Important Notice:
If you are adding a new dependent(s) to your health insurance you must complete the Dependent Data Information form and provide proof of dependency within 30 days after the effective date of coverage (January 1).
The dependent data information form can be found by clicking the “Dependent Data Information Form” button. Forms are due by December 30th and proof of dependency is due by January 31st.

The Choice Plan is an EPO Plan (Exclusive Provider Organization) that uses a Broad Network. You can go to any provider in the network without the need for a referral. This plan does not require you to select a Personal Care Physician (PCP). You are free to receive treatment from any Choice Network provider found on myuhc.com.

The Charter plan is an ACO Plan (Accountable Care Organization) that uses the Kelsey Seybold Network of providers and facilities in and around the Houston area. This plan does not require a referral if you stay within the Kelsey Seybold Network. You are encouraged to select a Kelsey PCP to coordinate your care but you are not required.

The Nexus Plan is an ACO Plan (Accountable Care Organization)that uses a smaller network, the Memorial Hermann and UT Physicians- a preferred network of quality providers who work together to coordinate your care. You are required to select a PCP. However, you are free to see any physician (primary care or specialty care) in the Nexus Open Access Network without a referral.

The City of Pasadena offers Flexible Spending Account (FSA) administered through WEX to help employees budget and save for qualified medical expenses incurred over the course of the plan year.
A Flexible Spending Account (FSA) is a plan that allows you to budget and save up to the IRS max contribution for the year in pre-tax dollars through payroll deductions to pay for qualified expenses (as defined by the IRS) incurred for the plan year.
Dollars invested in an FSA are tax-free and the entire amount is available on the first day of the plan year (January 1). However, any money not used after March 15 of the following year is forfeited.
An FSA covers eligible out-of-pocket healthcare expenses such as medical copays, coinsurance, prescription drugs, and dental and vision costs. It's available to you and any eligible dependents including spouse, children, and any other person who is a qualified IRS dependent.

A Dependent Care Flexible Spending Account (FSA) allows you to use tax-exempt funds to pay for childcare expenses as well as cover care expenses for qualifying dependent adults who live in your home, including a spouse and parents.
The money used to fund your dependent care FSA is pretax—meaning it is taken from your paycheck before taxes are deducted. For whatever amount you contribute to a dependent care FSA, you'll save whatever percentage you would have paid on that money in federal taxes

Basic Life and A&D Insurance
The City provides Basic Life and AD&D insurance to all full-time employees for $25,000 at no cost to the employee. Enrollment in this benefit is automatic.
Voluntary Life and A&D Insurance
Employees can purchase additional Life and AD&D insurance coverage through United HealthCare for themselves up to a maximum of $500,000, not to exceed 5 times annual earnings.
Employees who elect Supplement Life and AD&D for themselves may purchase Spouse Supplements Life and AD&D up to a maximum of $25,000 and $10,000 for each child not to exceed 100% of the employee amount. Be sure your beneficiary information is up-to-date to ensure your wishes are fulfilled.

Unable to work for an extended period of time due to an illness or injury? As an added supplemental benefit the City offers Aflac as a payroll deduction to all full-time employees. Aflac is a cash benefit that is paid to the policyholder directly and not to the doctor or hospital. Policyholders can use the cash to pay their mortgage, car note, or buy groceries.

The City of Pasadena recognizes how important it is to invest in your future. That is why we participate in the Texas Municipal Retirement System (TMRS). TMRS is a governmental defined benefit pension plan that provides retirement, disability and survivor benefits for eligible City of Pasadena employees. Each pay period, 7% of an employee's monthly gross salary is put into a TMRS account and the City matches this amount 2:1. An employee becomes eligible for retirement after 20 years of service in TMRS at any age or 5 years at age 60.

The City Health & Wellness Clinic is open to any employee/retiree or dependent participating in a City medical insurance plan. All services performed within the clinic are free. The clinic offers services similar to a primary-care physician, i.e. lab work, Personal Health Assessments, prescription refills, treatment for colds/flu/allergies, wellness checkups, sports physicals, diabetes management, and other minor illnesses and injuries.
