Welcome
New Hires
What You Need To Know
Enrollment in benefits can be one of the most important decisions you will make this year.
Take time to review your benefits information.
Attend benefit enrollment meetings.
Ask questions when you do not understand.
As a New Hire at QCR, here is what you need to know:
-
Retirement options: You may begin contributions upon hire. However, your access is typically granted as part of your first payroll cycle. If you would like access sooner, reach out to me & I can manually create the account for you.
-
Beneficiaries: Please do not forget to designate a beneficiary. It is an important step to protect your investment.
-
Distribution: At end of employment, balances less than $5000 are automatically distributed & charged a $50 distribution fee.
-
-
30 day deadline for retirement elections: IF you do not login to Principal to elect your retirement contributions within your first 30 days, we will automatically defer 6% pre-tax to the 401K & increase that by 1% annually until you reach 15%. IF you want ANY other election, please login to Principal to make the election that works best for you.
-
15 day deadline for New Hire Benefit elections: You have 15 days from your hire date to complete your benefits enrollment. IF you miss the deadline, you will not be eligible to make changes until Open Enrollment in November unless you experience a Qualifying Life Event (QLE) such as marriage, divorce, birth/adoption of a child, etc.
-
Guaranteed Issue opportunities: As a new hire, you have a Special Opportunity for guaranteed issue with no Evidence of Insurability. These coverages may still be available at future enrollments BUT you would then be required to prove that you are healthy enough to receive the coverage for:
-
Long Term Disability
-
Voluntary Life Insurance (Employee up to $200,000, Spouse up to $50,000, & Child(ren) up to $20,000)
-
Voluntary Accidental Death & Dismemberment (ADD) (Employee up to $200,000, Spouse up to $50,000, & Child(ren) up to $20,000)
-
-
-
-
Individual vs family deductibles, Out of Pocket Maximum:
-
Individual is for a plan that covers only the employee
-
Family is for a plan that covers the employee & 1 or more family members.
-
Although it is more likely that multiple family members will contribute to meeting the family deductible, it is possible for one family to meet it alone because there is no individual limit within the family plan.
-
-
-
CALM App!:
-
Company Paid Subscription
-
Once signed up, can add up to 5 dependents (age 16 years or older). Note: must be via a computer, then each can download the app.
Important Notes:
- Your next opportunity to enroll in benefits (without a qualifying life event) is during open enrollment that is typically held in November for benefits that would be effective 1/1 of the new plan year.
-
If you are choosing not to enroll in benefits at this time, you will need to elect to waive coverage when completing your online enrollment in Bswift - accessible by visiting Paylocity.
-
As a reminder, the QCR medical plan no longer partners with UMR for administration. As of January 1, 2026, our third-party administrator is Consociate Health.

Medical
2027 MEDICAL BENEFITS
Administered by Consociate Health.
QCR Holdings is committed to offering a variety of medical benefits that provide generous coverage for employees and their covered dependents.
As a reminder, the QCR medical plan no longer partners with UMR for administration. As of January 1, 2026, our third-party administrator is Consociate Health.
There has been some confusion among providers regarding who the insurance is through, who to verify coverage with, and where to submit claims. We hope this can help provide clarity in the event your providers ask questions.
Verifying Coverage
To verify coverage, your provider should call Consociate Health using the number on the front of your ID card.
The provider will need to drop the last two characters of your member ID number. These two characters indicate whether the cardholder is an eligible plan member - employee, spouse, or dependent.
Our health insurance is NOT through Aetna. If your provider calls Aetna to verify coverage, Aetna will not have a record of you.
The Aetna logo on the card indicates that the plan utilizes the Aetna Signature Administrators to determine in-network/out-of-network providers. If the provider is in-network through the Aetna Signature Administrators network, then they are in-network for our plan. This also means you benefit from the pricing stated in the contract between the provider and Aetna.
Claims Submission
Claims should be submitted to Consociate Health as indicated on the back of the card. Claims should NOT be submitted to Aetna. If your provider submits the claim to Aetna, it will be denied.
For full plan details, please review the plan documents.

The Voluntary Accident Plan through Assurity gives you a cushion to help cover medical expenses and living costs when you get hurt unexpectedly. This insurance will work in addition to other insurance you may have. This policy covers benefits for fractures, dislocations, lacerations, concussions, burns, emergency dental work, eye injuries, and surgical procedures. There is also coverage for x-rays, diagnostic exams, follow-up treatments (including physical, speech and occupational therapy) and for hospitalizations. Additional Assurity Accident Plan details: coverage, rates, exclusions, etc. are available on the benefits portal under My Benefits, Voluntary Products, Plan Documents. |
Assurity Critical Illness |
The Voluntary Critical Illness plan through Assurity policy pays a lump sum benefit to you upon diagnosis of covered conditions to help cover out of pocket expenses and time off work. Some of the covered conditions are Heart Attack, Stroke, Sudden Cardiac Arrest, End Stage Renal (Kidney) Failure, Major Organ Failure, and Coma. Additional Assurity Critical Illness Plan details: coverage, rates, exclusions, etc. are available on the benefits portal under My Benefits, Voluntary Products, Plan Documents.
|
Prosperity Cancer Plan |
The Voluntary Cancer plan through Prosperity pays a cash benefit upon initial diagnosis of a covered cancer, with a variety of other benefits payable throughout cancer treatment. Additional Prosperity Cancer Plan details: coverage, rates, exclusions, etc. are available on the benefits portal under My Benefits, Voluntary Products, Plan Documents. |
MetLife Hospital Indemnity |
Even with medical insurance, a hospital stay can be costly. Plan deductibles, copays, and out-of-network costs can add up fast, not to mention costs outside of medical needs, like your mortgage, childcare expenses, transportation and more.
Hospital indemnity insurance can help with out-of-pocket costs while you’re in the hospital and recovering to help reduce the financial impact while you heal.
This fixed indemnity policy may pay you a limited dollar amount if you’re sick or hospitalized. You’re still responsible for paying the cost of your care.
