Team members that take advantage of Option Care Health infusion services will pay a low $5 copay with any PPO plan, or 5% coinsurance after deductible with any HDHP HSA plan.
If you enroll in the $5,000 Deductible HSA plan, you must use BlueHPN doctors and hospitals to receive benefits. There are no out-of-pocket benefits. Therefore, it's important to make sure your hospitals and doctors are part of the BlueHPN network.

Getting sick after hours or on the weekend used to mean a lengthy and costly trip to the emergency room or urgent care center. Instead, see a doctor from the comfort of home with a virtual visit, provided by Blue Cross and Blue Shield of Illinois (BCBSIL) and powered by MDLIVE.
Examples of commonly treated diagnoses include: sinus infection, ear infection, cough, fever, nausea/ vomiting, pink eye, and urinary tract infections. There is no cost to you for this program, unless you are enrolled in the HSA Plan.

Rx Savings Solutions is a program available to BCBSIL members only, participants can choose to opt-in to the program and have their prescription purchases securely shared between Prime Therapeutics (BCBSIL) and Rx Savings Solutions (RxSS). Their comprehensive analytics can identify less expensive drug options when available and then deliver them to you via text message or email. Sometimes it’s as simple as driving down the block to have your prescription filled. Other times it’s asking your physician to switch your prescription or take other action. RxSS can work directly with your doctor as well. Rxss can help to find all the medication options for your conditions, and you and your doctor decide what’s best for your health and budget.

Team Members: If you were enrolled in your employer’s health plan (medical, dental, vision) you are eligible to continue your coverage under COBRA after employment ends.
Spouses and Dependents: Your spouse and dependent children who were covered under your plan while you were employed are also eligible for COBRA continuation coverage if you experience a qualifying event, such as your job loss, a reduction in hours, or a change in family status (e.g., divorce, legal separation, or death).
Duration of COBRA Coverage
18 months: Most employees who lose their job or experience a reduction in hours can continue coverage for up to 18 months.
29 months: If the qualified beneficiary (e.g., the employee or a dependent) is determined to be disabled by the Social Security Administration, the coverage period may be extended to 29 months.
36 months: Coverage can extend to 36 months for spouses and dependent children in the case of the employee's death, divorce, or legal separation, or if the child loses dependent status.
How to Elect COBRA Coverage
You will receive a COBRA election notice detailing your eligibility and coverage options from NueSynergy. You must elect COBRA coverage within 60 days of receiving the notice or from the date you lose your benefits (whichever is later). If you do not elect COBRA coverage within this period, you will lose your right to continue benefits under COBRA.

Short term disability benefits pay depends on your years of service and job classification, and is 100% employer-paid.

PTO
Eligible Option Care team members begin accruing PTO immediately upon hire.
Full-time and part-time team members (0.5 FTE or greater) are eligible to accrue PTO each pay period.
Accrual Rates
Accrual rates are determined by length of service. Team members will advance to the next accrual rate with the pay period they reach the next length of service tier. Hourly team members will accrue based on eligible hours in the pay period.
Hourly team members accrue PTO based on eligible hours worked each pay period.
FTO
Full-time manager-level and above roles, as well as other designated salaried positions, are eligible for FTO. Remaining salaried roles will follow the standard PTO accrual table.
Option Care Health allows for two (2) weeks of paid BTO to team members following the birth of a team member’s child or the placement of a child with a team member in connection with adoption or legal guardianship. BTO can be taken on as needed basis, provided the time off does not exceed 10 days.
Requirements:
To be eligible to use BTO, you must meet the following requirements:
Approved BTO may be taken at any time during the 12-month period following their Family Addition.
All part-time and full-time team members, including those eligible for FTO, will receive a Floating Cultural Observance Day to observe a day of cultural significance. Time will be granted at the beginning of the year based on their full-time equivalency status (FTE) as of January 1st. Once the hours have been granted, they will not change even though the team member’s FTE may change during the course of the calendar year. However, if a team member moves to a non-benefited status (per diem), they will forfeit any unused hours unless otherwise required by law.
New team members hired in the current calendar year between January 1 and June 30 will receive a Floating Cultural Observance Day for that calendar year. Team members hired between July 1 and December 31 will not receive a Floating Cultural Observance Day for that calendar year. The floating Diversity Day must be used during the calendar year in which it is granted, or it will be forfeited except in jurisdictions that prohibit forfeiture of floating holidays, the unused days will be paid out within the first few pay periods of the following calendar year at the team member’s straight time rate of pay or base salary.
All Full-time and part-time team members are eligible to receive holiday pay for Company holidays immediately upon hire, in addition to one floating holiday.
Per Diem team members are ineligible for holiday pay.

If a company holiday falls on a Saturday, it will be observed the Friday before the holiday. If it falls on a Sunday, it will be observed on the Monday after the holiday.

A Health Savings Account (HSA) is a tax-free account that you can use to pay for current and future medical expenses.
If you are enrolled in one of the HSA plans, you're eligible to contribute on a pre-tax basis to an HSA, administered by HSA Bank.
Option Care Health team members enrolled in the HSA plan will receive a quarterly employer sponsored match to their HSA funding of up to $500 annually for individuals and $1,000 annually for non-single tiers.
The 2026 IRS maximum contribution is $4,300 for an individual or $8,550 for a household and requires an active re-enrollment each year.
Money you don't spend rolls over from year to year. So, if you change jobs, switch to another medical plan or even retire, your HSA and the money in it is still yours to keep. You can choose to save it for eligible healthcare expenses tax free in retirement.

A Healthcare FSA enables you to set aside pre-tax funds from each paycheck to reimburse yourself for eligible out-of-pocket healthcare expenses including copays, deductibles, coinsurance, expenses above the plan’s maximum payment amounts, and certain over-the-counter items that are purchased with a Doctors written prescription. You may incur eligible Healthcare and Dependent Care FSA
expenses from January 1, 2026, to December 31, 2026. You must submit all 2026 FSA claims for reimbursement by March 31, 2027. While $680 in unused Healthcare FSA funds may be rolled from one year to the next, any contributions in excess of $680 that remain in your account from the prior year, will be forfeited. Be certain to carefully calculate your projected expenses and elect an FSA contribution that you will fully utilize over the coming year.
Option Care Health also offers the option to contribute to a Dependent Care FSA and a Commuter Transit Account. The Dependent Care FSA allows you to pay for eligible childcare and eldercare expenses with pre-tax funds. The 2026 IRS maximum contributions for the Dependent FSA is $3,750 for an individual or $7,500 for a family and requires an active re-enrollment each year. The Commuter Transit Account allows you to set aside pre-tax dollars to reimburse yourself for approved mass transit expenses including train, subway, light-rail, ferry, and bus fares.
