Wellbeing is open to all Culver employees and spouses covered under a Culver medical plan. Both the employee and covered spouse must complete a biometric screen and Wellbeing assessment each year. By completing activities aimed at whole-person wellbeing, the employees enrolled in Culver medical plans can earn incentives.
Our wellbeing program has been designed to support you and your families as whole individuals – mind, spirit, and body. You as a WHOLE person come to work every day and we want to recognize and support that in the programs and resources we provide. Now you can stretch your income, reduce your costs and pay less in taxes. How? By using your Flexible Spending Account (FSA), which allows you to use pretax dollars to pay for healthcare and dependent care expenses. Culver partners with PBA for FSAs.
How Does it Work?
The FSA allows you to deposit a set amount of pretax dollars into your account annually.
FSA dollars can be used for “Qualified Medical Expenses” (including dental and vision expenses) and Daycare/Dependent Care costs incurred by you, your spouse and any of your legal dependents.
You do not have to be participating in any of the Culver Academies healthcare plans to utilize the FSA.
The IRS allows participants to carryover up to an IRS determined amount of unused medical FSA funds at the end of the plan year.

If you would like to re-enroll in the FSA you must log in to your PBA account and make elections for the upcoming plan year each year during Open Enrollment.
Annual contribution limits are set by the IRS.
Culver offers two types of Medical Flexible Spending Accounts: Healthcare FSA and Limited Use FSA.
If you have elected an HRA or have healthcare coverage from outside of Culver, you may be eligible for a Healthcare FSA. The Healthcare FSA can be used to pay for eligible medical, prescription, dental, and vision expenses. This is the version of FSA with which most people are familiar.
If you have elected an HSA, you are eligible for a Limited Use FSA. Limited Use FSAs can not be used for medical or prescription expenses. They can only be used to pay for eligible dental and vision expenses.

A Dependent Care Account creates a tax break for dependent care expenses such as: child care or day care expenses for children ages 12 or under or for children of any age who require custodial care that are physically or mentally incapable of self-care.
Additionally, it can be used for custodial adult day care for a disabled spouse or senior citizen dependents such as parents or grandparents who are considered an IRS tax dependent and require someone to come into the home to assist with day-to-day living.
Dependent Care Spending Account
The minimum annual contribution is $200 and the maximum is $7,500 (or $3,750 if married and filing separately).
The caregiver must have a tax ID number to be considered an eligible caregiver.
The FSA dollars can only be used as they are accrued.
Culver offers both a medical plan with a Health Savings Account and the option to enroll in Flexible Spending Accounts.

Culver offers a variety of incentives and resources to help employees as they contemplate financial wellbeing in retirement. Employees who actively participate in Culver’s 403b program and engage in financial education and planning, enter their retirement years with confidence.
TIAA is Culver's 403b retirement partner. A full-time employee receives:
Defined Contribution: Deposits of 7.5% of gross wages into TIAA each pay period.
Match Contribution: For every dollar that an employee contributes, Culver applies Match dollars at $0.50 on the dollar. The maximum Culver Match dollars is 2.5% of gross wages.
Investments available in Culver’s 403b offering are reviewed quarterly and updated if needed. Reviewing your TIAA contributions, balances and investment performance can be done at any point in time by accessing your account at TIAA.org. TIAA offers investment planning, counseling, and education.

Life insurance pays a lump-sum benefit to your beneficiary(ies) to help meet expenses in the event of your death. AD&D Insurance pays a benefit if you die or suffer certain serious injuries as the result of a covered accident. In the case of a covered accidental injury (e.g., loss of sight, loss of a limb), the benefit you receive is a percentage of the total AD&D coverage you elected based on the severity of the accidental injury.
Basic Life and AD&D - Employee is set at 2x your annual earnings up to a maximum of $400,000*.
Basic Life and AD&D - Dependent covers all of your dependents at one low price. Payout is set at $5,000 for a spouse and $2,000 per child dependent under 26 years of age.
*Imputed Income - Under current tax laws, imputed income is the value of your Basic Life insurance that exceeds $50,000 and is subject to federal income, Social Security, and state income taxes, if applicable. This imputed income amount will be included in your paycheck and shown on your W-2 statement.
Voluntary Life and AD&D insurance can help protect your family during difficult times. Culver Academies provides you with the opportunity to purchase additional coverage for you, your spouse, and child(ren).
Voluntary Life and AD&D - Employee: Coverage in increments of $10,000 up to $500,000 or up to 5 times your annual earnings. Evidence of Insurability required for amounts greater than $240,000 (guarantee issue). Election can be increased by a maximum of $10,000 per year during Open Enrollment.
Voluntary Life and AD&D - Spouse: Coverage in increments of $5,000 to a maximum of $150,000, not to exceed 50% of the employee’s elected and approved coverage. Evidence of Insurability required for amounts greater than $50,000 (guarantee issue).
Voluntary Life and AD&D - Child(ren): Coverage available in flat amounts of $2,500, $5,000, and $10,000.
Guaranteed Issue and Evidence of Insurability: Employees and spouses who elect coverage when first eligible can elect up to the Guaranteed Issue (GI) amount without Evidence of Insurability (EOI).
Long-Term Disability is an electable coverage that picks up where Short-Term Disability ends. Long-Term Disability pays at 60% on your normal monthly salary directly from the Hartford.
Short-Term Disability is provided by Culver to all Full-time employees at no cost to the employee. Short-Term Disability can be used to pay for time off related to a qualifying condition experienced by the employee. If approved, Short-Term Disability pays at 100% of your normal salary, on your normal pay schedule, directly through Culver Payroll, for up to 13 weeks.

Employee assistance benefits are available to all full- and part-time employees and their immediate family members living in the household. You may find at times that circumstances interfere with your happiness, relationships, or balance between work and home life. The EAP is there to help you proactively manage these occasions.
The SupportLinc Employee Assistance Program (EAP) is a benefit, separate from your medical insurance, offered by Culver to help you manage life’s daily challenges.
SupportLinc can refer you to professional counselors, services and resources that will help you and your eligible family members resolve a broad range of personal and work-related concerns.
