program, replacing Livongo.
Omada is a virtual care program for people who want to manage diabetes and improve their
overall health and wellbeing.
Omada members get a dedicated health coach and specialist, smart devices, plus a

Preventive care can help you avoid potential serious health conditions and/or obtain early diagnosis and treatment. Hamra Enterprises company health plan provides preventive care services at no cost, without charging co-pays or deductible payments. Please see below for more details.
If you are on the Anthem Medical Plan Preventive/Wellness Exams are Covered at 100%
• Preventive care is equal to one physical exam per year per enrolled member
• Females get an annual well-woman exam covered at 100% in addition to their annual exam
• No deductible expenses apply—the exam is completely no cost to you provided it’s coded as preventive
