Duration of COBRA Coverage
18 months: Most employees who lose their job or experience a reduction in hours can continue coverage for up to 18 months.

36 months: Coverage can extend to 36 months for spouses and dependent children in the case of the employee's death, divorce, or legal separation, or if the child loses dependent status.

PTO
Eligible Option Care team members start accruing PTO immediately upon hire.
Full-time and part-time team members (0.5 FTE or greater) will be eligible to accrue PTO benefits each pay period.
Accrual Rates
Accrual rates are determined by length of service. Team members will move up to the next accrual rate in the pay period that includes January 1 of the year in which they will achieve the level of service corresponding to the PTO accrual rate. Hourly team members will accrue based on eligible hours in the pay period. Salaried team members will accrue a flat rate each pay period.
FTO
Full-time manager level and above team members are eligible for FTO.
Option Care Health allows for two (2) weeks of paid BTO to team members following the birth of a team member’s child or the placement of a child with a team member in connection with adoption or legal guardianship. BTO can be taken on as needed basis, provided the time off does not exceed 10 days.
Requirements:
To be eligible to use BTO, you must meet the following requirements:
Must be an Option Care Health Team member with a FTE of .75 or greater.
Have worked for the company for at least six (6) months.
Meet one of the following criteria:
have given birth to a child
be the legal spouse or named parent on the child’s birth certificate or
have adopted a child or been placed under legal guardianship (in either case, the child must be under age 18).
Approved BTO may be taken at any time during the 12-month period following their Family Addition.
All part-time and full-time team members, including those eligible for FTO, will receive a Floating Cultural Observance Day to observe a day of cultural significance. Time will be granted at the beginning of the year based on their full-time equivalency status (FTE) as of January 1st. Once the hours have been granted, they will not change even though the team member’s FTE may change during the course of the calendar year. However, if a team member moves to a non-benefited status (per diem), they will forfeit any unused hours unless otherwise required by law.
New team members hired in the current calendar year between January 1 and June 30 will receive a Floating Cultural Observance Day for that calendar year. Team members hired between July 1 and December 31 will not receive a Floating Cultural Observance Day for that calendar year. The floating Diversity Day must be used during the calendar year in which it is granted, or it will be forfeited except in jurisdictions that prohibit forfeiture of floating holidays, the unused days will be paid out within the first few pay periods of the following calendar year at the team member’s straight time rate of pay or base salary.
All Full-time and part-time team members are eligible to receive holiday pay for Company holidays immediately upon hire, in addition to one floating holiday.
Per Diem team members are ineligible for holiday pay.
There are 6 holidays in which eligible team members receive holiday pay:
New Year's Day
Memorial Day
July Fourth
Labor Day
Thanksgiving Day
Christmas Day
If a company holiday falls on a Saturday, it will be observed the Friday before the holiday. If it falls on a Sunday, it will be observed on the Monday after the holiday.

To further our DE&I initiatives and give back to employees as well as the community, we are excited to share a new matching program. Option Care Health has partnered with four national charities that support our DE&I initiatives. We will be matching team member donations to each of these organizations, up to a collective total of $5,000 for each charity.
Dress for Success - Empowers women to achieve economic independence by providing a network of support, professional attire and the development tools to help women thrive in work and in life.
National Inclusion Project - Ensures no child with disabilities has to sit on the sidelines, and that children of all abilities have the chance to play, laugh, and learn together.
Out & Equal - The global convener, thought leader and catalyst actively working to achieve workplaces of equality and belonging - supporting LGBTQ+ employees and leaders.
Thurgood Marshall College Fund (TMCH) - As the nation’s largest organization representing the Black college community, TMCH ensures student success by promoting educational excellence and preparing the next generation of workforce talent through leadership development.
Once you’ve given to one of these charities you need to request a donation match. For instructions on making this request, click here or search ‘Create Request’ and select ‘Employee Matching Gift Program’ in Workday.
Note: You will need to upload a copy of the donation receipt with the request.
A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you are enrolled in one of the HSA plans, you're eligible to contribute on a pre-tax basis to an HSA, administered by HSA Bank.
Option Care Health team members enrolled in the HSA plan will receive a quarterly employer sponsored match to their HSA funding of up to $500 annually for individuals and $1,000 annually for non-single tiers.
The 2025 IRS maximum contribution is $4,300 for an individual or $8,550 for a household and requires an active re-enrollment each year.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax free in retirement.
Questions? Visit www.hsabank.com or Call 800.357.6246
A Healthcare FSA enables you to set aside pre-tax funds from each paycheck to reimburse yourself for eligible out-of-pocket healthcare expenses including copays, deductibles, coinsurance, expenses above the plan’s maximum payment amounts, and certain over-the-counter items that are purchased with a Doctors written prescription. For a complete list of eligible expenses, visit www.irs.gov.
You may incur eligible Healthcare and Dependent Care FSA expenses from January 1, 2025, to December 31, 2025. You must submit all 2025 FSA claims for reimbursement by March 31, 2026. While $640 in unused FSA funds may be rolled from one year to the next, any contributions in excess of $640 that remain in your account from the prior year, will be forfeited. Be certain to carefully calculate your projected expenses and elect an FSA contribution that you will fully utilize over the coming year.
Option Care Health also offers the option to contribute to a Dependent Care FSA and a Commuter Transit Account. The Dependent Care FSA allows you to pay for eligible childcare and eldercare expenses with pre-tax funds. The Commuter Transit Account allows you to set aside pre-tax dollars to reimburse yourself for approved mass transit expenses including train, subway, light-rail, ferry, and bus fares. Please review your 2025 Benefits Guide for additional information.
