CALM app for UHC members

Team members currently enrolled in our medical plan, now have access to CALM through their myUHC account or mobile app. Calm Health is included in our health plan and no additional cost.

Short-term and Long-term Disability Coverage (Home Office and Regional Only)

AlerisLife provides income protection benefits to all regional and home-office based team. This includes Short Term Disability (STD) and Long Term Disability (LTD) insurance to help provide income if you are unable to work due to illness or injury. Disability coverage, administered by Cigna, is fully paid by Five Star. You are eligible for benefits on the first of the month on or after 90 days of active service.

You can set aside up to $650 pre-tax total for both transit and parking costs, but keep in mind that the funds are considered two separate buckets of money and cannot be mixed and mingled. PTO Program for Community Team Members

Team members scheduled to work a regular schedule are eligible for Paid Time Off. This program combines traditional vacation, sick time and holidays into one program. Please check with your manager for details on the plans for your community.

Leave of Absence Program

Family Medical Leave (FMLA or non-FMLA) and Personal Leave is available for team members who are out for a medical reason, or who need to take care of a family member or need to take time off for an extended period of time. The leave of absence program is administered by NY Life
