A health savings account (HSA) is a tax-favored personal savings account that works with your high deductible health plan (HDHP). You can use it to pay qualified medical expenses such as deductibles, copays, over-the-counter items, dental, and vision care. If you are electing an HSA for the first time, you must complete the Voya enrollment form here. You can also download the Voya app.
Once your account is open, you will receive a bank card which will allow you to pay for qualified medical expenses. If you want checks, you may also request them but there is a nominal fee required.
When arriving for your appointment, provide them with your Anthem insurance card.
After your visit, your claim will be submitted to Anthem for processing.
After the healthcare provider has received notification from Anthem that the claim has been processed, you will receive a billing statement outlining the balance you are responsible to pay.
Use your HSA debit card to pay for these expenses.
When picking up your medication, provide them with your Anthem insurance card.
The pharmacy will run it through their system and provide you with a balance due.
You then use your HSA debit card to pay for these expenses at that time.
Congress recently passed the Coronavirus Aid, Relief, and Economic Security (CARES) Act. One provision within the CARES Act allows individuals to use money from pre-tax accounts (health savings accounts (HSA) and flexible spending arrangements) to purchase over-the-counter drugs. It also allows pre-tax purchase of menstrual products.
You may open and contribute to an HSA if you’re enrolled in the Boyd high deductible health plan (HDHP) and you:
Are not enrolled in a traditional PPO plan through your spouse or other employer-sponsored plan options.
Are not enrolled in a government-sponsored program (Medicare, Medicaid, Tricare, etc.).
Have not received VA benefits within the last three months (unless for a service-related disability).
Are not claimed as a dependent on someone else’s tax return.
Do not have a healthcare FSA; your spouse also cannot have a healthcare FSA through their own employer.

For employees who do not elect the company’s medical insurance, this benefit pays $500 annually ($19.23 per pay period). You can receive this benefit as an FSA contribution or cash.
*You are not eligible to receive this benefit if you are covered on the medical insurance plan of another employee of Boyd.
The healthcare FSA helps you pay IRS-approved medical expenses. You may contribute from $100 up to $2,500 annually. Funds you elect to contribute to the healthcare FSA are available in full on the first day of the plan year. You can only open an FSA if you aren’t enrolled in an HDHP.
The dependent care FSA helps you pay for dependent care. You can contribute from $100 up to $5,000 (or $2,500 if married and filing separately) annually. Your account works like a debit card; you need to accumulate the funds before you can use them.
If you contribute to an FSA, you must carefully determine your annual election amount during the plan year. According to IRS regulations, the money you set aside must be used for expenses incurred during the plan year in which you make the election. Funds must be used by December 31 of the current plan year, and you can submit claims until March 15 of the following year. Any funds left in the account as of March 2027 (from the 2026 plan year) will be forfeited.

Providing economic security for your family is a major consideration in personal financial planning. Boyd provides you with life and accidental death and dismemberment (AD&D) coverage as well as the option to purchase additional coverage at affordable group rates through Lincoln Financial Group.
Boyd pays for the cost of life insurance for all employees. The purpose is to guarantee each active employee term life insurance in the event of death. The company-paid life insurance benefit amounts are:
Hourly Employees: 1.5 x your annual base pay, up to $50,000
Salary Employees: 1.5 x your annual base pay, up to $500,000
Commissioned Employees: 1.5 x your 3-year average salary up to $200,000
Your AD&D coverage equals the amount of your basic employee term life coverage.
Additional amount payable for loss of life because of an accident in a private passenger car while using a seat belt is the amount equal to the lessor of 10% of your amount of insurance or $25,000
Additional amount payable for loss of life because of an accident in a private passenger car equipped with an airbag is the amount equal to the lessor of 5% of your amount of insurance or $5,000
Excludes ATVs
Employee eligibility requirement: You must be actively at work (able to perform all normal duties of your job) and have completed your 30-day benefits eligibility period to be eligible for coverage.
Minimum work hours: You must be working a minimum of 30 hours per week to be eligible.
Coverage payment: You pay 100% of the premium for this coverage through easy payroll deduction.
Dependent eligibility requirement: To be eligible for coverage, your dependents must be an unmarried child who meets the following requirements:
A child from live birth but less than 26 years old;
A child who is 26 or more years old, primarily supported by you and incapable of self-sustaining employment by reason of mental or physical incapacity.
* Eligible employees can elect or increase up to one increment ($10,000) for themselves, up to $200,000, and one increment ($5,000) for their spouse, up to $50,000, without going through underwriting for approval. There is no medical underwriting at any time for voluntary AD&D coverage, so employees can elect coverage up to $500,000 (not to exceed five times their annual salary); spouses can elect 50% of the employee’s amount.
Short Term and Long Term Disability coverage is provided through Lincoln Financial Group. Disability coverage helps protect your income if you are unable to work due to a covered non-work-related injury or illness (work-related injuries or illnesses may fall under workers’ compensation).
Elimination periods and benefit maximums as defined by the policy will apply. Proof of disability must be submitted upon request to maintain eligibility to receive this benefit.
Company-Provided Coverage
Boyd pays the premium for a basic short term disability benefit of 40% of your weekly earnings to a maximum benefit of $550 per week.
Optional Buy-Up Coverage
You are eligible to purchase buy-up coverage. You may elect an additional 20% of coverage based on your weekly earnings to a maximum benefit of $1,000 per week.
Company-Provided Coverage
Boyd pays the premium for a basic benefit of 40% of your weekly earnings to a maximum benefit of $1,153 per week.
Optional Buy-Up Coverage
You are eligible to purchase buy-up coverage. You may elect an additional 20% of coverage based on your weekly earnings to a maximum benefit of $2,307 per week.
Boyd also provides a basic long term disability benefit equal to a portion of your monthly earnings up to a monthly maximum as defined by the policy.

Critical Illness insurance pays a lump sum benefit directly to the covered person, unless otherwise assigned, if they are diagnosed with a covered critical illness, or specified disease condition. This plan can help ease some of your financial worries so that you can stay focused on your health.You choose how to spend or save your benefit. It can be used for expenses such as:*
Paying for childcare or helping around the house
Copays and deductibles
Travel costs to see a specialist
Prescription drug costs
Medical treatment and doctor visits
Health Screening Test Benefit: Examples include (but are not limited to) mammography and certain blood tests, including annual onsite wellness screenings. Welcome to the Boyd team! As a new employee, you’re eligible for coverage after you have completed the benefits eligibility period, generally 30 days. View the 2026 benefits video so you will be prepared to make your benefits elections on your first day as a new hire. If you have a spouse or dependent you would like to enroll, you must fill out the Proof of Relationship form as well as provide a marriage certificate and/or birth certificates as applicable.
You are eligible for benefits on the date you meet the following requirements.
You are a regular, full-time, active employee whose regularly scheduled work week is at least 30 hours.
You perform your duties for your employer at your employer’s business establishment, or other location to which your employer’s business requires you to travel.
Dependents are eligible for benefits if they meet the following requirements.
They are a lawful spouse of a Company employee, provided no legal separation or divorce has been obtained (if spouse is eligible for coverage through their employer, a spousal surcharge will apply to the medical premium).
They are children of a Company employee who are under the age of 26 (this includes stepchildren, legally adopted children, or children under an employee’s legal guardianship).
Dependents are covered until end of month of their 26th birthday.

If you are enrolled in an HSA-eligible medical plan, you can set aside pre-tax dollars to use in retirement. You can use it to pay qualified medical expenses such as deductibles, copays, over-the-counter items, dental, and vision care. If you are age 55 or older, you can contribute an extra $1,000 on top of the annual contribution limit.
Your HSA follows you into retirement. You will use it just as you did before you retired. It’s important to know that you can only contribute to your HSA while you are enrolled in a high deductible health plan. Once you enroll in Medicare, or switch to a different kind of medical plan, you can no longer contribute. You can only spend your HSA funds.

Most people pay the standard Part B premium amount. Original Medicare pays for much, but not all of the cost for health care services and supplies. Medicare Supplement Insurance (Medigap) policies, sold by private companies, can help pay some of the remaining health care costs, such as copayments, coinsurance, and deductibles.
Medicare Advantage (also known as Part C):
An “all in one” alternative to Original Medicare. These “bundled” plans include Part A, Part B, and usually Part D.
Plans may have lower out-of-pocket costs than Original Medicare.
Most plans offer extra benefits that Original Medicare doesn’t cover, such as vision, hearing, and dental services.
Part D (prescription drug coverage):
Helps cover the cost of prescription drugs (including many recommended shots or vaccines)
Run by private insurance companies that follow rules set by Medicare
May help lower your drug costs and help protect against higher costs in the future

First percent contributed will be matched at 100%, next 5% will be matched at 50%.
To receive the full match, you must contribute 6%.
* At its discretion, the company may suspend the company match upon notification.
You are always 100% vested in your own contributions.
After two years of service, you are 100% vested in employer contributions.
